Cooking WorkshopsCollective Kitchen for Single Older AdultsThis group meets once a month and creates three main dishes and one soup or one salad to take away. We assign tasks and duties as a group. Please bring your own containers suitable for freezing and reheating. This is a hands on workshop. Food Costs $8 per session.

Men’s Cooking ClubCalling all gents who enjoy being in the kitchen! This class is open to any man who has participated in the ‘Men’s Cooking Club’ before or who enjoys food and is looking to increase their cooking skills and confidence. Participants may sign up for either group A or group B, as both groups will cook the same meals. These classes are hands on and we always enjoy the delicious food together. $5 per class.

Diabetes WorkshopsCarb CountingCome and learn about what carbohydrates are and why they are important in diabetes management. We will teach how to read a food label and count your carbs to help get better control of your blood sugars and eat a balanced meal!

Just the Basics: DiabetesWould you like to prevent diabetes? Or have you recently been diagnosed with prediabetes or diabetes? This workshop includes a discussion of the basic facts about managing diabetes, healthy eating, and physical activity. We encourage people to bring a friend/family member for support.

Heart Healthy - Do Your Part to Protect Your HeartPeople with diabetes, prediabetes or even people at risk for diabetes are more likely to develop heart disease. Join a Certified Diabetes Dietitian in learning how YOU can do your part to protect your heart.

Diabetes Cooking WorkshopsDiabetes Cooking Workshops are hands on cooking classes which are offered to those living with prediabetes, type 1 or type 2 diabetes. Each workshop will be conducted by a Registered Dietitian, who is also a Certified Diabetes Educator.

Type 1 Diabetes Support Group 18+On the 2nd Thursday of every month, come and meet other people that are living some of the similar experiences that you are. Through encouraging each other, sharing ideas and talking about your experiences you can find new ways to cope with type 1 Diabetes in your life!

Diabetes Drop-In ClinicEvery Friday from 1:30pm to 3:30pm drop-in for diabetes supplies pick-up, glucometer checks, blood sugar checks and urgent appointments. For existing clients only. Must sign in at reception. Port Hope location only.

Community WorkshopsCommunity LunchesCome out and connect with others in your community while enjoying a healthy lunch prepared as much as possible from local, seasonal ingredients by our community lunch volunteers. Join us for our monthly drop-in donation based lunch. Suggested donation of $5

Souper Douper Yummy Soup LunchOn the third Thursday of each month our amazing Soup Sisters will be offering a bowl of delicious soup and a roll. Join us for soup and a chat in the multipurpose room. Suggested donation of $2.

Powerful Tools for CaregiversFree six week Powerful Tools for Caregivers is a workshop series designed to help family caregivers take better care of themselves while caring for a family member or friend. You will receive a free copy of the caregiver help book. Register by calling 1-866-971-5545.

Caregiver Support GroupHave you completed the Powerful Tools for Caregivers workshop? Are you looking to connect with other caregivers? This support group meets the third Friday of every month and provides support, follow-up and information for caregivers.

Stroke Survivor & Acquired Brain Injury Support GroupOn the first Friday morning of each month from 10:30am to 12pm we invite all stroke survivors, those living with ABI and caregivers to meet and provide mutual support and encouragement. This is an open group so please join us.

Osteoporosis Support GroupYou will gain practical information in the monthly meetings, meet others living with Osteoporosis and share common experiences. We cover a variety of topics such as nutrition, safe exercises when living with Osteoporosis and more. For more information Contact Cathy Pearcy by email [email protected]

Hoarding Support GroupTo provide a positive, welcoming venue where people who have hoarding issues can speak freely and openly, and learn new skills to help themselves cope. The group meets on the second Wednesday of each month.

Mindfulness Based Cognitive TherapyMindfulness-Based Cognitive Therapy (MBCT) is an 8 week program especially useful in managing difficult, persistent emotion. MBCT combines the ideas of cognitive behavioral therapy with meditative practices and attitudes based on the cultivation of mindfulness.

Women’s Book ClubThroughout the year we will meet to discuss the books of choice, and enjoy a cup of tea in a fun and positive space. Check out our website for the current books of the month and to see when we are meeting next.

Ladies Social GatheringsThroughout the year the CHC hosts themed social gatherings for ladies where we enjoy a chat, a cuppa and some games. Such themes include a fall tea, christmas treats, winter tea and a Canada day party. Registration is requiredDance For Parkinson’sThis class integrates movement from ballet, jazz, tap and contemporary dance techniques to stretch and strengthen the body, improve posture and balance and engage the mind. Thinking like a dancer helps participants regain physical confidence in their own bodies. Classes are taught by experienced dance educators/dance artists. This class is also suitable for those living with MS, A.B.I and Stroke. Caregivers are encouraged to attend. (Spring & Fall)

Youth Drop-InThis after-school program is provided to youth in grades 7 to 12 at the Community Health Centres of Northumberland at 99 Toronto Rd, Port Hope, every Tuesday starting at 3pm. This program is offered in partnership with the Municipality of Port Hope, Rebound and the Port Hope Police Services.

Home Alone CourseThis FREE 2.5 hour course will provide youth ages 10 to 14 with the skills and knowledge that will assist them in being confident about staying home alone. Through interactive games and activities participants will learn about how to stay safe in various situations and what to do in case of an emergency. (offered in July, August and over March Break)

Exercise Workshops Active, Fit & FunDesigned for community members ages 55+ who are wishing to improve their cardio, strength, balance and flexibility regardless of their current ability level. The program will be led by trained volunteers, who ensure the program is run in a safe manner. Classes can be done seated or standing.

Balanced FitnessThis class is offered through We Fix U. It meets twice weekly for a balanced exercise routine which focuses on the basic building blocks of strength, balance and flexibility.

Balance, Strengthen and StretchThis mat based class combines 3 key elements of fitness into one session for the time crunched person. Class segments will include balance training, body weight exercises, and an extended stretch period to end off the class. Suitable for all levels of fitness.

Chair YogaUsing the aid of a chair our yoga instructor will guide you through a series of movements. Poses are done seated or the chair is used for support during standing poses and bends.

Falls PreventionAre you feeling unsteady at times? This class is offered through We Fix U and includes both falls education and exercises for you. 45min classes are twice weekly. Learn how to minimize your risk of falls.

Nordic Pole WalkingCome out and join our trained Nordic Pole Walking instructors as they take you on a 30-45 minute walk around the neighbourhood. (Spring & Fall)

PilatesA mat based, Pilates inspired class designed to increase your posture awareness, learn to use breath to aid movement, increase muscle tone, and strengthen your core. Suitable for all levels of fitness.

Relax and RenewRestorative yoga focuses on relaxation of the mind and body. Bolsters, straps and other props will be used to support the body in various postures which allows one to move towards a state of balance.

Simplified Tai Chi - BeginnersTai chi is a system of gentle exercise based on the principle of effortless breathing, rhythmic movement and weight balance. It gives attention to muscular strength, flexibility, breathing, relaxation, endurance and mental focusing.

YogaYoga has many benefits such as increasing flexibility, improving breathing, helping with balance and helping one to live a healthy and stress free lifestyle. Yoga is a floor based program.

Our Heart Wise Exercise Programs All of these programs meet the following criteria 1. Encourage regular, daily aerobic exercise 2. Incorporate warm-up, cool-down and self-monitoring with all exercise sessions 3. Encourage exercise at a safe level and offer options to modify intensity 4. Are suitable for those living with a chronic condition and cardiovascular disease 5. Have a documented emergency plan.

Dental Services The Community Health Centre is pleased to offer a basic dental program to those who cannot access care due to financial hardship, doors will also be open to those who have private insurance. If you are receiving assistance through Ontario Works, Ontario Disability Insurance or have an annual income below the Canadian Low Income cut off, give us a call. There is a cost to this program which will be determined at your first visit. Ext. 208

Dietitian Services Our Community Registered Dietitian is available to members of the community & provides one-on-one counselling, presentations/workshops & cooking demonstrations. Dietitians provide clients with strategies to help manage & prevent a wide range of nutritional related concerns.Clients of the CHC may be referred by their primary health care provider. For community members who are not clients of the CHC, please ask your family doctor for a referral or you can call the Centre for more info.

Diabetes EducationTo support the growing group of people living with diabetes, the Community Health Centre has nurses, dietitians & social worker teams who are available to meet with individuals & groups. Our diabetes centre provides support and education to individuals who are over the age of 18 and have pre-diabetes, Type 1 or Type 2 diabetes, and Gestational diabetes. We are also a certified pump centre providing insulin pump therapy. The diabetes team welcomes the opportunity to share information at groups or local events.Locations: Port Hope, Cobourg and Colborne

Food Cupboard The food cupboard is open every Thursday afternoon between 1:00-3:30 p.m. The cupboard is open to anyone living locally in need of food. Due to limited space & resources available food does change from week to week. You will be asked to register as a user on your first visit to the cupboard. Donations are essential to the success of this program, non-perishable food items, cash and fresh produce on Thursdays are always welcome.

Legal Centre The Legal Centre is here on the 2nd and 4th Thursday of each month, 12pm-2:30pm. Individuals living on a low income can get free, confidential legal help. Please call 905-373-4464 with any questions.

Smoking Cessation Treatment In partnership with CAMH the Centre is pleased to offer a smoking cessation program that includes free Nicotine Replacement Therapy (patch, gum, inhaler, lozenge) and counselling support for those who want to quit smoking. To find out more, please call the Centre to make an appointment.

OTN - Ontario Telemedicine Network A telemedicine appointment is just like a regular appointment, only the specialist you will be seeing and speaking with is on a monitor. Free to residents of Ontario, OTN connects you with the right health care professional, in the right place, at the right time. Visit otnhub.ca for more information or call Tammy Kantor, RPN. Geriatric Assessment & Intervention Network The GAIN team is for older adults living at home experiencing one or more of the following: changes in memory or thinking; difficulty doing daily activities (such as cooking, bathing, or dressing); falls or risk of falls; difficulty moving around; incontinence; multiple medications. We help older adults address risk and safety concerns and access supports they need to live independently at home. Talk to your primary care provider to discuss if the GAIN team could help you or a family member, or call Ext. 254 for more information about how to complete a GAIN referral.

Memory Clinic The Community Health Centres of Northumberland Memory Clinic is a specialty clinic dedicated to helping patients with memory concerns. On the day of the appointment, a team of skilled health care professionals will complete a thorough medical history and conduct cognitive testing including those related to driving safety. Our team will prepare a management plan for the patient/family and work with your doctor or nurse practitioner to help you link with supports and resources, if needed. Ask your doctor or nurse practitioner to send the necessary referral today. For more information call Ext. 264

Arthritis Clinic Kait Robbins, Occupational Therapist with the Arthritis Society, hosts a clinic here at the Centre on the second Thursday of each month. You can self refer. To make an appointment please call: 1-800-321-1433 Ext. 3381.

The Community Health Centres of Northumberland is a safe needle exchange site and partners with the HKPR Public Health Harm Reduction Works Program. Our site supplies new, sterile equipment including needles, pipes, ties, and more to prevent the transmission of HIV, Hepatitis C and other infections. This program also provides a safe environment to exchange used drug equipment (works). When arriving at the Centre let the front desk know you would like to access the harm reduction supplies and they will assist you directly or connect with a staff member to help get what you need. Naloxone Kits are also available free of charge.

Harm Reduction Supplies

PARN comes every 2nd and 4th Thursday, where their Harm Reduction Outreach Worker and Hepatitis C Outreach Coordinator come to provide information about substance use and preventing Hepatitis C, answer any questions and distribute supplies. When you arrive, just let the front desk know you would like to use the Harm Reduction Program and you will be connected with a staff member!
