General Form of Employment ContractTIDAL WAVES DJ ServicesAgreement made this _____________day of _________________2010, between TIDAL WAVES DJ Services, a Truro company having principal place of business at 116 Hillcrest Ave, Bible Hill NS and ______________________________ residing at #___________________________ of __________________________ , province of _________________________ , hereinafter called the Employer, WITNESSETH:

WHEREAS, the company is about to enter into an agreement with one _______________________________ , a true copy of which is hereto annexed and made part of this contract of guarantee, and

WHEREAS, the company is willing to enter into such an agreement with _______________________________ , only if the guarantor guarantees the faithful performance of all the conditions thereof.

NOW, THEREFOR, in consideration of the sum of $__________ paid by the individual or the company to the Guarantor, the receipt whereof is hereby acknowledged and the execution of such contract by the Company with the said __________________________________ , the Guarantor does hereby agree as follows:

1. The Employer hereby employs the Employee for Disk Jockey services at # _______________ street, in the city of _____________________ , Province of ______________________ with such powers and duties in that connection as may be fixed with the Employer.

2. For services rendered by the Employee as hereinbefore required, the Employer shall pay to the Employee a salary of $_____________ , the entire balance due on or before the day in which the service is performed.

3. The term of employment shall be ________ hours in length.

4. During the term of this agreement the Employee shall devote his best efforts and his entire time to advance interests of the Employer, shall perform his duties to the best of his ability and shall be at the place of business required during the hours that the Employer has previously specified and shall perform such other work as may be required of him by the Employer and subject to its instruction, direction and control.

5. Whereby transportation outside the town of Truro is a factor, the Employee agrees to deliver services to the Employer at $____________ , and to reimburse the Employee for any expenses incurred in transporting the equipment.

6. This agreement shall insure to the benefit of and shall be binding upon the Employer, its successors and assigns.

If paying by cheque, please make it payable to Darryl A. MacIsaac or TIDAL WAVES DJ Services. The balance remaining toward the cost of your show is due before the commencement of the event.

A receipt will be issued at the commencement of the show on the designated date for the full cost of the occasion. Thank you for your patronage. We will do our best to make your occasion a festive and memorable one.
