Introduction

The New York State Office of Mental Health’s County Capacity and Utilization

Data Book includes inpatient and community-based psychiatric service utilization

and capacity statistics for calendar years 2015-2016, displayed at the statewide,

region, and county levels. The County Data Book also summarizes service

utilization based on per capita rates from the US Census 2015 population

estimates.1 Inpatient service utilization is summarized separately by provider county

of location and by patient county of residence. Community-based service utilization

is summarized by provider county only. Both inpatient and community-based

service capacity and utilization are displayed separately for the adult (18 and older)

and child (under 18) populations, where appropriate.

The data presented come from the following administrative data sources:

CAIRS, CONCERTS, ICR, Medicaid, MHARS, PCS, and SPARCS.2 The data

limitations are as follows:

• A county’s average daily census (ADC) may exceed its capacity. This is for

two reasons: 1) ADC is based on calendar year 2015 information whereas

capacity is based on November 2016 data, and 2) ADC is reported by the

patient’s county of residence while capacity is reported by provider county.

For an examples of this phenomenon, see the adult capacity and ADC

results for Albany and Kings counties in Table 1.

• ADC includes psychiatric inpatient units that closed during calendar year

2015.

• Community-based service utilization is reported by county of program

location, even though program service areas may extend beyond county

borders. For example, a county relatively rich in outpatient services may

serve both its own residents and those of a neighboring county.

• Clinics are not licensed for specific slot capacities, therefore clinic capacity

is estimated using the total number of persons served annually. The

following sources are used to estimate these capacities:

o Medicaid-enrolled adults and children receiving clinic services.

o Medicaid and non-Medicaid adults and children receiving State-

operated clinic services.

o The estimated annual count of non-Medicaid adult and children

served in non-State clinics from the 2015 OMH Patient

Characteristics Survey (PCS).

• Inpatient capacity may be over-reported due to off-line beds resulting from

staffing shortages, construction, or other reasons.

Glossary of Terms Average Daily Census (ADC)

ADC calculation is based on the total length of stay in a hospital for all patients divided by 365.

CAIRS OMH’s Child and Adult Integrated Reporting System (CAIRS) records, facilitates, monitors, and evaluates mental health services provided to children and adults with mental health needs in NYS.

Community-based Services

All outpatient programs, including emergency services, licensed outpatient, housing and support programs. Excludes prison-based forensic units.

CONCERTS Certificate of Need Certification System (CONCERTS) data are collected by the OMH Bureau of Inspection and Certification (BIC) from local service providers that are licensed and/or funded by OMH.

CY Calendar Year.

ICR Institutional Cost Report.

Inpatient Treatment Includes Residential Treatment Facilities (RTFs), state psychiatric center inpatient units, licensed private hospitals or hospitals for the mentally ill, and psychiatric units in general hospitals.

Medicaid Department of Health (DOH) Medicaid Claims Database.

MHARS OMH's Mental Health Automated Recordkeeping System (i.e., EMR) for State-operated Hospitals. Data does not include forensic units, Sex Offender Treatment Programs or Research Programs.

PCS The Patient Characteristic Survey (PCS) is conducted biennially at all OMH-funded or licensed mental health programs. Providers report client-level demographic, clinical, social and diagnostic information for all clients served during the last week in October of the survey year. Statewide and regional annual estimates of clients served are available on the PCS web portal (https://www.omh.ny.gov/omhweb/statistics/pcs-message.htm). Weekly results are included in this report because the estimation results are unreliable with the smaller county-level counts.

RTF Residential Treatment Facility (RTF) for Children and Youth is a 24 hour per day inpatient treatment program which provides intensive treatment services to children and adolescents age 5 - 21 who need longer term treatment than would be provided on an inpatient psychiatric program operated by a general, private mental hospital, or state psychiatric center.

SPARCS DOH's Statewide Planning and Research Cooperative System (SPARCS). This system gathers health care industry data on patient characteristics, diagnoses, treatments and services.
