BACKGROUND INFORMATION

The C l i n i c a l Immunology Branch o f t h e D i v i s i o n o f A l l e r g y , Immunology and T r a n s p l a n t a t i o n (DAIT) o f t h e N a t i o n a l I n s t i t u t e o f A l l e r g y and I n f e c t i o u s Diseases ( N I A I D ) s u p p o r t s r e s e a r c h on humoral, c e l l u l a r and molecular mechanisms o f immune system f u n c t i o n s i n h e a l t h and d i s e a s e and t h e a p p l i c a t i o n o f t h i s b a s i c b iomedica l knowledge t o c l i n i c a l l y r e l e v a n t problems. T h i s Request f o r A p p l i c a t i o n s (RFA) is in t ended t o encourage and i n v i t e t h e development o f program p r o j e c t a p p l i c a t i o n s from c o l l a b o r a t i n gb a s i c s c i e n c e r e s e a r c h and c l i n i c a l i n v e s t i g a t i v e groups concerned w i t h i n t e g r a t e d s t u d i e s on autoimmune mechanisms i n d i s e a s e .

RESEARCH GOALS AND SCOPE

Immune system mediated t i s s u e i n j u r y (autoimmunity) is a major area o f endeavor under t h e purview o f t h e N I A I D C l i n i c a l Immunology Branch. Wi th in t h i s s p e c i f i c r e s e a r c h a r e a , t h e g o a l s o f t h e program p r o j e c t s are: 1 )i n c r e a s e d unde r s t and ing o f t h e e t i o l o g y and p a t h o g e n e t i c mechanisms involved i n autoimmune d i s e a s e s ; and 2 ) t h e g e n e r a t i o n o f an expanded knowledge base t h a t can be a p p l i e d t o t h e development o f improved measures o f d i a g n o s i s , t r e a t m e n t , and p r e v e n t i o n o f a wide v a r i e t y o f d i s o r d e r s i n v o l v i n gimmunological ly mediated inf lammatory r e a c t i o n s .

The d e s i g n and scope o f t h e s e program p r o j e c t s are in t ended t o i n c l u d e s t u d i e s on a l l a s p e c t s o f immune r e sponses r e s p o n s i b l e f o r o r a s s o c i a t e d w i t h d i s e a s e s i n v o l v i n g t i s s u e i n j u r y o r damage i n which a r o l e f o r elements o r f u n c t i o n s o f t h e immune system can be i d e n t i f i e d . Broad approaches t o r e s e a r c h on autoimmune mechanisms i n d i s e a s e may i n c l u d e s t u d i e s concerned wi th r e l e v a n t a r e a s o f g e n e t i c s , c e l l and molecular b io logy , b iochemis t ry , phys io logy ,microbio logy , and pharmacology.

O f s p e c i a l i n t e r e s t t o N I A I D are program p r o j e c t s w i t h r e s e a r c h emphasis i n d e f i n e d a r e a s o f i n v e s t i g a t i o n t h a t show promise i n e l u c i d a t i o n o f b a s i c immune mechanisms and t h e i r a p p l i c a t i o n t o c l i n i c a l autoimmune d i s o r d e r s . The N I H p l a c e s s p e c i a l emphasis on t h e need f o r i n c l u s i o n o f m i n o r i t i e s and women i n s t u d i e s o f d i s e a s e s which d i s p r o p o r t i o n a t e l y a f fec t them and a l s o u r g e s t h a t a p p l i c a n t s g i v e added a t t e n t i o n , where f e a s i b l e and a p p r o p r i a t e , t o t h e i r i n c l u s i o n i n o t h e r c l i n i c a l s t u d i e s . For proposed popu la t ion -baseds t u d i e s which i n c l u d e n e i t h e r women n o r m i n o r i t i e s , a c lear r a t i o n a l e f o r n o t i n c l u d i n g them must be p rov ided . I n a t t e m p t i n g t o i n c l u d e e i t h e r group i n a p a r t i c u l a r s t u d y , a t t e n t i o n must be p a i d t o such i s s u e s as r e s e a r c h d e s i g n and sample s i z e . A program p r o j e c t g e n e r a l l y i n v o l v e s t h e o rgan ized e f f o r t s o f groups o f i n v e s t i g a t o r s , members o f which conduct r e s e a r c h p r o j e c t s r e l a t e d t o t h e o v e r a l l program o b j e c t i v e . PURPOSE

The purpose o f t h i s Request f o r A p p l i c a t i o n s (RFA) i s t o award a c o o p e r a t i v e agreement t o s u p p o r t a d rug abuse t r e a t m e n t r e s e a r c h demons t r a t ion program i n t h e D i s t r i c t o f Columbia. T h i s program w i l l e s t a b l i s h , t e s t and e v a l u a t e t h e e f f e c t i v e n e s s o f a d rug abuse t r e a t m e n t approach based on sho r t - t e rm ( i . e . , 6 months d u r a t i o n ) t r e a t m e n t i n a t h e r a p e u t i c community fo l lowed by i n t e n s i v e a f te rcare i n o r d e r t o examine t h e e f f e c t i v e n e s s o f t h i s approach i n p a r t i c u l a rd i a g n o s t i c and p a t i e n t / p o p u l a t i o n subgroups. The District o f Columbia was s e l e c t e d a s t h e s i t e f o r t h i s model t r e a t m e n t r e s e a r c h demons t r a t ion p r o j e c tbecause o f t h e e x t e n t and i n t e n s i t y o f t h e d rug abuse problem, a v a i l a b i l i t y of c l i e n t s , and because t h e combinat ion o f p a t i e n t subgroups and t y p e s o f d rugabuse problems l e n d s i t s e l f t o a r e s e a r c h demons t r a t ion p r o j e c t t h a t may be a b l e t o b e r e p l i c a t e d i n o t h e r p a r t s o f t h e c o u n t r y . The ADAMHA program w i l l be funded i n FY 1990 a t $2,790,000. The programp r o v i d e s awards r a n g i n g from $ 2 0 , 0 0 0 t o $ 1 0 0 , 0 0 0 t o e l i g i b l e i n s t i t u t i o n s . E l i g i b l e i n s t i t u t i o n s a re t h o s e t h a t had f i v e or more a c t i v e ADAMHA r e s e a r c h o r c a r e e r g r a n t s awarded i n FY 1989. A t l e a s t one o f t h e awards m u s t be from t h e f o l l o w i n g t y p e s o f developmental awards: R03, R23, R29, K07, K 0 8 , K 1 1 , K20. PURPOSE

The N a t i o n a l I n s t i t u t e o f D i a b e t e s , D i g e s t i v e and Kidney Diseases ( N I D D K )i n v i t e s i n v e s t i g a t o r - i n i t i a t e d r e s e a r c h g r a n t a p p l i c a t i o n s (Ro l l t o advance u n d e r s t a n d i n g o f t h e i n t e r a c t i o n of e n d o c r i n e , c e n t r a l nervous system (CNS),and immunological r e s p o n s e ( s ) t o s t r e s s .

DISCIPLINES AND EXPERTISE

I n t e r d i s c i p l i n a r y approaches may be needed f o r t h i s s t u d y w i t h e x p e r t i s er e q u i r e d i n s e v e r a l o f t h e f o l l o w i n g a r e a s : neuroendocrinology, c e l l and molecular b i o l o g y , h i s t o l o g y , neurophysiology, immunology, and p e p t i d e and/ors t e r o i d b i o c h e m i s t r y .

BACKGROUND

C o r t i c o t r o p i n r e l e a s i n g f a c t o r ( C R F ) i s t h e hypothalamic hormone r e s p o n s i b l ef o r s t i m u l a t i n g t h e r e l e a s e o f p i t u i t a r y p reop iomelanocor t in (POMCI-derivedn e u r o p e p t i d e s , i n c l u d i n g ACTH and E-endorphin. I n a d d i t i o n t o i t s well-known r o l e i n i n i t i a t i n g h y p o t h a l a m i c - p i t u i t a r y - a d r e n a l (HPA) a x i s r e s p o n s e s t o a c u t e s t r e s s and modula-ting long-term a d a p t a t i o n t o c h r o n i c stress, CRF h a s been found t o ac t c ; i t h in o t h e r areas o f t h e CNS t o modulate b e h a v i o r s a s s o c i a t e d w i t h t h e r e sponse and a d a p t a t i o n t o stress. I t h a s been sugges t edt h a t CRF a c t s t o i n t e g r a t e t h e o v e r a l l i n t e r r e l a t e d endoc r ine , n e u r a l , and b e h a v i o r a l r e s p o n s e s t o s t r e s s . I n a d d i t i o n , t h e r e i s p r e l i m i n a r y , b u t p r o v o c a t i v e ev idence f o r b i d i r e c t i o n a l communication between t h e immune, e n d o c r i n e , and c e n t r a l nervous systems mediated by p e p t i d e hormones and r e c e p t o r s common t o t h e s e sys t ems . A v a r i e t y o f n e u r a l c e l l s secrete i n t e r l e u k i n s and growth f a c t o r s , w i t h neu rona l c e l l growth and p r o l i f e r a t i o nr e g u l a t e d by t h e s e monokines and growth f a c t o r s . More r e c e n t l y ev idence h a s been p r e s e n t e d t h a t lymphocytes produce n e u r o p e p t i d e s i n c l u d i n g endorph ins ,enlccphal ins , v a s o a c t i v e i n t e s t i n a l p e p t i d e ( V I P ) , s u b s t a n c e P and c o r t i c o t r o p i n , a l t h o u g h t h e p h y s i o l o g i c a l s i g n i f i c a n c e of t h i s c a p a b i l i t y h a s y e t t o bc e s t a b l i s h e d . These c e l l s a l s o e x p r e s s r e c e p t o r s f o r n e u r o p e p t i d e sand respond t o t h e s e hormones. The same p e p t i d e s may r e g u l a t e p r o d u c t i o n o f c o r t i c o t r o p i n i n lymphocytes and i n t h e p i t u i t a r y . T h e r e f o r e , one element o f t h e immune s y s t e m ' s r e sponse t o s t r e s s e s , such as i n f e c t i o n , inf lammation,t o x i n s , tumors , t r a u m a and p o s s i b l y p s y c h o l o g i c a l stress, may i n v o l v e a c t i v a t i o n o f t h e HPA a x i s a t m u l t i p l e l e v e l s . I t h a s become a p p a r e n t t h a t a c u t e i n f e c t i o n a c t s v i a CRF i n i n i t i a t i n g a c t i v a t i o n o f t h e neu roendocr ine system as p a r t o f t h e r e sponse t o t h i s stress. Recent i n f o r m a t i o n i n d i c a t e s t h a t immune system cy to l t i nes , i n c l u d i n g I L ( i n t e r l u e k i n l - 1 , IL-2, thymosin,and thymopo ie t in , i n t e r a c t a t c e n t r a l hypothalamic and/or a n t e r i o r p i t u i t a r ys i t e s t o r e g u l a t e release o f ACTH wi th consequent e f f ec t s on a d r e n a l g l u c o c o r t i c o i d r e l e a s e . These d a t a have been i n t e r p r e t e d t o s u g g e s t t h a t c y t o k i n e a c t i o n a t t h e l e v e l o f CRF r e l e a s e l i n k s t h e a c u t e r e sponse t o inf lammation t o t h e s t r e s s r e s p o n s e . Many q u e s t i o n s remain, i n c l u d i n g t h a t o f t h e c e l l u l a r and molecu la r mechanisms o f a c t i o n o f CRF on CNS c e n t e r s and t h e r o l e ( s ) and mechanism of a c t i o n o f immune e f f e c t o r s on t h e HPA a x i s and v i c e v e r s a . F i n a l l y , demons t r a t ion o f a c lear b i o l o g i c a l r o l e f o r t h e i n t e r r e l a t i o n s h i p between t h e neu roendocr ine and immune systems remains t o be e s t a b l i s h e d .

OBJECTIVES
