Phase II area. A partial survey was performed and deficiencies are noted in the following report(s). This is not a final or complete issues list. The deficiencies listed are examples of areas of concern and review by the Commissioning Agents. The next scheduled visit for functional testing and room surveys is for the week of July 9, 2012. The expectation of the Commissioning Team is that all work will be finished, the controls issues will have been resolved with all systems tested by installation contractors and previous Commissioning Issues Log will have been reviewed and all items corrected by contractors or accepted by district. Roof debris needs to be cleaned up. The amount of debris left on the membrane roof is a significant risk of punctures and leaks and not acceptable. The entire roof area needs to be surveyed and free of debris.

DAVENPORT K-8 SCHOOL

Date: June 28, 2012 Site: Davenport K-8 School, Davenport School District

Weather: Clear, mid 60s Activities: HVAC Functional Testing and Commissioning Back Checks Commissioning Team Russ Chambers and Rick Alexander visited Davenport Elementary School on June 27 & 28 to perform Functional Testing and Room Surveys in preparation of project close out. Summary: Phase II HVAC equipment was tested to assure functionality per project specification and sequence of operations. Issues identified in Phase I testing were back checked and room and exterior surveys were performed. The testing went well with few issues observed. At this time all mechanical equipment and controls are functional. There is an air flow with one heat recovery unit. The mechanical contractor is in the process of submitting an RFI to determine resolution. Issues found which were not resolved while the Commissioning Agents were on site are listed below and have been added to the Commissioning Issues Log.

Specific Observations and Comments: • Stage lighting – light #2 does not work (2nd light from East end) • Two damaged ceiling tile in entry commons

• Room 206 – Bad fluorescent light outside row South end • Library 205 – Bad fluorescent light in row above Southeast door • Room 113 – Fluorescent light above window has very dirty diffuser lens • Room 111- Fluorescent light North row 3rd from West has 1 lamp out • Room 109 – Bad fluorescent light South row 2nd from West end • Heat Pump Loop Pump #2 is very noisy. Sounds like a bad motor bearing on

the inboard end • Heat Pump Loop discharge side pressure gauges need to be changed out for

100psi gauges. Project documents call for 60psi gauges and that is what was installed, however they are running “pegged” beyond the 60psi mark. For accurate measurement the gauges should be changed.

• Area 1 roof flashing at the “pop up” needs to be checked out for proper installation. The ridge cap is installed over the flashing and the flashing corners are not fully caulked. It is the Commissioning Agents opinion that the flashing is to be installed over the roofing.

Weather: Clear, mid 90s Activities: Commissioning Back Checks Commissioning Agent Russ Chambers visited Davenport Elementary School on July 7 to perform Commissioning Issues Log backchecks preparation of project close out. Summary: Most outstanding Commissioning issues have been corrected. Two new issues were identified and described in the body of this report. Issues found which were not resolved while the Commissioning Agent was on site are listed below and have been added to the Commissioning Issues Log.

Specific Observations and Comments: Issues Log:

• Item 21 – Science Room floor Drain - This issue has been on the log since August 2011. I would expect there will be no change to floor contour. Need disposition per issues log instruction page.

• Items 26-29 – Basketball hoop height - Minor issues, installation contractor has been notified and is working the time for adjustment into schedule.

• Item 57 – Stage Lights – The #2 light is not working. At re-test the remote panel would not work and main controller had an error. Should be checked by knowledgeable person. May have been operator error.

• Item 64 - #2 Heat Pump Loop Pump – the pump motor has been worked on since the June 27 issues log, however it is still has excessive vibration and the pimp bearing is running hot. If this issue is not correctly resolved it will cause premature pump failure. It needs to be properly investigated and resolved.

• Item 74 – Asphalt Repairs – Repaired areas are poor quality and do not match surrounding area. Area around drain will cause water pooling, area by door will allow water penetration and eventual cracking and breakouts.
