Draft Dates
Choose any day between the 1st - 28th or a day/week to coincide with bank deposits.
Complete the One Time Electronic Transfer for immediate draft of first premium (cash with app).
If using an application that does not include the Week / Day option, please make a note in
the Remarks section, i.e., “Draft the 3rd Wednesday of each month beginning in August.”
Thirty Day Right to Cancel
NOTICE OF THIRTY DAY RIGHT TO EXAMINE POLICY
If this policy is unsatisfactory, the Owner may return it within
thirty (30) days after receiving it. The Policy should be
returned to the Company, the Agent through whom it was
purchased, or any Agent of the Company; then all premiums
paid will be refunded and the Policy will be deemed void
from the beginning.

Avoiding PHIs
Please pay special attention to Section C, Question 6 on the application.
We need complete details for all ”yes” answers, including the treatment,
diagnosis, dates and duration.
• High Blood Pressure: Provide the date diagnosed and treatment,
including name of medication and last blood pressure reading.
• Diabetes: Provide the date diagnosed and treatment, including name of
medication, last fasting blood sugar and/or Hemoglobin A1C reading, and
list any hospitalization or complications in the last 10 years.
Please be sure to list the proposed insured’s height and weight (Section B,
Question 4). This is easy to miss.

consulted a physician or medical facility for any reason?”
