We are the
Group 1, deployed at
Barangay Nangka
together with ourclinical instructors.
Barangay Nangka is
part of the District II in
the Municipality of
Marikina.
Most of the
people living in the
Barangay are those
families that arerelocated by the local government. They were given houses as their shelter and a
health center as their primary source of Health Assistance.
We, the student Nurses, promoting health in different ages and people, were
given the chance to apply our knowledge in the community setting and help the two
adopted families improve their hygiene and health.
We learned how the two families live in their barangay. How they are working
and how they are able to work to live.
The adopted families
were chose based on their
socioeconomic status. This
includes their daily income, the
materials used for their house,
how they were able to get their
resources and their family
orientation, which are the
number of family and the
availability of health careservices that they were able to
get.
We, the student nurses, are
going to adopt them and help
them to improve their health status.

First Day (January 15, 2014)
Our meeting place is at school.
We had our planning for the whole day
about the things that we are going to do
until the rest of the week. We hadfollowed the COPARs first phase, which
is pre-entry phase. We brainstormed to
formulate a good plan that involves
different criteria about the specific
community like the age of participants,
area of venue, topics selection and the
way that we are going to implement our
intervention not only for the community
participants but also for the family that
we adopted. Our main goal is to improve the way of living of the community people.

On the second day of our duty, our group met at Nangka Health Center and was
introduced to the Barangay Health Workers as the new group for the community health
nursing.
We went to the area to conduct our initial visit to our adopted families. We gave our
health teaching to the mother and did morning care to the children such as bathing,changing of clothes, brushing of teeth and combing of hair. In the afternoon, we did the
community profiling and the ocular survey. We have chosen a new adopted family that
we think is in really need of help.

Third Day (January 16, 2014)
FIRST FAMILY SECOND FAMILY
On the third day of our
duty, our meeting place is at
Nangka health center. We
waited for 30 minutes for Maam
Gina, the head of the barangay
health workers, to help us get a
new adopted family. We
decided to go to the community
outpost after we made a call. At
the outpost, we met MaamGina. We had an ocular survey
and had a look at the family that Maam Gina proposed. We decided to split into two
groups. The first group will go to the first family. We found our second family, gathered
information about them and informed them that we will return next week. The second
group went to the first family where they gave health teachings and they did some

morning care like giving bath, brushing and changing of clothes. After that, we met
again at the barangay outpost.
In the afternoon, we gathered our students for our tutorial then we taught them
the subjects they find difficult. We gave them some snacks and after that, we had our
exam and post-conference.
Fourth Day (Jan.20.2014)
Our meeting place is at school. We had our planning for the whole day about the
things that we are going to do until the rest of the week. We had followed the COPARs

HYPERTENSION
-Hypertension, also referred to as high blood pressure, is a condition in which the
arteries have persistently elevated blood pressure. Every time the human heart beats, itpumps blood to the whole body through the arteries.
-Blood pressure is the force of blood pushing up against the blood vessel walls. The
higher the pressure the harder the heart has to pump.
-The normal level for blood pressure is below 120/80, where 120mmHg represents the
systolic measurement (peak pressure in the arteries) and 80 represents the diastolic
measurement (minimum pressure in the arteries). Blood pressure between 120/80 and
139/89 is called pre-hypertension (to denote increased risk of hypertension), and a
blood pressure of 140/90 or above is considered hypertension

PREVENTICVE MEASURE
Hypertension can best be prevented by adjusting your lifestyle so that proper diet and
exercise are key components. It is important to maintain a healthy weight, reduce salt
intake, reduce alcohol intake, and reduce stress.
In order to prevent damage to critical organs and conditions such as stroke, heart
attack, and kidney failure that may be caused by high blood pressure, it is important to
screen, diagnose, treat, and control hypertension in its earliest stages. This can also be
accomplished by increasing public awareness and increasing the frequency of
screenings for the condition.

Second Family Visit
On the second day of our second week in the community, we were divided intotwo groups. The first group were assigned to the Ducay family and the second groupwere assigned to the Fernandez family. The student nurses that were assigned to the
first family did the morning care to the children of Mr. and Mrs. Ducay, they also didwound cleaning. Mr. Jeffrey Bucala also accompanied the child Sonny, the son of Mr.Ducay to the health center because a dog bitten by a dog on his right hand and he wasalso running a fever. They went there to have a check-up and to be vaccinated withanti-rabies. The second group cleaned the house of Fernandez family. They alsoseparate clean clothes from dirty clothes. They also provided some health teaching tothe family about proper hygiene.In the afternoon, we did our ocular survey around the area to find our target participantsfor our health teaching on the next day. Our target audiences are senior citizen withages ranging from 60 and above. After giving the stub/ticket to those who willparticipate, we did our tutorial to the children who are havingdifficulties in their studies.The students are from grade I to grade VI. We also decided to teach pre-schooler andthose children who are not yet studying.

On the third day of duty at Nangka community, we conducted our health teaching
about Hypertension in the Del court near Nangka High School. We have 23 participants
in the said event. We discussed what Hypertension is about, the signs and symptoms,
prevention and some complication of Hypertension. After the health teaching the
participants are thankful to us because the knowledge we have provided them.
In the afternoon we did our family visits, the first family that we visited is the
Fernandez family. We told them that it would be our last day in the community. We did
health teaching and after that we gave some donations like old clothes and we gave
some foods like rice, biscuits etc. after the gift giving, the family was so thankful and
happy. We also did the same thing with the Ducay family. We did gift giving and we
gave two pregnancy tests to the two women who are also living there.

After all our activities we had a post conference together with our clinical instructor and
head nurse and we are delighted that we finally finished our community duty and are
satisfied about what we have learned in the community. We feel blessed because we
have Clinical Instructors, a Head Nurse and group mates that are responsible enough to
do their task individually.
