The scoring system used was based on the WHO Criteria for Problem Prioritization which focuses on the magnitude of the problem, vulnerability of the problem to technology, social concern about the problem, and existing health policies to address the problem. The high prevalence of hypertension is the first priority problem (Score=8) because it comprises 35.71% of all the diseases in the community, which is worse than national health standards, and there is a high prevalence of hypertension in both males and females. It is vulnerable to technology because there are a lot of available ways of prevention and maintenance drugs that are widely available. There are existing policies and is strictly implemented in the community. However, no household viewed the problem as a community concern. High annual population growth rate has a score of 8 due to an annual population growth rate of 13.9, and many are not knowledgeable in family planning despite the presence of policies that already address this matter. Hypertension was chosen over high population growth rate as the main problem in the community because although it is the most prevalent morbidity among the residents, they still don’t see it as a community concern. Drugs and technology are readily accessible to the public. High prevalence of acute respiratory infections ranked last priority (Score=7) because there are only 21.42% affected by the disease. There are available technologies to solve the problem and medications can easily be acquired in RHUs and drug stores. B. Statement of the Priority Health Problem

Project Title:

Background of the ProblemA. Situational Analysis

The top three problems in Block 25, Barangay St. Peter II, Dasmariñas City, Cavite are as follows (in

descending order of prioritization): high prevalence of hypertension, high annual population growth rate, and

high prevalence of acute respiratory infections. The scoring system used was based on the WHO Criteria for

Problem Prioritization which focuses on the magnitude of the problem, vulnerability of the problem to

technology, social concern about the problem, and existing health policies to address the problem. The high

prevalence of hypertension is the first priority problem (Score=8) because it comprises 35.71% of all the diseases

in the community, which is worse than national health standards, and there is a high prevalence of hypertension

in both males and females. It is vulnerable to technology because there are a lot of available ways of prevention

and maintenance drugs that are widely available. There are existing policies and is strictly implemented in the

community. However, no household viewed the problem as a community concern. High annual population

growth rate has a score of 8 due to an annual population growth rate of 13.9, and many are not knowledgeable

in family planning despite the presence of policies that already address this matter. Hypertension was chosen

over high population growth rate as the main problem in the community because although it is the most

prevalent morbidity among the residents, they still don’t see it as a community concern. Drugs and technology

are readily accessible to the public. High prevalence of acute respiratory infections ranked last priority (Score=7)

because there are only 21.42% affected by the disease. There are available technologies to solve the problem

and medications can easily be acquired in RHUs and drug stores.

B. Statement of the Priority Health Problem

The priority problem that is focused in this study is the inadequacy of knowledge on the cause, risk

factors, effects and symptoms of hypertension. The conducted problem investigation also showed us the poor

practices of the respondents regarding hypertension and this can be due to the lack of knowledge regarding the

importance of blood pressure monitoring and maintenance of normal blood pressure hence this will also be

explained in the lecture. This project aims to give the respondents useful information regarding hypertension

and consequently reduce the prevalence of hypertension in the community.

C. Explanation of the Priority Health Problem

In the problem investigation conducted, this showed that on average, the households have inadequate

knowledge regarding risk factors of hypertension which is 51.28%. In terms of their knowledge about the causes

of hypertension they got 71.79% which is also inadequate. The household knowledge on the effects of

Page 2: Community Medicine

hypertension is inadequate which on the average was 53.85%. In regard to preventing hypertension they got an

adequate knowledge which is 78.02. Lastly, their knowledge with regard to the symptoms of hypertension such

as dizziness, headache and blurring of vision (69.23%) is still inadequate.

With these values we can conclude that there is great necessity for education regarding the risk factors,

causes, effects and symptoms of hypertension among the respondents. Adequate education can improve health

seeking behaviour, attitude towards a healthy lifestyle and the quality of life. Teaching the risk factors of

hypertension will help prevent the development of the disease among those who are at high risk of developing

hypertension and in return can reduce economic burden to the society and to the family.

Practices regarding hypertension also showed poor results. Survey showed that appropriate practices for

the prevention and control of hypertension were found inadequate among respondents. This is evident in terms

of the BP monitoring practices wherein only 7 (58.85%) of the respondents monitor their BP, 5 (38.46%) do the

monitoring monthly. 6 (46.15%) of the respondents do not monitor their blood pressure and lack awareness of

their usual BP. There are only 4 (30.77%) of the population who exercise regularly and only 4 (30.77%) has

normal BMI, many have low BMI. Among 6 hypertensives, there are 5 (83.33%) who are compliant to

medications and 1 (16.68%) do not take the medications regularly. Lectures or seminars about monitoring blood

pressure, benefits of maintaining blood pressure within normal levels, importance of physical activity and

teaching them DASH diet can be beneficial for the respondents to gain better practices on the management of

hypertension.

MethodologyA. Strategies

I. Identification of Strategies

Health education with the purpose of positively influencing the health behaviour of individuals is the

strategy of choice for the respondents of Block 25, St. Peter II, Dasmariñas City, Cavite. Upon investigation, the

main health problem identified was the high prevalence of hypertension among the residents. Therefore, the

focus of the health education will be on the increasing awareness on the cause, risk factors, symptoms and

effects of hypertension, as well as reinforcing appropriate practices for the prevention and control of

hypertension. In line with this, giving of lectures, small group discussions and pamphlets will be utilized for the

health education.

The WHO criteria of (1) Potential impact, (2) potential scope, (3) feasibility, and (4) social acceptability

were taken into consideration for the selection of the most appropriate method to approach the cause of the

priority problem which were amenable to change.

II. Prioritization of Strategies

The learning strategy chosen to be utilized for the residents of St. Peter II, Dasmarinas City Cavite will be

a lecture type of learning strategy to be made possible by holding a big group lecture and pamphlet distribution.

According to Cantillon 2003, providing lectures are efficient means to transfer knowledge and concepts to large

groups as it can be used to stimulate the interest, explain the concepts and provide core knowledge and direct

learning.

The lecture which will be requiring the active participation of the residents and the visual aids will be

used to aid in relaying the information to the participants and this will show the aforementioned points to be

discussed regarding hypertension. Pamphlet distribution after the session will aid the participants in having a

review on the basics principles on knowledge and practices regarding hypertension. The advantage of giving out

pamphlets is that it will be available for the participants to read during their free time.

For the social concern, both big group lecture and pamphlet distribution are effective means to get the

attention of participants as well as achieving the objective of the group which is to impart knowledge on

Page 4: Community Medicine

hypertension. In the interview conducted, the respondents showed a very good attitude about learning more of

hypertension and this project is expected to give positive outcomes after its implementation.

Video presentation will not be used because there will be more equipment needed and is not feasible

for the available resources.
