due to injury, illness, or eligibility, or• Student-athlete competes before suffering season-ending
injury:– Prior to completion of the first half of the season and– For team sports: Prior to participation in more than 30% of
scheduled in-season contests; or– For individual sports: Prior to participation in more than 30%
of the maximum permissible dates of competition plus one date for a conference championship.

5th Year Waiver Criteria• Preserved year of eligibility, and• Academic need to return as an undergraduate
for a 5th year– Academic advisor will certify that:
• The student appropriately considered academic and career goals when choosing this enrollment pattern,
• The decision was in the student’s interest regardless of athletics participation, and
• The decision was made by the student, without pressure to do so from a coach or other athletics department staff member.
