A very warm welcome to all of you to the Annual Scientific Meeting (ASM) of the College of Surgeons, Academy of Medicine of Malaysia, to be held at Shangri-La Hotel in Kuala Lumpur, from 13th to 15th May 2016.
This year, our ASM is special in many ways. The theme, taken from the Greek Mythology “Between Scylla and Charybdis”, describes the dilemma of today’s surgeons when they face critical moments in “Navigating the Treacherous Waters of Modern Surgery” whereby patients’ lives are at risk. Surgeons dread to deal with high risk patients undergoing high risk operations. In a rural setting, a surgeon may also have no choice but to carry out emergency or salvage operation even though he has not been specifically trained for it.
In this modern world, surgeons are expected to keep abreast with the latest advances and best practice, and to stay on par with their peers. With the increased expectation and demand from patients or relatives, they can be pressurised into new treatment options which may not be the best for the patients.
This ASM serves to update our surgeons and trainees. The workshops provide hands-on opportunity for participants to enhance their operative skills. The sharing of ways and means on how best to care for our patients will help promote professional integrity and responsibility towards delivering the best and safe practice.
This ASM covers several areas of interests. A symposium is dedicated specifically to “Women in Surgery”, in recognition of their contributions. The summit meeting of the ASEAN Federation of Surgical Colleges is held for the first time in Malaysia. The Royal College of Physicians & Surgeons of Glasgow is participating in this meeting by having a surgical symposium. This meeting incorporates the 3rd Asian Trauma Congress and a separate one-day programme for the nurses and medical students has also been incorporated.
This gathering of experts, surgeons, doctors, medical students and allied medical staff will provide an opportunity for the sharing of knowledge and experience. It serves as a forum for regional and international collaboration, and helps to foster fellowships and networking.
I hope you will have a great time here. Malaysia has a rich cultural heritage with multi-ethnicity and multi-cultural base, and offers a wide variety of Asian culinary. We hope you will take time off to visit the places of interest both in and outside of Kuala Lumpur.

The past year has been a difficult one for surgeons. While ‘primum non nocere’ remains the cornerstone of our practice, the interventional nature of surgery can result in surgeons being held accountable, even for recognized complications, by disgruntled patients. Trial-by-media increasingly shapes public perception of a surgeon’s ability and conduct. Surgery is painted in shades of black or white, when it is actually many shades of grey, a fact that the judiciary is not always equipped to recognize.
Even our interactions with peers and subordinates have been placed under scrutiny. Surgery, it seems, is a hostile environment, with a culture of discrimination, sexual harassment and bullying, where working time directives may have adversely affected surgical training while not preventing burnout. The profession is facing attack from every conceivable direction. Unaddressed, fewer and fewer will contemplate surgery as a career.
It is a surgeon’s nature to battle on, and it was during a particularly trying time last year that I was reminded of another battle: Odysseus, the Greek hero, steering his ship between two perilous foes – Scylla, a vicious multi-headed monster, and Charybdis, a cavernous deadly whirlpool. Passing too closely to either would mean certain death.
This is the dilemma often faced by surgeons, whether deciding the adoption of new technologies, maintaining surgical standards, ensuring the well-being of trainees, working in multi-disciplinary teams without losing autonomy, navigating work-life balance, conducting ethical research or grappling with complex medicolegal issues. Do, or do not – both could have fatal consequences for a patient, or a career.
Ironically, neither Scylla nor Charybdis were originally monsters, but cursed by selfish gods denied their desires. Our solutions, therefore, must be multi-pronged: improve ourselves, educate others, and work with our perceived foes to reach a common goal: better patient outcomes, reduced surgeon burnout and better quality of life for all.
To that end, the Organising Committee has assembled a comprehensive programme addressing each of these issues, with stellar speakers from near and far. And to celebrate the Golden Jubilee of the Academy of Medicine of Malaysia, the College of Surgeons is proud to present an expanded Congress programme.
We have an unprecedented number of pre- and post-congress workshops; plenaries and symposia addressing critical issues in clinical decision-making, research and training; international collaborations with the Royal College of Physicians and Surgeons of Glasgow, the Royal College of Surgeons of Edinburgh, the ASEAN Federation of Surgical Colleges and the Asian Trauma Congress; and, for the first time, concurrent programmes for medical students and nurses. Our hugely popular meet-the-expert sessions return with topics ranging from controversies in breast-conserving surgery to the thorny problem of faecal incontinence. Issues regarding the dearth of women in surgery will be addressed in an inaugural half-day symposium.
We are greatly honoured to have some very special guests: the Malaysian Director-General of Health, Datuk Dr Noor Hisham Abdullah, who has graciously agreed to officiate; Dato’ Dr J C Mehta, our esteemed AM Ismail Orator, a pioneer in Malaysian surgery; and Malaysia’s Iron Lady herself, Tan Sri Rafidah Aziz, who, as a past Minister of Trade and Industry, is well-placed to deliver the Golden Jubilee Plenary.
Finally, we hope to have the pleasure of your company at our Greek-themed Gala Dinner. Dust off your chitons and peplos, and put on your laurel wreaths to be our best-dressed for the night! With special performances and a sumptuous buffet, it is our small way of thanking you for taking time out of your busy schedules to be with us.

Dato’ Dr Jasper Cuthbert Mehta is an illustrious giant of Malaysian Surgery. Born in 1934, he qualified in Medicine at the University of Malaya, Singapore, in 1958, and obtained the Fellowship of the Royal College of Surgeons of Edinburgh, in 1966. He served as Consultant Surgeon in many states in the country culminating with his appointment as Head of Surgery for the nation when he was posted to Hospital Kuala Lumpur, in 1983. Dato’ Mehta was President of this College from 1993 to 1996.
Throughout his career, he introduced many surgical procedures and specialised clinical services. More importantly, he trained and motivated numerous surgeons who are now senior surgical leaders themselves.
Dato’ Mehta possesses many admirable qualities. He is respected for his ethics, integrity and the willingness to learn from all, including those many years his junior. In fact, I first met Dato’ when I was working under Professor MMS Krishnan at the University Hospital, Kuala Lumpur, in the mid 1980s. Dato’ Mehta, then head of national surgical services with the Ministry of Health, had come to observe and learn operative techniques in transhiatal oesophagectomy.
This enthusiasm to constantly acquire new knowledge and skills, and to share his vast experience and wisdom, continues unabated today, when he has the responsibility of being Person-in-Charge of a large private hospital in Kuala Lumpur.
Our surgical fraternity recognises Dato’ Mehta as an inspiring doyen and a true friend.

WORKSHOP DESCRIPTION
In laparoscopic surgery, intracorporeal suturing is an advanced skill. Mastering this skill is difficult and there is a steep learning curve. However, it is essential that every laparoscopic surgeon possesses this skill in their armamentarium as it can help them get out of difficult situations. Many procedures are limited by this step as well. If a surgeon can suture well inside, most procedures can be done laparoscopically.
This one-day intensive practical workshop aims to address this issue by helping the participants progress from basic to advanced laparoscopic suturing. The workshop is delivered by experienced laparoscopic surgeons, providing laboratory-based practice of basic and advanced laparoscopic suturing using dry and wet models.

The main objective of this one-day workshop is to update Malaysian surgeons on the evolving treatment of breast and colorectal cancer. The lectures and interactive sessions include updates on screening, diagnostics, surgical margins and axillary staging in breast cancer and role of screening, diagnostics, neoadjuvant chemo-radiation and surgery in colorectal surgery.
A parallel session on evolving roles and responsibilities of cancer care personnel and an introductory course on breaking bad news and risk communication skills will be included.
Breast and colorectal cancer make up 30% of all cancers in Malaysia. The talks and interactive sessions include updates on surgical margins and axillary staging in breast cancer and the role of screening and treatments in colorectal surgery. The evolving roles of nurses in breast care and stoma nursing will be presented. Malaysia aspires to developed nation status by the year 2020. Nevertheless, provision of healthcare still lags behind that expected of developed nations in a number of areas. In particular, Malaysia is critically short of specialist surgeons, with a ratio of approximately 0.3 per 100 000 population, as compared to the US, which has 8 per 100 000 population.
While more than 5000 junior doctors are currently being produced every year, women form less than 10% of surgeons in Malaysia, and less than 20% of surgical trainees. This is despite medical school enrolments of women comprising more than 60% of the total intake. It is critical that we address the reasons why very few go on to a career in surgery, whether it’s work-life balance issues, gender discrimination, bullying etc. if we are to ensure adequate surgical services for the nation in the future.
It is with this in mind that the College of Surgeons Annual Scientific Meeting in 2016 (concurrent with the Academy of Medicine’s Golden Jubilee) is organising a half-day symposium looking at the current status, obstacles and issues, as well as developments in future, with regards to redressing the balance of women in surgery. A distinguished panel of speakers and mentors has been assembled, to share their experiences coming through the ranks, to help generate discussion from students, trainees and junior surgeons on strategies and tips for success. It is hoped that this would culminate in the establishment of a Women’s Chapter within the College, to help mentor trainees to achieve success.
