DESIGNATED RESPONSIBLE PARTY – Willing adult that agrees to assist the Employer with responsibilities for CDS

PARENT – Natural, Legal, Adopted, Step-Parent, Foster Parent, Court Appointed Guardian

SPOUSE –Married with Formalities (Legal) or Married without Formalities (Common Law) Texas Family Code
