SPEECH-LANGUAGESERVICES

TheMasterofSciencedegreeinCommunicationDisordersandSciencespreparesstudentstoworkwithindividualswithspeech,languageandhearingdisorders.EarlypracticumtrainingisofferedthroughtheSIUCClinicalCenter(CC)andtheCenterforAutismSpectrumDisorders(CASD),twofacilitiesthatprovidedirectcareservicestothecommunity.TheCCprovidesawiderangeofspeech-languagediagnosticandtherapeuticservices.TheCASDspecializesinservicestopersonswithautismspectrumdisorders.Allactivitiesrelatedtospeech-languageservicesareperformedincompliancewithASHA,HIPAA,StateofIL,University,andCDSgraduateprogramrequirements.AllclinicsupervisorsareASHAcertifiedandlicensedtopracticeinIllinois.AllpractitionersfollowtheASHACodeofEthicsandScopeofPractice.CDSgraduatestudentsareextendedclinicprivilegesthroughtheirenrollmentingoodstandingintheprogram,andinCDS594&595.Studentswhodonotmaintainenrollmentingoodstanding,orwhodonotcomplywithclinicalpoliciesandprocedureswillhavetheirprivilegesrevoked.ClinicalCenterContactInformation:SIUCClinicalCenter Phone:618-453-2361625WhamDrive Fax:618-453-6130Room141 Carbondale,IL62901 Longdistancecode:CenterforAutismSpectrumDisordersContactInformation:CASD625WhamDriveRoom124Carbondale,IL62901

CDSClinicalProgramPolicies&Procedures–AllCliniciansCredentialsofGraduateCliniciansClinicalprivilegesareextendedonlytograduatestudentsingoodstandingintheCDSprogramandwhoareenrolledinCDS594–AdvancedClinicalPracticum,andCDS595–ClinicSeminar.StudentsmusthavecompletedacriminalbackgroundcheckwiththeFBIandStateofIllinois,HIPAAtraining,andtheRehabilitationInstituteandClinicOrientationandTrainingsessions.Studentsretainclinicalprivilegesbycomplyingwiththepoliciesandproceduresasoutlinedinthismanual,and/orprovidedthroughothermeans(e.g.clinicseminar,supervisionmeetings,etc.).Clinicianswhoexperiencedifficultymaintainingtheserequirementsmayhavetheirprivilegesrevoked.Eachgraduatestudentmustpresentdocumentationof25ASHAobservationhourswithsupervisorsignatureandASHAIDnumber.Ifthesehoursarelacking,theymustbecompletedduringthefirstmonthofgraduatepracticum.ItisrequiredthatallstudentsenrolledinCDS594&595havecompletedallrequiredundergraduatecourseworkwithafinalgradeofBorbetter.AgradeofCorbelow,oragradeofINCinanycourserelatedtoadisorderarea,mayresultinaninterruptioninclinicassignments.Studentsmustsuccessfullycompletealldisorder-relatedcourseworkpriortoprogressingtointernship/student-teaching.StudentswhoearnedaCorlower,orwhohaveINCinanydisorder-relatedcourse,willnotbepermittedtoenrollinCDS597–PublicSchoolPracticumor598–InternshipinCDS.ClinicAdvisementTheClinicCoordinatorreceivesinformationabouteachstudent’sclinicalperformancefromclinicsupervisors,throughoralorwritten(descriptiveorgrade)reports,orthestudenther/himself.Advisementtostudentsabouttheroutineprocessthroughtheclinicalsequenceistheresponsibilityofthecliniccoordinator.Thisisaccomplishedthroughclinicseminars,individualappointmentsasneeded,andelectronicstorageofASHAhours.Ifastudentexperiencesproblemsintheclinicalprogram,his/herfirstredressistospeakwiththeclinicsupervisormostcloselyassociatedwiththesituationinwhichthedifficultyisexperienced.Iftheproblemisnotresolvedatthislevel,thenthestudentmaycontactthepersonsbelow,preferablyintheorderlisted:

StudentshaveavailabletothemtheAcademicGrievancePolicy/ProceduresasdetailedintheGraduateSchoolCatalog.ClinicalStandingAgraduatestudent’sperformanceinclinicalcoursesisroutinelyreviewedatmidtermandfinalgradingperiods.AfinalgradeofBorbettermustbeearnedineachclinicalassignmenttopassthecourse.IfatanypointduringpracticumthesupervisorevaluatesthestudenttobeperformingatagradeequivalencyofCorlower,informalorformalremediationmaybegin.PleaseseetheCDSAstudentmayloseclinicalprivilegesifhe/she:

1. Isonacademicprobation,orhaveanexcessivenumberofINCgrades.2. EarnsagradeofCorlessinclinicalcoursework.3. ViolatestheSIUCStudentConductCode.4. Engagesinunethicalbehavior.5. ViolatestheASHACodeofEthicsand/orScopeofPractice.6. Violatesthepoliciesandproceduresofthismanual.7. Demonstratesnon-compliancewithanypolicies/proceduresthatmayapply.

ClinicalPracticumTheminimumpracticumrequirementsforallSIUCgraduatestudentsmeetASHAcertificationrequirements.AllhoursrequiredbyASHAmustbecompletedpriortograduation.ASHArequiresaminimum400hoursofclinicalhoursforcertification.Ofthese400,25mustbeobservation,and325mustbeaccruedatthegraduatelevel.Amaximumof50hoursaccruedattheundergraduatelevelmaybeconsideredinthe400total.Undergraduatehoursareonlyconsideredifastudentisshortgraduatelevelhours.StudentswishingtotransferclinicalhoursintotheSIUCCDSprogrammustrequestfromtheirpreviousprogramauthorizedcopiesofpracticumandobservationhours,andsubmittothecliniccoordinatorforfiling.StudentsmusthavethreeenrollmentsinCDS594atSIUC,buthoursaccruedelsewherecanbeusedtosupplementtotalASHAhoursearned.Thereisnominimumnumberofhoursrequiredinanyspecificdisorderarea,age,severity,orculturaldiversityofclients.TheSIUCCDSgraduateprogramwillattempttoprovideeachgraduateclinicianwithabreadthofexperiencethatspanstheexpectationsasimpliedinASHAStandard3.0B.OverthecourseofthreesemestersofCDS594,andonesemestereachofCDS597&598,studentswillgain

experiencewithawidevarietyofclientsfordiagnosisandtreatmentineachofthe“ASHABig9”disorderareas.TheClinicCoordinatormonitorsassignmentsbothon-andoff-campuspracticatoensurestudentsreceivethemaximumbreadthofexperiencespossible.ClinicalTrainingSequenceDuringthecourseofgraduatetraining,studentsparticipateinfivesemestersofclinicalexperiences.Thetypicaltimeframeisoutlinedbelow.However,thisgeneralsequencemaybechangedtoaccommodateindividualneeds,perrequesttotheClinicCoordinator.Semester Course CreditHoursFirst CDS594 3 CDS595 1Second CDS594 3 CDS595 1Third CDS594 2

CDS595–ClinicSeminar:Allseminarsaremandatory.Topicsareofferedonlyonceperyear(thefall,springandsummerscheduleoftopicsalldiffer,anddonotrepeat).Topicscoveredareofprofessionalinterestandnecessity,asmandatedbytheAmericanSpeech-Language-HearingAssociation,theStateofIllinois,andtheSIUCCommunicationDisorders&Sciencesprogram.CDS597–PublicSchoolPracticum:StudentsseekingtheStateofILProfessionalEducatorLicensewithSchoolSupportPersonnelEndorsementforSLP:Non-teachingwillenrollinCDS597duringtheirfourthorfifthsemesterinthepracticumsequence.Thisisafull-timeexperienceduringwhichthestudentworksunderthesupervisionofacertifiedandlicensedspeech-languagepathologistinapublicschoolsetting.Thestudent-teacherwillparticipateinarangeofprofessionalexperiencesexpectedinapublicschoolsettingandacquiredirectcontacthoursineducation-based,pediatricassessmentandtreatmentofcommunicationdisorders.Student-teachingisafull-timeexperience,whichmayrequirethattheclinicianresideinareasdistantfromcampus.Meetingstandardsforpublicschoollicensureisnotaprogramrequirement;however,intheStateofIllinois,publicschoollicensureisrequiredinordertopracticespeech-languagepathologyinthepublicschoolsetting.MoreinformationaboutthePELrequirementsareoutlinedlaterinthismanual.CDS598–InternshipinCDS:Allgraduatestudentswillenrollforaminimumofonesemesterofinternshipoutsidethepublicschoolsetting,andmayenrollunderthiscoursenumberforuptotwosemestersifnopublicschoolexperienceisdesired.Studentsenrolledinthiscourseareassignedtositesoutsideofthepublicschoolsystem,whichtypicallyincludehospitals,rehabcenters,skillednursingfacilities,privateclinics,homehealthcare,earlyintervention,privateschools,andprivatepractice.Internshipplacementsmustberequestedduringthefirstsemesterofenrollmentinpracticum(CDS594).Alistofactiveinternshipsiteswillbemadeavailablefromthecliniccoordinator.Studentsmayrequestinternshipsitesnotlistedonthecurrentmasterlist,buttherequestshouldbeeitherinageographicareawheretherearenocurrentsitesavailable,orthenewsitewillprovideadistinctlydifferentexperience.Allrequestsandassignmentsaregrantedatthediscretionofthecliniccoordinator.Priortooff-siteinternshipassignments,thecliniccoordinatormustapprovethesiteandstudentplacement.Onoccasion,astudentmayberequiredtoremainatalocalsitesothatthecliniccoordinatormayprovideadditionalsupervisorysupport.

IllinoisPublicSchoolLicense-StateofILProfessionalEducatorLicensewithSchoolSupportPersonnelEndorsementforSLP:Non-teachingStudentswhowouldliketoholdthecredentialnecessarytoworkinanILpublicschoolneedtocompleteadditionalrequirements.TheStateofILProfessionalEducatorLicensewithSchoolSupportPersonnelEndorsementforSLP:Non-teaching(PEL)permitstheSLPtoworkasanitinerant,and/orprovideservicesotherthaninaself-containedclassroom(i.e.“push-in/pull-out”therapies).StudentswishingtocompletethePELwillidentifythemselvestothecliniccoordinatorandhaveaseparateadvisementmeetingtoreviewrequirements.PlansforcompletingtheserequirementsshouldbedonecollaborativelybetweenthecliniccoordinatorandtheCDSacademicadvisor.ClinicalPracticaAssignmentsAllclinicalpracticaassignments(i.e.CDS594,595,597,598),aremadeatthediscretionofthecliniccoordinator,inconsultationwiththefaculty.Studentsmaynotrefuseclient/siteassignments,unlesssuchanassignmentpresentsaconflictofinterest.Clinicschedulesaremadeinconsiderationofclassesandclientappointments.Itisimpossibletoaccommodateeachindividualstudent’spersonalschedule.Likewise,internshipassignmentsareultimatelyatthediscretionoftheCDSprogram.Studentsareencouragedtoinformthecliniccoordinatoroftheirclinicalinterests;everyeffortwillbemadetoaccommodate.Students,whoareunabletoacceptclientassignments,orattendotherrequiredclinicalactivities,willbegiventheoptiontodelayenrollmentinclinicalpracticumuntilthefollowingsemester.ProfessionalConductClinicianswillmaintainthehigheststandardsofintegrityandethicalprinciplesinordertoensuretheresponsibledischargeofservices.ClinicianswillbefamiliarandactinaccordancewiththeCodeofEthicsandScopeofPracticesetforthbyASHA.Followingaresomehighlightedprofessionalbehaviors.Thisisnotanexhaustivelist.CliniciansarerequiredtoapplytheASHACodeofEthics,ScopeofPracticeandprofessionaljudgmenttoallsituations.

Confidentiality:Allinformationregardingaclientisconfidential.Informationisreleasedtootherpartiesonlywiththewrittenconsentoftheclientorguardian.Allmaterialscontainingidentifyinginformationmustremainonthepremises.Emailcorrespondencemayonlycontaintheclient’s

initialsorI.D.number.Pleasedonotdiscussyourclientwithanyoneoutsideoftheprogram,andnotinpublicvenues.

StudentObservationofSessions:Graduatestudentsareencouragedtoobservetheirpeers’sessions.Whenintheobservationroom,pleaserefrainfromdiscussingtheclientwithanyoneotherthanthesupervisor,andonlywithpriorapproval.Nevercommentonanythingtoanyoneelseinthepresenceofafamilymemberorcaregiver.Studentsaregivenprioritytoobserve.Pleaseinformthesupervisorsothattheyareawareofyourpresencebeforehand,andcanmakeaccommodation,asthemaximumnumberofobserversintheroomis4.

Relationships:Toavoidconflictofinterest,clinicianswillrefrainfromtreatingfamilymembersorclosefriends/associates.Theclinicianwilladvisethecliniccoordinatorshouldsuchanassignmentoccur.Clinicalrelationshipswillremainprofessional.Ifapersonalrelationshipappearstobedevelopingbetweenyouandyourclient,theclinicianwillinformthecliniccoordinatorandadecisionwillbereachedtoprotecttheobjectivityandjudgmentoftheclinician,andtheintegrityoftheclinicalassignment.Studentsareprohibitedfromsocializingwithclients,andfromprovidingpersonalfavorssuchasbabysitting,transportation,loans,etc.

Attire:CliniciansintheCCwilldressmodestly,in“businesscasual”attire.ItisrecommendedthatstudentspurchaseCCshirts/blouses,whichmaybewornwithpants/slacks(nodenimplease).CliniciansassignedtoCASDwillfollowthedresscodeofthatagency.Shirts/blouseswillcovertheentiretorsotothewaist,includingasleeve.Necklineswillbemodest.Pantswillrisetothewaistandlengthwillbenoshorterthantotheknee.Theentiretorsowillremaincovered,regardlessofmovementorposture.Shoeswillbecomfortable.NosneakersintheCC.Jewelryshouldbeminimalandsimple.Avoiddangling,sparklingorotherjewelrythatmaydistracttheclient,and/orresultintheclientattemptingtotouchorremoveit.Visiblebodypiercingstoareasotherthantheearshouldberemovedduringclinicalactivities.

Substanceuse:Alcoholandillegalsubstancesareprohibitedinclinicalsettings.Studentsmaynotconsumealcoholorillegalsubstancespriortoclinicalsessions.

Weapons:Anyitemthatmaybeconstruedasaweaponisprohibited.Sprayscarriedforself-defensemayremaininyourpossession;however,pleasesecurethecanistersothatclientsdonothaveopportunityforaccidentalmisuse.

Food/Drinks:CCcliniciansmaynotconsumefood/drinksinsidetherapyrooms.Onlyfooditemswhichareusedfortherapeuticpurposesmaybebroughttotherapyrooms.CASDstudentswillfollowthepoliciesofthatagency.

Cellphones:Useofacellphonefornon-emergencypurposesisprohibited.Leavetheclinicareawhenusingacellphone.

ReceptionArea:Pleasedonotusereceptionareasorthehallwaystoconductconsultationsortosocialize.Consultationswillbeconductedinaprivateoffice;socializingisconductedoffclinicalpremises.

Gossip:Thefunctionofgossipistohurtthereputationofanotherperson.Informationaboutthepersonalsituationofaclient,peer,oranyotherpersonshouldbesharedonlyonaneed-to-knowbasis.Usegoodprofessionaljudgment,andrefrainfromengaginginsuchdiscussions.

SocialMediaPostings:Nostudentwillpostcommentsonanysocialmediaoutletaboutclients,peers,faculty,supervisors,stafforanyotherpersonrelatedtotheCDSprogram,theClinicalCenterorCASD.Whensuchpostsarenegative/criticalinnature,thisreflectspoorlyonyou.Publicstatementsofcriticismindicatealackofprofessionaljudgment,andmaybeindicativeofastudent’sneedtoremediateandmatureintoreadinessforclinicalpractice.

Fragranceuse:Forthecomfortofourclients,clinicalfacilitiesarefragrance-free.Pleaserefrainfromtheuseofperfume/cologneasitmaycausesensoryresponsesfromclients.

Smoking:Nosmokinganywhereoncampus.Pleasedonotcometotherapysessionswiththesmellofsmokeonyourperson.Thismayhaveanegativesensoryresponsefromyourclients.

Physicalcontactwithclients:Physicalcontactwithclientsisonlyasnecessarytoassist,re-directorpreventinjurytotheclientand/orothers.Itispermissible,infactrecommended,toholdachild’shandbetweenthereceptionareaandthetherapyroom.Itisalsopermissibletoprovidegentle,

discreet,andlimitedphysicalcuestoredirectorpreventabehavior.Consultwithyoursupervisorregardingindividualclientneeds.

Toiletingassistancewithclients:Consultwithyoursupervisorpriortoconsultingwiththeclient’sparentregardingtoiletingpreferencesandskilllevel.Ifthechildneedsassistance,theparentmustremaininthewaitingareatobeavailablefortoiletingneeds.Ifanunexpectedsituationoccursandnoparentisavailable,seekanotherpartytoaccompanyyouandthechildtotherestroom.Ifanotherclinicianisnotavailable,askanysupervisorfromanydisciplineorPaulaChapatohelpyou.Pleasedonotasksupportstaff.Followtoiletingroutinesasinstructedbytheparent.Ifthechildisoftheoppositesex,andthechildisnotindependentintoileting,usethebathroomofyourownidentifiedsexafteritiscleared.Wheneveryouareassistingaclient,keepthemainbathroomdooropen,andthesecondpartyinviewoftheprocess.Theseprocedureswillprotectyoufromanypossiblemisrepresentationsofinteractionsbetweenyouandtheclient.

CLINICIAN-RELATEDDOCUMENTATIONFORASHACERTIFICATION

EachclinicianwillmaintaintheaccrualofASHAhours.Studentswillestimatedirectservicehourstothenearestquarterhour.Pleasedonotuseincrementssmallerthanone-quarterhour.DirectServiceConversionTable15minutes = .2530minutes = .5045minutes = .7560minutes = 1.0ASHAStandard3.0BFormAllstudentswillmaintaintheirASHAhoursonthisform.Studentswillmonitoraccrualofhoursandreporttothecliniccoordinatoranydeficienciesthatneedtobeaddressed.Thisformissubmittedeachsemesterattheconclusionofpracticum(includinginternship).Attheconclusionoftheclinician’sfifthandfinalsemester,theclinicianwillalsosubmitacumulativeform,representingallhoursaccruedthroughouttheprogram.Theonlyvalidapprovalofclinicalhoursfordepartmentalpurposeswillbethesignatureofthecliniccoordinator.ThecliniccoordinatormaintainstheseformswiththeCDSprogram.

ClinicalActivityScheduleTheClinicalActivityScheduleisascheduleofallclinic-relatedactivitiestakingplaceduringthesemester.Itiseachclinician’sresponsibilitytoreadthescheduleandparticipateinrequiredactivities.AppraisalofClinicalCompetenceTheAppraisalofClinicalCompetenceisanonlinetoolusedtoevaluateeachclinician’sperformanceatmidtermandattheconclusionofthesemester.Theevaluationwillreflecttheclinician’scurrentlevelofcompetence,notacumulativeoraverageassessment.AgradeofBorhigherisneededtopasspractica.SupervisorEvaluationCliniciansinallpracticumcourseswillsubmitanevaluationoftheirsupervisorattheendofeachsemester.Theseevaluationsaresubmittedtothecliniccoordinator.Theyareanonymoustothesupervisor.Thecliniccoordinatorreviewstheevaluationswiththesupervisorforthepurposesofprofessionaldevelopment.ClinicCheck-OutEachsemester,allclinicianswillbeclearedtoreceiveafinalgradeforclinicalpracticumbycompletingclinic‘check-out.’Attheconclusionofpracticum(includinginternship),clinicianswillsubmit:

ClinicAssignmentsClinicassignmentsaremadeatthediscretionofthecliniccoordinator.Generally,clientappointmentsarescheduledMonday-Friday,8:00am-5:00pm,inconsiderationofthegraduateclassschedule.Eveningappointmentsmaybescheduledatthesupervisor’spreference.Clinicappointmentsbeginthethirdweekofthefallsemester,thesecondweekofthespringsemester,andtheseconddayofthesummersemester.Titanium(Ti)ClinicianswilldocumentallreportsinTitanium,ourelectronicdocumentationsystem.TitaniumisaccessiblefromCCcomputersonly.WeeklyGroupandIndividualSupervisoryMeetingsClinicianswillattendweeklygroupsupervisionmeetingsasscheduledbythesupervisor.Thesupervisormayalsorequireadditionalindividualmeetingsasdeemednecessary.InitialContactwithClientAssoonasassignmentismade,clinicianswillcontacttheirclienttointroducethemselvesandconfirmthefirstsessionappointment.Contactmaybemadebyemail,(ifsuchisprovided)orbyCCtelephone.Additionalinformationmayalsobeobtainedatthistime(e.g.additionaldetailsregardingcommunicativecomplaint,preferredactivitiesorreinforcersforchildren,separationstrategies,etc.).Donotuseyourpersonalphonetocontactclients,anddonotgiveclientsyourpersonalphonenumber.PleaseusetheCCtelephones(availableintheMaterialsLibrary)onlytocommunicateviatelephonewithclients.Youmayonlyuseyoursiu.eduemailaccounttoexchangeemailmessageswithclients.AbsencesfromClinicTherearethreepossiblereasonswhyasessionmaybecancelled:(1)theclientcancelsthesession;(2)theclientdoesn’tshowupforasession;(3)thecliniciancancelsasession.

(2) Ifaclientis10minuteslateforasession,attempttoreachthembyphone.If

youareunabletodoso,waituntil15minutesafterthehour,thenconsiderthesessioncancelled.Scheduleamake-upassoonaspossible.Ifaclientisano-showtwice(notnecessarilyforconsecutivesessions),pleasebringthistotheattentionoftheClinicCoordinator.

(3) Cliniciansnevercancelsessionswithoutadvancepriorapprovalfromthesupervisor.Sessionsshouldneverbecancelledunlessunavoidable(i.e.seriousacuteillness)orforscholarlyactivities(i.e.ISHAorASHAconvention).Repeatedcancellationsmayresultinterminationofclientassignment.Asinsituations1&2,pleasemakeupthecancelledsessionassoonaspossible.

ClientsareawarethatconsistentattendanceisnecessarytoremainaclientoftheCC.Cliniciansmustaccrueanaverageof20-22hourspersemestertofulfillcourserequirements.Ifaclienthasfrequentabsences,pleaseinformthecliniccoordinatorsothatstepscanbetakentoassureyourcoursecompletion.Whenaregularlyscheduledsessioniscancelled,pleaseinformthefrontdesktodeleteyourappointmentandroomreservation,andtoprocessyourEncounterform.YouwillalsowriteaSOAPnoteaccountingforthemissedsession.ObservationPolicyforCaregiversThesupervisorhassolediscretionindeterminingwhowillbepresentintheobservationroom.Guardiansmaybeinvitedtoobserve,ifthesupervisorisavailabletoescortthemtoandremainintheroom.Ifacaregiverrequeststoobservethesession,pleasereplythatyouwillinformthesupervisor,whowilldiscusstheiroptions.RecordingSessionsClinicianswillrecordeachsessionandsavethemtothevideofolderontheharddriveoftheroomcomputer.VideosareerasedbytheDirectorattheendofeachsemester,unlesssheisinstructedotherwise.Client-RelatedDocumentationClient-relateddocumentation,suchasconsentforms,testprotocols,etc.,willbesubmittedtothetraybehindtheCCreceptionist.ThetrayislocatedinsideofthetopdrawerofthefilecabinetundertheprinterinWham141.AllclientdocumentationmustincludetheclientI.D.number.Alltestprotocolsmustbesignedbythesupervisor.

InformedConsentCliniciansmustobtainasignedconsenttorecordtheclient.TheInformedConsentformispresentedtotheclientjustpriortothefirstsessionandfilledoutinthereceptionarea.TheCCreceptionistwillserveasthewitness.Theformissubmittedtothetraybehindthereceptionistforelectronicfilingtotheclient’sfile.Thisconsentmustberenewedatthebeginningofeachsemester.Whenpresentingformsrequiringasignature,providetheformonaclipboard,andgivetheclientablackpen.PutanXnexttothelineswheretheclientmustsign,date,and/orinitial.Immediatelypresenttheformtothereceptionistforawitnesssignature.

AuthorizationforReleaseofInformationTheAuthorizationforReleaseofInformationformisanoptionalformusedwhenitisnecessarytorelease,obtain,orexchangeinformationwithsomepartyotherthanthelegalguardian.Theclientmustinitialthenatureofinformationtobedisclosedandthepurposeofthedisclosure.Theformmustbesignedandwitnessed.Theoriginalofthisformissubmittedforfiling.Acopyshouldbesenttothepartywithwhomtheinformationwillbeexchanged.Paulaorthereceptionistwillfirstapprovetheformpriortoputtingitinthetrayforscanning.

EncounterformTheEncounterformisaformsimilartothetimeofschedulingandbillingformthatmanyofyouseeatadoctor’soffice.TheEncounterformforyourfirstsessionwillbegeneratedforyou,andcanbeobtainedfromthereceptionistjustpriortoyoursession.EachsubsequentEncounterformisgeneratedbythedateofnextappointmentindicatedonthecurrentform.Afteryouseeyourclient,completetheencounterformasfollows:

• Completetheboxindicatingthetimeofthenextappointment.• ChecktheCPTcodethatmatchestheservicethatyouprovided(seesection

onCPTcodes).• Checkadiagnosticcategory.Pleasenotethatyoumayselect“Diagnosis

deferred”(799.9)orother(providecode)sinceyoumayneedtimetodeterminethecorrectdiagnosis.Youneedonlyselectonecodeevenifyourclientmeetscriteriaformorethanonediagnosis.

• Eachoftheseareasontheencounterformmustbecompleted.TheclinicianisrequiredtocompleteanEncounterformforeachscheduledsession.

ClientDispositionformPriortotheendofthesemester,eachclinicianwillcompleteaClientDispositionform.ThedistributionandcollectionoftheDispositionformisindicatedintheclinicalactivityscheduleeachsemester.Thisformisusedtoscheduletheclientforthefollowingsemester.Pleaseconsultwithyoursupervisorregardingrecommendationforcontinuedtreatmentordischarge.ProgressNotesProgressnotesarewrittenintheSOAPformat.Thesearesubmittedtoyoursupervisorwithin24hoursofthecompleted(orcanceled)session.Thefinaldraftisduetotheclient’sfilewithin5clinicdaysofthesession.SOAPstandsforsubjective,objective,assessment,andplan.TheClinicCoordinatorandyoursupervisorwilltrainyouinhowtowritethesenotes.EvaluationReportAnevaluationreportisrequiredforallnewcasesattheinitiationofservice.Thereportisduetothesupervisorwithinthreedaysfollowingtheeval.Thefinaldraftoftheevalreportisduetotheclient’sfilewithin10clinicdaysofthesession.TreatmentPlanThetreatmentplaniswrittenatthebeginningofthesemester.Youwillbegivenduedatesforfirstandfinaldraftsfromyoursupervisor.Typically,thefirstdraftisduetoyoursupervisorafterthethirdorfourthsession;thefinaldraftisduetotheclient’sfilewithin30daysofthestartoftreatment.ClosingSummaryAClosingSummaryiswrittenattheendofthesemesterforallclients.Youwillbegiventheduedatesforfirstandfinaldraftsfromyoursupervisor.TheClosingSummarymaybeareportofcontinuationofservices,oradischargefromservices.AClosingSummaryisalsowrittenforclientswhoaredischargedmid-semester.
