Our Community Health Needs Assessment

In 2015, IU Health Goshen prepared a Community Health Needs Assessment (CHNA) for Elkhart County, Indiana, as required by the Patient Protection and Affordable Care Act of 2010. The purpose of the report was to understand and prioritize the health needs of the community and develop strategies reported to the needs. IU Health Goshen’s prior CHNA Report and corresponding Implementation Plan were adopted in 2012.

The 2015 CHNA Report is based on quantitative and qualitative community health information and provides a comprehensive understanding of health improvements that

a systematic data informed process and with the input of individuals throughout Elkhart County and in the Elkhart County Health Department.

IU Health Goshen began assessing community health needs long before the Affordable Care Act of 2010, developing programs to

Medically Underserved Areas (MUAs)

We are implementing plans to enhance healthcare access in medically underserved areas.

In order to address the Dunlap medically underserved area, an urgent care facility is currently being constructed.


Partnership with Maple City Health Care Center

Since 2012, IU Health Goshen has worked to increase community access to health services.

We have enhanced healthcare access through our partnership with the Maple City Health Care Center in Goshen.

IU Health Goshen Foundation

IU Health Goshen and Maple City Health Care Center (MCHCC) have created a partnership committed to expanding access to primary care for an additional 5,000 members of the community. In November 2013 IU Health Goshen purchased the property located in North Goshen, often referred to as the Abshire Mansion. Through renovation of the existing structure and new clinical construction in 2014, a second location for MCHCC has been developed.


Community Health Programs

In 2012, IU Health Goshen enhanced its community programs in order to aggressively address

IU Health Goshen undertook initiatives and enhanced programming in a variety of areas.

Educational workshops addressed nutrition, weight management and health literacy topics. Diabetes came particularly into focus as the topic of health fairs, preventive educational seminars and ongoing support group meetings. Cancer screening and education programs were offered with an emphasis on tobacco cessation.

In prioritizing the health needs which IU Health Goshen can best address, we carefully considered the effectiveness and feasibility of those interventions we could best undertake.


Health Priorities for IU Health Goshen

Tobacco use. Community classes, individual education, remote educational sessions and outreach events and activities aimed at helping people quit, or never start smoking, are ongoing and expected to grow with staff dedicated to the effort.
