Foreword The Ministry of Public Health (MoPH) of the Islamic Republic of Afghanistan is pleased to present this first edition of the Pharmaceutical Services Directory (PSD). The PSD includes information about stakeholders’ activities, coverage areas, and the number and types of health facilities they support. The availability of such information will help improve coordination of pharmaceutical activities throughout the public pharmaceutical sector in Afghanistan. For several years, the MoPH has worked with multiple stakeholders to provide essential medicines for the Basic Package of Health Services (BPHS) and Essential Package of Hospital Services (EPHS). These partners are involved with pharmaceutical supply and distribution, and include donors such as the US Agency for European Union (EU), International World Bank, and Development (USAID), and a number of national and international nongovernmental organizations (NGOs). Although their collective efforts guarantee reasonably well the availability of essential medicines in the public sector, pharmaceutical supply operations among partners are highly fragmented. To address this issue, the CPDS was created in 2010 to establish coordinated procurement and distribution supply system in pharmaceutical public sector of Afghanistan, and to facilitate and harmonize the roles and responsibilities of partners involved in pharmaceutical procurement and distribution activities. In response to the felt need for comprehensive and accurate information about the stakeholders of pharmaceutical sector, the CPDS collaborated with the MoPH/General Directorate of Pharmaceutical Affairs (GDPA) to compile this directory. For this reason, a taskforce of thirteen experts from different organizations and donors was established with coordination and support of CPDS. The MOPH wishes to express its heartfelt gratitude to the taskforce for its commitment and dedication in initiating and successfully ending the development of the PSD. We would also like to express our sincerest appreciation to the Strengthening Pharmaceutical Systems (SPS) program of Management Sciences for Health (MSH) for their additional technical support, the United States Agency for International Development (USAID) for their financial support.

The MoPH is responsible for ensuring access to safe, effective, and quality essential medicines for the country’s public sector. The MoPH does this in close partnership with national and international partners.

Background Many stakeholders are involved in health care service delivery in Afghanistan, including donors such as US Agency for International Development (USAID), European Union (EU), World Bank, and a number of national and international nongovernmental organizations (NGOs). They cover a wide range of health care services including procurement, supply, and distribution of essential medicines and other pharmaceuticals for the Basic Package of Health Services (BPHS) and Essential Package of Hospital Services (EPHS). Very little information is available about the roles, responsibilities, coverage areas, contacts, mechanisms, and expertise of these stakeholders regarding the procurement and distribution of pharmaceutical products. Additionally, the information that is available is incomplete and poorly documented, in part because it is not always shared with Afghanistan’s Ministry of Public Health (MoPH) and other stakeholders. Stakeholder communication is necessary for better coordination and control of pharmaceutical supply management (PSM) activities. For that reason, the MoPH’s Coordinated Procurement and Distribution System (CPDS), in objective 1-1 of its strategic roadmap, committed to strengthen leadership, management, and governance of the MoPH and its General Directorate of Pharmaceutical Affairs (GDPA), with the goal of better coordinating stakeholder activities. To this end, the CPDS—with the support of its stakeholders—assigned a taskforce to capture critical information on stakeholders involved in PSM, and to aggregate that information into this Pharmaceutical Services Directory (PSD).

TECHNICAL AREAS: DEFINITIONS
Clinical Services Participate in or provide support to the activities that provide preventive or curative services to the public, such as vaccination, diagnosis, clinical consultation, patient counseling, medical or surgical treatments, physiotherapies, etc.
Health Services Provide any service (i.e., not limited to medical or clinical services) that aimed at contributing to improved health or to the diagnosis, treatment, and rehabilitation of sick people (modified from WHO definition).
Health Systems Strengthening Participate in the process of identifying and implementing changes in policy and practice in the health system; or any array of initiatives and strategies that improves one or more of the functions of the health system and that leads to better health through improvements in access, coverage, quality, or efficiency (modified from WHO definition).
Pharmaceutical Capacity Building Provide management or technical support to the activities for developing or improving competency and skills of the personnel who manage pharmaceutical affairs and supplies, or perform pharmaceutical services.
Pharmaceutical Quality Assurance Participate in or provide support to the activities required to ensure that the medicine that reaches patients is safe, effective, and acceptable to the patient. Pharmaceutical quality assurance is the totality of the arrangements made with the object of ensuring that pharmaceutical products are of the quality required for their intended use (adapted from WHO definition).
Pharmaceutical System Strengthening Participate in the process of identifying and implementing changes in policy and practice in the pharmaceutical system; or any array of initiatives and strategies that improves one or more of the functions of the pharmaceutical system and that leads to better health through improvements in access, quality, or efficiency (modified from health systems strengthening definition above).

PHARMACEUTICAL SERVICES: DEFINITIONS
Dispensing of Pharmaceuticals Participate in or provide support to the activities for preparing, packaging, and labeling of medicines according to prescriptions and giving instructions to patients on the use of medicines, or counseling patients about prescribed medicines.
Distribution of Pharmaceuticals Participate in or provide support to the activities for delivering pharmaceuticals from the suppliers to the warehouses or health facilities. It encompasses all transportation, storage, and inventory procedures that affect supplies as they move from central points to users.
Evaluation A selective exercise that attempts to systematically and objectively assess progress towards and the achievement of an outcome.
Monitoring An ongoing function that aims primarily to provide the management and main stakeholders of an ongoing intervention with early indications of progress in the achievement of results.
Monitoring & Evaluation Participate in or provide support to the processes of monitoring a program and evaluating its impact on the target population, to assess successes and gaps in program implementation.
Pharmaceutical Capacity Building Provide management or technical support to the activities for developing or improving competency and skills of the personnel that manage pharmaceutical affairs and supplies, or perform pharmaceutical services. The activities may include developing guidelines or training materials, facilitating classroom trainings, providing on-the-job trainings, etc.
Procurement of Pharmaceuticals Participate in or provide support to the activities for acquiring pharmaceuticals through ordering or purchasing from domestic or international suppliers.

Quantification of Pharmaceuticals Participate in or provide support to estimating and forecasting the need of the pharmaceuticals for procurement or distribution through quantification methodologies by using health, demographic, and/or supply chain logistics data or information.
Regulating Pharmaceuticals and Pharmaceutical Establishments Implementing the rules and laws that regulate the supply and use of the pharmaceuticals, such as license issuing, inspection, quality control, surveillance related to medicine safety, etc.
Storage of Pharmaceuticals Participate in or provide support to the activities for keeping pharmaceuticals stored in warehouses or stores, and for managing the conditions of the warehouses and stores that meet the requirements of good storage practices.

Central Medical Store – CMS
Vision: Ensuring access to essential medicines, medical equipment and bed instruments, and other materials required by health facilities. CMS seeks to make these available to the people of Afghanistan in an equitable and sustainable manner.
Mission: Distribution of essential medicines, medical equipment and bed instruments and other required of health facilities in right time, to improve the health and nutrition status of people of Afghanistan in an equitable and sustainable manner.
