Why we need these Guidelines

The aim of preceptorship is to welcome and integrate Registered Nurses and Allied Health professionals transitioning from student, new to role, into their new team or place of work (Nursing and Midwifery Council, 2020). NHFT has a duty to ensure that systems and processes are in place to offer adequate support and learning opportunities.

What the Guidelines are trying to do

These guidelines provide detail on how the Preceptorship Programme works so that all employees understand who’s eligible to participate and the roles and responsibilities associated in ensuring adequate support is given for Preceptees and Preceptors alike.

Which stakeholders have been involved in the creation of these Guidelines

Preceptees, Service Managers and Trust Nursing Advisory Committee (TNAC)/ Allied Health Professionals Advisory Committee (AHPAC)

Any required definitions/explanations

Preceptorship Programme

These guidelines will provide information around two preceptorship programmes run by NHFT

1. The Preceptorship Programme for staff making the transition from student to registered healthcare professional or those new to the Trust 2. The Prescribing Preceptorship Programme for Non-Medical Prescribers who are new to role, transferring services, new to the Trust or return after a hiatus of 18 months or more

Preceptorship

‘A period of structured transition for the new practitioner, during which he or she will be supported by a preceptor, to develop their confidence as an autonomous professional, refine skills, values and behaviours and to continue on their journey of lifelong learning.’ (Department of Health 2010)

Preceptor

‘A registered and regulated practitioner who has been given the formal responsibility to support newly qualified/registered practitioner through a period of preceptorship.’ (Department of Health 2010)

Preceptee

For the purposes of this document the term ‘preceptee’ will be used and identified as the following:- Newly registered practitioners who are entering employment in England for the first time

following registration with their professional body Professionals who are new to the Trust and are working within a patient-facing clinical service

including practitioners who are employed by the NHFT Staff Bank Registrants joining a new part of the register

Returning to practice or registrants who have experienced a break from working within a patient facing clinical environment

Overseas prepared practitioners who have satisfied the requirements of, and are registered with their regulatory body

Key duties

Professional Practice Education and Training Team (PPETT): Have a responsibility to offer structured preceptorship programmes to staff that are newly qualified or new to the Trust. Services: For all staff new to the Trust each team/service must contact [email protected] to enrol their new employees onto the preceptorship programme. The Line Manager: Must agree to the release from the clinical area via e-roster for planned sessions to facilitate the Preceptorship process. The Preceptor: Must be registered and occupationally competent, have received the necessary preparation and clinical support to achieve the portfolio requirements associated with the preceptorship portfolio. They should be supported to attend and participate in the sessions during the defined preceptorship period.

Preceptorship Processes

It is mandatory for eligible staff to attend the set programme and undertake a period of preceptorship in accordance with the agreed guidance of their regulatory body and this guidance. It is important qualified registrants working under a staff bank position, have their individual needs assessed and appropriate preceptorship support provided locally.

The Preceptorship programme has been developed to support the organisational development needs of staff working within NHFT. The Preceptorship programme is coproduced in partnership with operational teams in order to facilitate clinical competency development for local service need. Preceptorship is everyone’s business (Department of Health, 2010) therefore the Line Manager must ensure that all members of the clinical team are informed when staff (including bank staff), are undertaking their Preceptorship, to ensure support and guidance is offered to the Preceptee as required. A local Preceptor will be allocated to support the Preceptorship process. Where long term absence occurs, the Preceptorship process will be recommenced upon the staff members return to post. In negotiation with the Preceptee, Preceptor and the Line Manager there will be an opportunity for the staff member to resume their local clinical Preceptorship and re-join the Corporate Preceptorship programme.

1. Corporate Preceptorship Programme

NHFT identifies preceptorship as an important support mechanism for staff from Nursing and Allied Health Professions. Whilst these guidelines primarily focus on all newly registered/qualified Band 4 Nursing Associates and Band 5 professional graduate recruits, some of the principles can be applied to all NHFT staff new to their role. These guidelines should be read and adhered to alongside Professional Regulator Guidance. Course format

This programme is designed to complement individual clinical area specific induction processes. The preceptorship element will constitute part of a portfolio of evidence that can be utilised to meet the requirements of nurse revalidation and allied health regulatory standards (NMC 2018, HCPC 2016). The corporate preceptorship is facilitated by the PPET Team, in partnership with the Learning and Development Team.

Course length

Clinical and corporate preceptorship should be completed within 12 months.

Attendance requirements

Newly qualified and return to practice registrants - must attend all preceptorship sessions. Experienced registrants must attend mandatory sessions and those essential to role Missed sessions can be accessed in the following quarter as it is a rolling programme

Support

Attendance at the Corporate preceptorship is mandatory and requires services to engage and promote the process locally to ensure the preceptee feels supported to undertake the structured programme, the contents is designed to compliment the development of their clinical skills.

All preceptees will be allocated a local preceptor to assess and support them to achieve the required elements for successful completion

Preceptor will meet with preceptee a minimum of once per month to review progress and any difficulties to be shared with the manager

Programme requirements will take the following format:

Clinical competences relevant to the area will be identified locally Additional training requirements to meet service need will be highlighted to the preceptee During NHFT probationary period, local induction/preceptor requirements must be completed The portfolio of evidence will need to be signed off by the preceptor and manager on

completion

Timeframes

To be completed within one year of course commencement

Agreement for extensions resulting from mitigating circumstances will be the responsibility of the supporting manager and must be documented on the evidence sheet

Guidance

Competencies completed before commencement of the programme can be identified on the

evidence sheet and signed off by preceptor/ manager.

The Preceptees Line Manager: should ensure time is allocated for the Preceptor and Preceptee to meet, reflect and monitor progress. The Line Manager should ensure the preceptee has had an initial appraisal and is in receipt of regular clinical and managerial supervision. At the end of the preceptorship period, you will be required to sign the evidence log alongside the Preceptor. Preceptor – this could be the Line Manager or appointed registrant To meet with the preceptee a minimum of once per month to offer support and guidance to

enable completion of the programme. Meetings must follow Trust Guidance and be recorded on ESR.

Development of a contract/initial action plan co-produced with the Preceptee, outlining key objectives, setting goals, assessing competence and provision of constructive feedback during the preceptorship period

Have a clear understanding of regulatory practices, Trust policies and protocols Facilitating on the job learning and reflective practices which reflects role modelling

professional values, attitudes and behaviours Demonstrating appropriate clinical decision making skills, evidence based practice, good time

management and leadership skills Identifying resources available to guide the Preceptee Timely referral of causes for concern to the Manager/Team Leader for management via Trust

processes Sign the preceptee’s portfolio evidence log to confirm completion at the end of the

Preceptorship period

2. Prescribing Preceptorship Programme

In recognition that the initial steps to prescribe as an autonomous practitioner can be daunting, NHFT also offers a specific preceptorship programme of support for newly qualified Non-Medical Prescribers (NMP’s). This support programme provides and highlights a broad overview of the NMP’s responsibilities associated with the NMP role in accordance with the Royal Pharmaceutical Society’s Competency Framework (RPS, 2016). It also contextualises the NMP role within an individual’s scope of practice. For more information on NMP Preceptorship please refer to the PPETT intranet page on the Staff Room. Course format

This virtual programme is designed to support individuals as they embed their academic qualifications, transfer within services, join the trust as a new member of staff or take a hiatus of eighteen months or more. The preceptorship element will constitute part of a portfolio of evidence that can be utilised to meet the requirements of nurse revalidation and allied health regulatory standards (NMC 2018, HCPC 2016). The prescribing preceptorship will be facilitated by the PPET Team. Course length

As guidance NMP preceptees would be expected to complete the programme within 6 months The virtual sessions will last for an hour and provide a generic overview of the role in order to allow the NMP to develop their confidence and autonomy.

Attendance requirements

Newly qualified NMPs NMP’s returning to prescribing role after a break of 18 months or more Experienced NMPs transferring services within the Trust Experienced NMPs new to NHFT

Support

The NMP will be required to discuss their prescribing decisions with an experienced prescriber in their area of clinical practice and demonstrate how this links to the RPS Framework, (2016).

Due to the demands within clinical areas, PPETT will act as the preceptors until the programme is completed

The preceptee will need to complete the prescribing preceptorship portfolio to demonstrate their acquisition of knowledge in relation to the NMP role

Programme requirements will take the following format:

Prescribing decisions identified by the preceptee to be discussed and evaluated with an experienced prescriber in practice, this can include Consultants, Doctors, or fellow NMP’s. These discussions will need to be summarised and captured as evidence within the NMPs portfolio

Attendance at all five virtual sessions is mandatory for all newly qualified NMPs, those who transfer services, are new to the Trust or have a prescribing hiatus of 18 months or more

Successful completion of prescribing preceptorship portfolio Once completion of required criteria has been met the portfolio, will need to be returned to

PPETT and signed off by the NMP Leads Completion of the prescribing preceptorship programme will be recorded on ESR The Trust require any NMP who may have been completed the prescribing preceptorship to

subsequently repeat the programme should they change services or taking a break of 18 months or more from the NMP role so that the Trust have the quality assurance that NMPs are prescribing in accordance with the RPS Framework

Additional individual preceptee requirements identified following the completion of the preceptorship programme will need to be identified within the appraisal process

Non-attendance, completion of course requirements or issues of unprofessional behaviour during the programme will be referred to service manager

Timeframes

The Preceptees Line Manager: Should allow adequate time for the Preceptor and Preceptee to work and meet to reflect and monitor progress. A minimum of 1 hour per month should be allocated for the Preceptee to participate in the

Prescribing Preceptorship Programme The Preceptee should be allocated time to discuss their prescribing decisions with an

experienced prescriber in their area of clinical practice PPETT will ensure the signing of the evidence log with the Preceptee is undertaken at the end

of the Preceptorship period Should ensure the Preceptee records their prescribing role within their annual appraisal Experienced Prescriber Guidance – this may be their usual Line Manager or an additional person

Evidence logs to be completed and signed following discussions Encourage the NMP preceptee to think about their prescribing decisions and advance their

learning Additional guidance will be available from the NMP Leads if required Timely referral of causes for concern to the Manager/Team Leader for management via Trust

processes

The Preceptee: Will need to participate in the following:

Undertake all five sessions of the Prescribing Preceptorship Programme and record as Prescribing Supervision on ESR

Demonstrate appropriate clinical decision making skills and evidence based practice around the RPS framework (2016)

Identify resources available to guide the Preceptee Have a clear understanding of regulatory practices, Trust policies and protocols Complete the portfolio evidence log In accordance with Trust protocol allow the preceptor the opportunity to attend regular

prescribing clinical supervision and continuing professional development sessions – a minimum of 4 a year

Training requirements associated with these Guidelines

1. Preceptorship Programme

Preceptees are to attend 7 Mandatory Sessions over the 12 month period – day 1 will be available on a monthly basis and should be completed initially and the remaining 6 sessions will run 4 times per year (quarterly).

AHP preceptee’s are only required to attend 5 days. Clinical Decision Making optional

Supporting Student Nurses – The Clinical Supervisor Role – Nurses only A list of the current Preceptorship corporate training days and dates can be accessed on the Staff Room. Some sessions may be run virtually in order to enable compliance to social distancing guidance.

2. Prescribing Preceptorship Programme NMP’s who have qualified from September 2019 will need to complete the Trust Virtual Prescribing Preceptorship Programme which will run over 6 months Any NMP transferring services, new to the Trust or returning to the prescribing role after a break of 18 months or more must undertake the prescribing preceptorship in order to demonstrate their ability to work in accordance with the RPS framework (2016). Where a lack of compliance is found, the identified group, committee or individual will identify required actions, allocate responsible leads, target completion dates and ensure an assurance report is represented showing how any gaps have been addressed.

Equality considerations

The Trust has a duty under the Equality Act and the Public Sector Equality Duty to assess the impact
