Giving the Gift of Care
In Cabarrus County, there are 19,900 uninsured adult family members, friends, and neighbors
who have no access to healthcare. Those with low incomes make tough decisions: provide for
basic needs like food and shelter -OR- attempt to pay for needed healthcare and medications.
Without Community Free Clinic, they rely upon occasional costly visits to our emergency
departments to treat non-emergency chronic health conditions. The services provided at
Community Free Clinic makes the difference in health and life, making and keeping Cabar-
rus County a healthier place to live, work, and play.
Here is the real life case of Jerry, who came to the clinic after enrolling in health insurance in
the Affordable Care Act Marketplace. Initially, he was so pleased with his ability to have
insurance to cover his healthcare needs and keep his asthma controlled. He made an appoint-
ment with a medical practice. Upon checking in, he was told he must pay for the visit up-
front since his insurance policy had a $5,000 deductible. Jerry was devastated and left the
medical practice. His total monthly income is $1200. Even with the subsidy to assist with
out of pocket costs, he cannot afford the deductible and co-pays. His asthma continued to be
untreated and uncontrolled until he walked into the Community Free Clinic. He was provided
with expertise and support to dis-enroll from his insurance and obtain an exemption due to
financial hardship. He was provided access to the medical care and medications he needed.
He was able to take a deep breath and breathe a sigh of relief.
On a Thursday morning, Mrs. Sanders walked into Community Free Clinic. She has a very
serious heart condition and had recently lost her health insurance due to changes in her hus-
band’s employment. She was holding five life sustaining medicine bottles that were empty
as she asked if we could help her. She left feeling relieved and knowing that she had a medi-
cal appointment and access to her medications.
Community Free Clinic provides advocacy for the medically uninsured. Mr. Johnson has
received a diagnosis of cancer. He needed special treatment. Community Free Clinic advo-
cated on his behalf with specialists who will provide needed treatments at no cost to the pa-
tient so that he will be able to return to work and provide for his family.
Patients receive high quality care at Community Free Clinic. In 2015, 87% of our diabetic
patients had controlled blood sugar levels based on their HbA1c’s (long term blood sugar
level). 86% of our patients diagnosed with hypertension maintained controlled blood pres-
sures.
As individuals, businesses, civic groups, churches, foundations, and other donors who pro-
vide financial support to Community Free Clinic, you can know your investments make a
real difference to real people right here in Cabarrus County.

2015 Annual Report
Since childhood I have believed in volunteering. Even then it seemed important. In 1988, I
joined Odell Volunteer Fire & Rescue as an EMT serving people in my fire district. In 1996,
I began volunteering as a nurse at the Community Free Clinic. Both have been, and continue
to be, a most rewarding experience. The appeal of the Community Free Clinic has been the
opportunity to help those in need with chronic diseases such as Hypertension and Diabetes by
being a medical home to provide continuity of care. I have been surprised to find that they
come from all walks of life. I have treated acquaintances and neighbors, people just like my-
self who happen to be in a bad situation. Seeing the care and attention given to our patients is
the most rewarding part of my service. The staff of the clinic are all people who care about
addressing the health care needs of others. Everyone (patients, staff, and volunteers) is spe-
cial. Aside from the volunteer work, I enjoy paper crafting, working out, and spending time
with Patrick (my husband) and Cosette (my wonderful Pekingese). Jaye B Lawrence
I began volunteering at Community Free Clinic in 2015. I had left a position in a hospital setting
after 19 years because patient care has changed so much over the years. I spent less time talking to
patients and trying to get their needs met and more time clicking boxes in a computerized chart. At
the Free Clinic, I feel like I can really make a difference. I volunteer two days most weeks, so I get
to see many of the same people over and over. I have one patient, in particular, who has been re-
warding. The patient is a gentleman who can’t hear at all on one side and poorly on the other. He is
also homeless and has difficulty reading and writing. We managed to form a bond because he could
also hear me better than other staff . As it got colder, he proudly showed me his “new” jacket. I
decided to get gloves for him and some to keep on hand in the clinic. At his next visit, I had taken
an extra back pack from home, put in a hat, gloves, a few pair of new socks, some long underwear,
and some non-perishable snacks. The dear man teared up, hugging me and thanking me. It’s amaz-
ing how something so simple can mean so much to someone. In my free time, I enjoy traveling, hik-
ing, cooking, and various arts and crafts.
Karen Chow
I have been volunteering at the clinic over two years to keep up my RN skills without the
commitment of a job and as a way to give back to the community. The clinic exists for those
patients who fall through the cracks in our current insurance/Medicaid system. They have
incomes so low they are unable to afford healthcare or medications they need. Many cur-
rently work in low paying positions or have worked for years but have lost their jobs due to
their medical conditions or other reasons. Sometimes the patients come in feeling disheart-
ened and dejected by the position they find themselves in; but the clinic communicates hope
and respect.
The staff is very respectful, supportive, and go out of their way to help patients providing
medical care, medications, food and clothing items as well. Dr. Grace Galloway, the nurse
practitioner at the clinic, has a “special way” with patients, getting them to laugh, communi-
cating a “we are in this together” attitude, while maintaining respect, demonstrating empathy,
and providing quality health care.
The clinic is a teaching facility as well for numerous nursing students, nurse practitioner students, medical students,
and other volunteers. I enjoy the atmosphere and supportive, upbeat nature of everyone who works there. It has mo-
tivated me to return to school and pursue a degree as a psychiatric nurse practitioner. One of my favorite patients is
a special gentleman. who has diabetes and struggles with depression. He continues to work and strives to improve
and take responsibility for his illnesses. One day, he came in feeling really down having insight into his depression
and irritability with family and co-workers, upset at his failure to handle things better. We had a long talk about
how we all have our bad days, struggles. I assured him that he wasn’t alone and we are here to help. I was im-
pressed with his insight and told him that. He earnestly wants to live a productive and happy life but struggles with
his medical conditions. Grace talked with him, made some adjustments with his medications, and after a few hugs
he left. He thanked us for our help and that we cared! I wiped away a few tears and felt honored to be a part of his
care that day. The clinic provides an invaluable services and support, to many patients just like him who otherwise
would not have healthcare services.

2015 Milestones at Community Free Clinic Community Free Clinic added on site behavioral health services with grant funding from NC Of-
fice of Rural Health.
Quality improvement initiatives increased substantially. Great improvements were achieved in
patients’ clinical outcomes. Improvements were achieved in the provision of national care stan-
dards for specific chronic diseases.
A no cost patient messaging (texting) system was implemented which decreased patient no show
rates and improved staff communications with patients. (7% no show rate in 2015 compared to
13% in 2014.)
Volunteer Pharm D’s were successfully recruited to support patient medical adherence.
A tenant was secured for the major portion of Community Free Clinic’s upstairs—this space had
been vacant since 2011.
The Community Free Clinic was named the 2015 Non Profit of the Year by Cabarrus Regional
Chamber of Commerce.
