As a teacher no one size fits all, assess on a case by case basis.
Many students suffer with ADD, ADHD, Dyslexia, Speech and language impairment ,dysgraphia ,auditory processing, autism spectrum disorders. Many variables to each case.
Additionally, teachers must make considerations for students with English as a second language(ESL).What are you trying to assess? Use different approaches.
Assessment is an important tool to identify students at risk of falling behind and where students are lacking understanding in order to plan & adapt.
Types of assessment used are : Summative:- exams, assignments, presentations, end of unit work. Formative:- observations, folios, conferences writing and journals. Diagnostic:-running records, checklists, formal assessment tools.

Strategies to consider in the classroom:
Reading: Provide short excerpts from a book, short stories and picture books, with rhyme and rhythm ( Dr Seuss). Give copies to each student
Model reading first out load with clear pronunciation and intonation Have students follow as you read text. Then again reading with you out load in unison. When comprehension of English is very limited as with ESL students start with small greetings, sentences and stories first and gradually increase.
Repetition is important for fluency and building confidence. Monitor: check for understanding.
