The LWAH provider:

Identifies risk factors

Educates about risk factors and approaches

Discusses their community living goal and community-

based services & support

Keeps family involved

Conducts regularly scheduled follow-up on progress

and successes with client

Ideas, action steps, and decisions are centered around the goal.

Risk Factors Causing Older Adults to Move From Home

1. Unable to perform everyday tasks

2. Had an injurious fall 3. No one available to help4. Available help is stressed5. Thinking of moving where

there is help6. Older adult lives alone7. Concerns about memory
