CONTRACT (FOR CONSULTANCy SERVICES) This Contract is made and executed by and between: The GOVERNMENT SERVICE INSURANCE SYSTEM, a social insurance institution created under Commonwealth Act No. 18., as amended, and operating under its present Charter, Republic Act No. 82 1, otherwise known as the GSIS Act of 1997, with principal office address at t e GSIS Headquarters Building, Financial Center Area, Pasay City, Philippines, erein represented by its Vice President, MARGIE A. 4. The Policies and Procedural Guidelines on the H ring of Consultants, approved under Board Resolution No. 94 on May 5, 2004, allows the hiring of consultants if the expertise required is no available within the unit where they will be assigned.

5. The CONSULTANT took up Bachelor of Arts i Psychology at the University of the Philippines in Diliman, Quezon Ci y. She is a licensed Coach facilitator for the Coaching Clinic Progr. of Coach U, an accredited program with the International Coa hing Federation; a regional coach-facilitator for L'Oreal Paris; a certifie. trainer professional with the Institute of Learning and Development an. the Asian Regional Training and Development Organization; and consu ant to the Philippine Australian Human Resource Development Facility. She is affiliated with the Organization Development Professionals' Net ork and Philippine Society for Training and Development. She is presrntly the president of i-CLD Consulting Inc., which she also founde・ in August 2007. Specializing in organizational and executive coachin:, strategic planning, management and leadership development, , d integrity-based facilitation, among others, she has conducted workshops and group discussions with clients from all sectors both local - d abroad, including multinational companies, educational institutions such as the Local Government Academy, and international bodies, such as the United Nations (UN) and the UN Children Fund (UNICEF).

6. The President and General Manager approved the hiring of the CONSULTANT from May 15, 2015 to October 31, 015.

NOW, THEREFORE, for and in consideration of the foreg.ing premises and in the interest of the service, the GSIS hereby engages the services of the CONSULTANT, and the latter accepts, subject to the i ollowing terms and conditions:

1. TERM OF THE CONTRACT. This Contract shall ・e for a period from May 15, 2015 to October 31, 2015, unless soon-r terminated by the GSIS on any ground; provided, however, that the G、 IS shall serve a prior written notice to the CONSULTANT when pretermin .: ting this Contract.

2. COMPENSATION. For this engagement, the CONS LTANT shall receive a compensation for the project in the amount o THREE HUNDRED SIXTY THOUSAND PESOS (P360,000.00), Philippi e currency, payable on a per-output basis. The breakdown of payme corresponds to the following deliverables of the CONSULTANT:

2.1 Submission of SURVEY methodology, survey instrument, and sampling frame P 80,000.00

2.2 Training of enumerators and FGD facilitators; Submission of the SURVEY Guidebook and FGD Workshop Manual and Guidebook

2.3 Submission of evaluation report on the SURVEY administration and FGD facilitation

2.4 Submission and presentation of the complete SURVEY results to the CCO, Corporate Services Group, Office of the President and General Manager, and Board of Trustees

Moreover, the Consultant may utilize on a consumable and reimbursah1e basis, as the CCO deems needed, a total reserved budget in the amount of

3. DUTIES. The CONSULTANT, in accordance with th『 Terms of Reference, attached as Annex' A' and made an integrai part nereoi, snau penorm the following services:

3.1 Submit to the CCO the SURVEY methodology and instrument in the form of a questionnaire;

3.2 Guide the CCO in systematically identifying the survey "universe" or population and in determining the final respondents, eventually coming up with and submitting the appropriate "sampling frame;"

3.3 Train all CFS enumerators from the HO and BOs who will administer the survey questionnaire, providing them a CFS guidebook;

3.4 Train all FGD facilitators from the HO and BOs, providing them a monitoring and evaluation system in conducting FGD, as well as workshop manuals and guidebooks;

3.5 Observe the implementation of the SURVEY in selected areas in the HO and BOs as deemed needed by, and in coordination with, the CCO;

3.6 Provide advisory services to the CCO in all phases of the project; and

3.7 Interpret gathered data, and submit and present the complete report on the SURVEY results. including the CSR

The GSIS shall possess, own, and hold exclusive right to all CONSULTANT's outputs pertaining to the SURVEY.

4. UNDERTAKING. The CONSULTANT agrees to perform her duties and responsibilities to the best of her ability and skill, wth utmost care at all times, and with the highest interest of the GSIS in mind.
