Bellwoods Milestones
1957• Kingston Road: Pioneered housing renovated for accessibility and services for six
women with cerebral palsy
• Bellwoods Organization: Began as the Adult Cerebral Palsy Institute of Metro Toronto
1967 • Bellwoods Park House: Housing construction and service start for 61 adults with physical disabilities
1983• Bellwoods Park House: Renovation to 32 apartments for independent living services
and transitional living program
• Cooperative Living Project: Shared living services and program support start-up
1985 • Attendant Outreach Services: Community-based services start-up
1989 • Mimico Apartment Project: Integrated independent living services start-up
1990 • Name change: Organization’s name was changed to Bellwoods Centres for Community Living Inc.
1996 • MILE Program: Community-based transitional program start-up
2007 • Accreditation Canada: First 3-year accreditation award

• Provides pre-booked support services for 75+ people in the community
• Services are provided: - in client’s home- place of employment - place where clients pursue a degree/diploma/certificate
• Service available 6 am to 12 midnight daily

• Pre-booked and ad hoc support services for clients in 90+ accessible apartments
• Services are provided in client’s home in dedicated and integrated apartment sites
• 24-7 access to support services

To qualify, the applicant must be working towards
obtaining a degree/diploma/certificate
AOS - continuedAOS provides pre-booked personal support services for people with physical disabilities living in the community. These services can be provided in the client’s home, place of competitive employment, and/or place where clients are pursuing a degree/diploma/certificate. Services are available throughout the Toronto area, between the hours of 6:00 a.m. and 12:00 midnight, on a pre-scheduled visitation basis.
Assistance in Pursuing Competitive Employment
Workplace support can be provided in the home or at work through assistance with:
• Personal hygiene
• Dressing
• Lunch preparation
• Packing up items required for work day
• Washroom assistance
• Setting-up the computer
• Meal assistance
• Organizing the daily work space
Assistance in Pursuing Formal EducationAssistance to pursue a degree/certificate/diploma
can take place at home or education facilities, including:
• Personal hygiene
• Dressing
• Lunch preparation
• Packing up items required for school
• Washroom assistance
• Escorting from classroom to classroom
• Meal assistance
• Setting-up in the classroom

Transitional and Educational ProgramsIndependent Living Programs maximize independence and quality of life and cover areas related to an individual’s well-being, such as home safety, communication skills, mobility and seating, and health and wellness. Program co-ordination support is provided by working with clients to support life-skills related to goals and to co-ordinate access to relevant resources in the system.

• Home and community skill development in client homes or in community
• The focus of the program is personal safety and the prevention of hospitalization
Community Connect Program
• At Bellwoods Park House
• To assist hospital patients or those living in Long Term Care Homes (LTCHs) to transition to the community in a safe environment (time-limited)
Housing Hubs
• At Sudbury Street and Deauville Place
• To assist individuals in the community, hospital patients, or those living in (LTCHs) to live in a safe environment within the community (permanent)
What is Bellwoods’ MILE Program?Facilitators and Independent Living Educators (ILEs) assist clients to achieve optimal safety and independence in their home through skill development and community linkage. Assessment and skill development is provided with focus on the following areas:
Home safety Accessibility Community orientation Mobility and seating Self-care skills Household management skills
Communication skills Health and wellness Housing, funding and linkage to community
resources Transition from hospital or Long Term Care Home
(LTCH) to a community destination
MILE services are based on identified objectives and are time limited.
Who can access Bellwoods’ MILE Program?Our Community Cluster Programs include access to the MILE Program which focuses on home and community skill development to maximize safety and independent living in the community through the provision of education and support for client-established objectives.
