History~ 2003-2004 11,855,702 Medicare beneficiaries discharged from the hospital (Lattimer, 2013). ~19.6% were readmitted within 30 days~ 34% were readmitted within 90 days~ 50.2% of those were readmitted within 30 days after a medical discharge (Lattimer, 2013).~ Research has shown that engaging a group of partners such as nursing facilities, home care agencies and social service agencies reduces the readmission rate and help deliver higher-quality care, in a more cost efficient manner.

SolutionA non-profit organization in the community that assists in the prevention of hospital readmissions of cardiac patients after discharge from the hospital. Funded by grants, Medicare, and Medicaidprovide a solid transition from an in-patient stay to home environment, which would begin with education while in the hospital and continued through extensive monitoring and rehabilitation during the first 30-90 days.

Continued Care MissionOur mission at Continued Care is to prevent hospital readmissions through high quality nursing services that maintain and promote health through collaboration, education and self-care.

Continued Care GoalsComprehensive Discharge Planning including medication education and medication coordination.Patient communication on the day of discharge and a personal visit with an Registered Nurse within 48 hours of discharge. Collaboration with dietician services.Coordinate care with additional outpatient services such as physical therapy or home health aids.Provide continuous monitoring for up to 90 days with a seamless transition for continued care through their Primary Care Physician.

Hierarchy of Continued CareJob DescriptionsBoard of Directors: This (unpaid) board consists of the CEO, CFO, Director, representative from each hospital in the city.CEO: Answers to the Board of Directors. Meets the needs of the patient population, the board of directors, the companys employees, customers and policy.Director: NP or Masters Degree level nurse. Reports to the CEO. Manages the patient care day to day operations of Intake Specialist and Nurse Care Managers.Intake Specialist: Interview clients, investigate their needs, facilitate care between the client and the Nurse Care Managers. Nurse Care Managers: Perform in home visits, assess patient needs, follow up care referrals, acts as liaison between patient and other care providers to facilitate care. Answers to the Director. CFO: Answers to the Board of Director. Evaluates and is responsible for managing the financial aspect of the organization, including record keeping, financial planning and reporting fiscal situation to the Board.Billing Specialist: Receives and processes all billing statements, audits coding and patient/insurance billing.Office Administrator: Handles day to day office procedures including light human resource work, scheduling, provider or client complaints, working with outsourced companies, and staffing issues. Answer to CFO.Admin Assistant: Answers phones, scheduling appointments, filing, first line of communication and routing customer or provider to the right person. Answers to the Office Administrator.

Continued Care BudgetIt is important to create a thorough, one-year budget for Continued Care to accurately plan for costs associated with the organization. A budget is the basic financial document used in most healthcare organizations (Yoder-Wise, 2011, p. 239). A personal budget is to determine how many staff members will be needed to provide the care. Full-time equivalent or FTEs are used as a measurement for staff members who work hours during the year. 1 FTE=40 hrs/per wk in a 52 week/year and a 0.5 FTE=20hr/per wk worked.

Expenditure/Revenue ChartGrants will be used the first month for start up costs and salaries. Revenue brought in for services rendered. $10,000 will be kept in account for future expenses in emergencies. Revenue during the year will be adjusted with future grants such as the Gannett Foundation, Blue Cross Blue Shield or a renewal of past grants. An evaluation of revenue home care costs will be done quarterly and adjusted accordingly so that there is a balance between expenditure and revenue.
