Who Are We...

Residential Parenting and Treatment Facility

established in 2007; The Mission of The Mustard Seed

Foundation Inc. is to empower teens, youth and families

to achieve their highest potential.

In 2009, The Mustard Seed Foundation Inc. launched Kardio

Kidz Camp The summer program promotes a fit, healthy an

active lifestyle for youth. Using the Motto, Healthy Mind/

Healthy Body our program focused on physical fitness,

team building, health and wellness activities, as well as

educational and cultural enrichment workshops.

The Mustard Seed Foundation Residential and Treatment

Facility exists to be a safe and nurturing environment to

promote healing for children, youth and families struggling

with mental illness, abuse and neglect, and homelessness.

2010, was the first year for the Mustard Seed Foundation’s

positive teen development initiative that includes hosting the

Annual Girls Conference in addition to community workshops

geared towards increasing self-esteem and peer relationships

among teen girls. My Sister’s Keeper

Our job training Jewel Cornerstone Journey and our

independent living program House of Ezekiel started in

2013; focus on mental health, behavioral health, substance

and alcohol abuse intervention and prevention encompassing

programs designed to address the core issues that cause

youth crime, child abuse and neglect and other

socio-economic factors that affect our most vulnerable

families.

The Vision of The Mustard Seed Foundation Inc. is to break

the cycles of generational poverty through supportive

programs that reach teach parenting skills and build

healthy stable families.

The Mustard Seed Foundation Inc. (TMSF) is the premier organization providing the highest

quality service to meet the physical, emotional, intellectual, cultural and spiritual needs of

children, youth and families. We accomplish this by:

Employing highly skilled professionals who work jointly with children, youth and families.

Provide services in a responsive, flexible and supportive environment.

Develop programs and maximize resources as a response to the needs of children, youth and families.

Utilize the talents and resources of the Board of Directors, Staff, and Volunteers to establish TMSF as an organization of

excellence.

Governing Body

TMSF Inc. welcomes and encourages the opportunity for

the family members, guardians and significant others to

participate as a member of the Board of Directors. All

interested parties are encouraged to solicit information

from the CEO regarding board recruitment practices and

timelines. All interested parties should make their interest

known by October. No current client may participate in

Board meetings. After 6 months of exiting ALL programs,

care, treatment and services former clients may

participate on any of the Associate Boards. Associate

Boards have direct linkage to the Executive Board of

Directors by way of their chairpersons.

Input from Individuals and Others

The Mustard Seed Foundation Inc. values input obtained

from stakeholders and will use it to create and provide

services that meet or exceed the expectations of the

persons served, the community, and other stakeholders.

We will solicit and collect stakeholder input through a

variety of mechanisms, including: annual client

satisfaction surveys, annual Community stakeholder

surveys, client suggestion box, complaint, grievance and

incident summaries. This information will be analyzed to

determine if the organization is meeting the current needs

of and offering relevant services to the persons served

and other stakeholders.

Weapons

Persons served, visitors, personnel, and all other

weapons of any kind while on TMSF Inc. property,

regardless of whether they are licensed to carry the

weapon or not.

Suicidal Thoughts

During your recovery, thoughts of suicide may occur.

PLEASE inform the treatment team of any suicidal

or self-harming thoughts you may have immedi-

ately! We (your treatment team) need to know if you are

experiencing any kind of suicidal or self-injurious thoughts

or behaviors in order to keep your recovery moving

Medication Problems

If you are prescribed medication as part of your treatment

and experience what you believe may be side effects,

please contact the agency immediately and ask to speak

with a nurse.

The Mustard Seed Foundation Inc. is a no seclusion, no restraint facility. We encourage

positive and leadership behavior. We have a reward system. Clients earn Seed Bucks for

following the High Five Principals. Every Friday, The Seed Store is open and you can shop for items with the Seed Bucks you have

earned for the week.

Background Checks

BCI/FBI checks performed on all staff and volunteers

Smoke Free Environment

Smoking of any kind will be prohibited on all property

owned by TMSF Inc. including buildings, grounds, parking

lots, and company owned or leased vehicles

Personal Safety Plan

A Personal Safety Plan will be developed with clients who

have been or who are exhibiting dangerous behavior.

The Plan will be individualized to identify the client’s

specific escalating behaviors, and how to respond to those

behaviors in a safe, effective, and clinically responsible

manner.

The purpose of the Personal Safety Plan is to ensure the

safety of the client, by identifying the client’s

individualized triggers, symptomatic behaviors, and

methods to safely and clinically assist the client.

The Personal Safety Plan will identify the following specific

and individualized information about the client:

Triggers (includes a risk assessment for

dangerous behaviors.

Current coping skills.

Warning signs.

Preferred interventions.

Advance directives, (if applicable)

The Personal Safety Plan is discussed and developed with

the client (or legal guardian if applicable).

The client, staff, and clinical supervisor sign and date the

Personal Safety Plan, and it is flied with the client’s ISP.

A copy is then provided to the client.

Notice of Privacy Practices This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

What is Protected Health Information (PHI) ? Protected Health Information (PHI) is information about your physical or mental health condition (past, present, or future), the provision of health care to you, or payment for that care, that includes your name, social security number, ID number, or other individually identifiable information. It may be written, electronic, or oral information. What will The Mustard Seed Foundation Inc. do with my PHI? It is the responsibility of The Mustard Seed Foundation Inc. (TMSF) to protect your PHI. By law, we must: 1. Protect the privacy of your PHI; 2. Tell you about your rights and our legal duties with respect to your PHI 3. Tell you about our privacy practices and follow our Privacy Notice currently in effect. We take these responsibilities seriously and, as in the past, we will continue to take appropriate steps to safeguard the privacy of your PHI. TMSF Inc. will use and disclose information about you to carry out treatment, payment, and health care operations. Both Federal and State laws govern how information is used and stored, what information is disclosed, and who gets this information. We will only disclose information about you that we are permitted to disclose. Treatment is the most important use and disclosure of your PHI. For example, your CPST, psychiatrist, nurse and other staff involved in your care use and disclose your PHI to diagnose your condition and evaluate your health care needs. For treatment purposes, we will not disclose information about you without your written authorization, except in circumstances that we reasonably believe are emergencies. For example, we may disclose information about you if you are in a hospital emergency room, and hospital staff request information to help them evaluate to treat you.. To receive payment for services, we disclose information about you to the Montgomery County Community Mental Health Board. It determines your eligibility, enrolls you in the County Behavioral Healthcare Plan, and pays us for services through the Board to the Ohio Department of Mental Health, the Ohio Department of Alcohol and Drug Addiction Services, and the Ohio Department of Job and Family Services. Your PHI may also be used or disclosed for health care operations, which refers to quality improvement activities, auditing, accreditation, licensing, and other activities that are required to meet our professional and legal obligations. For example, an auditor may see information about you, but we require that auditors agree to protect your information.

need to be a successful parent, to access needed

community resources and the skills needed to advocate

for your needs as well as the needs of your child.

Authorization to Bill

I hereby authorize The Mustard Seed Foundation Inc. (TMSF) to

submit claims to my insurance carrier and/or any Governmental

Agency.

I authorize the TMSF to release any information needed for the

processing of any/all claims. The Alcohol, Drug Addiction &

Mental Health Services (ADAMHS) Board is a payer of the

services which are funded through the Board. Member/Client

identifying information necessary to pay claims may be released to

the Alcohol, Drug Addiction & Mental Health Services Board, the

Ohio Department of Mental Health, the Ohio Department of

Alcohol and Drug Addiction Services, as well as any relevant

State Agency.

I hereby assign benefits for medical services and other clinical

services to TMSF and all payments of such benefits are made

directly to The Mustard Seed Foundation Inc. I agree to assume

full financial responsibility for payment of all charges not covered

by my insurance or designated as a deductible or co-payment by

my insurance company.

I understand that if I do not present all necessary insurance

information, I will be responsible for payment of all services

rendered. I certify that all information provided is true to the best

of my knowledge

Notice of Enrollment

To receive alcohol, drug addiction an/or mental health services paid for by pubic funds, you must provide information so that your Alcohol, Drug Addiction and Mental Health Services Board (ADAMHS) can:

Enroll you in the County Behavioral Healthcare Plan,

Determine if you are eligible for Publicly Funded Services

Pay the provider for your service through the Board to the

Ohio Department of Mental Health, the Ohio Department of Alcohol and Drug Addiction Services and the Ohio Department of Human Services. Information will not be available to other state departments, public or private organizations.

All information will be kept confidential, consistent with state and Federal law. Name identifying information will be used only to pay for services provided to you. Demographic information

will be kept without your name attached, and reported to the state departments and the Ohio Health Care Data Center. This information will not be available to other sources or used for other purposes. Billing information will only be kept for up to

Client Rights

Definition of “Client Rights Officer” A Client Rights Officer is one who investigates reported com-plaints, and helps to achieve suitable resolutions.

goal is to protect and enhance the rights of persons receiving services by educating both persons being serviced and staff of established rights and providing a procedure by which complaints can be made and resolved. Any client of TMSF has the option, at any time to file a grievance

with TMSF, or any outside organization, including Alcohol, Drug Addition and Mental Health Services Board, Ohio Legal Rights, Ohio Department of Mental Health, and the U.S. Department of

Health & Human Services—Civil Rights regional office in Chicago. It is the policy of TMSF to respect and uphold all rights given to clients by the Health Insurance Portability Accountability Act

Privacy (HIPAA) Law. These rights include: The right to timely access to the Individual Client Record

The right to request an amendment of information if the

Person served feels there is a mistake or omission. The right to choose how TMSF contacts the person served,

as long as we have the information needed to bill for services The right to request that TMSF restrict the use and disclosure

of information. The importance of making your grievance known is to

assure that the rights of those we serve are observed and re-spected and to provide a means for people receiving services to make complaints and have those complaints heard and acted on in a timely manner. A list of Client Rights, and the grievance procedure are posted in the dining room.

Consent to treat

I hereby give my consent to The Mustard Seed Foundation Inc. for the following services, as applicable: A. DIAGNOSTIC ASSESSMENT. This includes , but is not limited to (a) psychosocial assessment (b) psychological testing © psychiatric evaluation (d) assessment of abnormal involun-

tary movements (e) physical/neurological examination (f) lab

work (g) etc. B. MEDICATION/SOMATIC. This includes, but is not limited to (a) oral medication, (b) injectable medication, (c) generic, instead of brand name medication (d) seeing the doctor every ninety (90) days, (If applicable) etc. C. PARTIAL HOSPITALIZATION. This includes attending

Regularly scheduled treatment activities (for example 11:00 a.m. to 2:00 p.m. Tuesdays, Wednesdays). D. COMMUNITY SUPPORT PROGRAM. This includes, but is not limited to, coordination and advocacy of services at other agencies such as Rehabilitation Services Commission, Social Security Administration, County Department of Human Services, psychiatric hospitals, social service agencies, etc.

E. COUNSELING/PSYCHOTHERAPY. This includes a series of time-limited, structured, face to face sessions that focus on achieving, mutually defined therapeutic goals. 2. I am being informed of the risks and benefits of each of the above treatment, of alternative treatments, and I have the right to refuse treatment. 3. I am aware that I have the right to education and or assess-

ment of side effects of psychotropic medication, and I have been informed about alternatives to medication and result of not taking medication. 4. I am aware that I have the right to refuse treatment and The Mustard Seed Foundation Inc. response to the refusal or withdrawal of consent for treatment would include the following:

A. Reaffirmation of my rights to refuse treatment B. Efforts to develop alternative approaches collaboratively with me to ensure that I will receive needed services. C. Efforts to ensure that I understand the implication and potential consequences of refusing or withdrawing consent for treatment.

The Grievance Procedure

Works this way cont.

After the meeting, the Executive Team will send to you,

in writing, their findings and resolution to your grievance.

Resolution to your grievance must be made within

twenty-one calendar days from the time your grievance

was filed.

The Client Rights Officer will send a written acknowledge-

ment of the receipt of your grievance within 3 working

days of its receipt by The Mustard Seed Foundation Inc. It

will include the date your grievance was received, a

summary of your grievance, an overview of the grievance

investigation process, a timetable for completion of the

investigation and notification of the resolution, and the

treatment provider contact name, address, and telephone

number.

If you choose to take your grievance to an outside

organization, The Mustard Seed Foundation Inc. will

provide them with all relevant information about the

grievance if you so desire.

Confidentiality

We are dedicated to upholding the consumer's right to confidentiality of all information related to their treatment and service provision. We do not share any identifying consumer-related information unless we have written permission from the consumer or guardian or in cases of medical or psychiatric emergencies, or as permitted or required by law or regulation. Access to client records is limited to employees who are providing services to those clients or otherwise have the authority or specific permission to access those records. Staff will complete the Sharing of Information Form, in order to obtain or release information about any client. Staff will explain the form so the client can make an informed consent. Each request for information, including those received from other organizations, must comply with applicable federal and state regulations. All employees/contractors and Board members are required to retain and dispose of business documents and records in accordance with Record Retention Laws and the Record Retention and Destruction Policy. Referrals—if you have service needs that cannot be met by The

Mustard Seed Foundation Inc. we will assist you in finding appropriate help including referrals to other community service organizations, support groups specialized treatment and medical care. Emergency Services—are provided through affiliation with Samaritan Crisis Care What if I have an emergency? If you feel that you are having a psychiatric emergency during our normal business hours Monday through Friday, please call our offices to speak to a member of your care team immediately. If the emergency occurs outside of normal business hours, call any of the numbers listed below for assistance: Samaritan Crisis Care (937)-224-4646

Staff Expectations

The following are responsibilities you can expect

from the staff of The Mustard Seed Foundation Inc.

Listen to you without judging you, and accept you for who you are. Help you better understand your illness. Help you identify your strengths and use them effectively to address any

needs you may have. Recommend therapeutic activities between sessions. Teach you to relax, to cope with stress or to make decisions. Help you change your attitude toward yourself and others, and improve your

self-esteem. Point out patterns in your behavior or relationships that may be hindering your

Recovery. Help you take an honest look at the choices you’ve been making in your life to

determine whether these are healthy choices for you and those around you. Assist you in solving problems, but not fixing situations or making decisions for

you. Provide services in a professional manner. Ethical guidelines require no

dating , social, sexual, or business transactions either during or after your treatment

Decline any gifts that you might offer.

The Grievance Procedure

Works this way

It begins when you file a grievance with the Client

Rights Officer. The grievance must be in writing (the

Client Rights Officer can assist you with this) You

must sign and date it the grievance form.

If your grievance is with the Client’s Rights Officer a

member of the Board of Directors will be assigned as your

Client’s Right Officer.

The written grievance must clearly state the nature of

the complaint. It should include the date and time of the

incident as well as the names of all individuals involved.

There are no timelines for how quickly you must file your

grievance after an incident occurs; however, you are

encouraged to bring your complaints as soon as possible

to a Client Rights Officer. The Client Rights Officer will investigate the grievance

on your behalf, seeking a resolution to your grievance. If all involved parties are able to reach a resolution, the

written resolution will be given to you and the process will

be concluded. If a resolution is not reached, then the Client Rights

Officer will arrange for you to present your grievance to

the Executive Team, which is composed of The Mustard

Seed Foundation Inc. (Chief Executive Officer, Agency

Administrator and/or the Clinical Leadership Team). information about the grievance if you so desire. The

Executive Team will meet with you to discuss your

concerns, if you wish, you may have assistance from the

Client Rights Officer or outside representation during your

meeting. The Executive Team will also speak with staff

involved and may request to speak to any witnesses.

The right to receive visitors at reasonable times The right to have reasonable access to telephones to make and

receive confidential calls, including a reasonable number of free calls if unable to pay for them and assistance in calling if requested and needed. The right to have ready access to letter writing materials,

including a reasonable number of stamps without cost if unable

to pay for them, and to mail and receive unopened correspondence and assistance in writing if requested and needed subject to the facility's rules regarding contraband. Notification to family or physician The right to have a physician, family member, or representative

of the client’s choice notified promptly upon admission to a facility.

IN THE EVENT OF AN EMERGENY OR CRISIS

SITUATION; CLIENTS WILL BE VERBALLY ADVISED

OF THEIR RIGHTS ESPICALLY AS IS RELTATES TO

THE RIGHT TO REFUSE CARE, SERVICE AND

TREATMENT AND THE CONSEQUENCES OF THE

REFUSAL OF CARE, SERVICES AND TREATMENT.

Client Expectations

The following are responsibilities of

Individuals receiving services at TMSF Inc. It is your responsibility to provide accurate and complete information about all matters pertaining to your behavioral health, including medications and past or present medical problems. You are responsible for following the instructions and advice of your behavioral health care team. If you refuse treatment or do not follow the instructions or advice, you must accept the consequences of your actions. It is your responsibility to notify a member of the behavioral health care team if you do not understand information about your care treatment. You are responsible for reporting changes in your condition or symptoms, to a member of the behavioral health care team. It is your responsibility to act in a considerate and cooperative manner and to respect the rights and property of others. You are responsible for following TMSF Inc. rules and regulations. You are expected to keep your scheduled appointments or to cancel them in advance if at all possible. t is your responsibility to treat agency staff and other clients with courtesy and respect without regard to race, color, age, sex, disability, religion or national origin. It is your responsibility to cooperate with agency directions, rules and regulations to the best of one’s ability. It is your responsibility to keep the agency informed of changes in status affecting your Medicaid status. It is your responsibility to refrain from swearing or using abusive language. It is your responsibility to avoid initiation of, or participation in, any situations involving violent, harmful, threatening or abusive behaviors. It is your responsibility to respect and safeguard agency property, equipment and supplies.. It is your responsibility not to offer gifts, tips or bribes to program staff. It is your responsibility to communicate problems or concerns.

Client Rights

Each person who accesses mental health services is informed of these rights: The right to be informed within twenty-four hours of admission of the rights described in this rule, and to request a written copy

of these rights.

The right to receive information in language and terms appropriate for the person's understanding. The right to be fully informed of the cost of services.

Services are appropriate and respectful of personal liberty The right to be treated with consideration, respect for personal dignity, autonomy, and privacy, and within the parameters of relevant sections of the Ohio Revised Code and the Ohio Administrative Code.

The right to receive humane services The right to participate in any appropriate and available service that is consistent with an individual service plan, regardless of the refusal of any other service, unless that service is a necessity for clear treatment reasons and requires the person's participation.

The right to reasonable assistance in the least restrictive setting. The right to reasonable protection from physical, sexual and emotional abuse, or harassment.

Development of service plans The right to a current individualized service plan (ISP) that addresses the needs and responsibilities of an individual that specifies the provision of appropriate and adequate services, as available, either directly or by referral.

Privacy, dignity, free exercise of worship and social interaction. The right to enjoy freedom of thought, conscience, and religion; including religious worship within the facility, and services or

sacred texts that are within the reasonable capacity of the facility to supply, provided that no resident shall be coerced into engaging in any religious activities. The right to meet with surveyors, and the right to communicate

with family, guardian, custodian, friends and significant others outside the facility in accordance with the minor's individualized service plan.

Private conversation, and access to phone, mail and visitors. The right to reasonable privacy and the freedom to meet with visitors, guests, or inspectors, and make and/or receive phone calls.

The right to write or receive uncensored, unopened

correspondence subject to the facility's rules regarding

contraband. The right to send or receive mail subject to the facility's rules regarding contraband and directives from the parent or legal custodian, when such rules and directives do not conflict with federal postal regulation.

The right to communicate freely with and be visited at reasonable times by private counsel or personnel of the legal rights service and, unless prior court restriction has been obtained, to communicate freely with and be visited at reasonable times by a personal physician or psychologist

The right to communicate freely with others, unless specifically restricted in the resident of a type 1 facility's treatment plan for reasons that advance the person's goals, including, without l imitation, the following:

The right to vacate the facility at any time, except that the responsibility to pay for incurred costs of room and board shall continue unless appropriate notification has been provided to the facility concerning the termination of the residential agreement.

Development of service plans

The right to formulate advance directives, submit them to

residential staff, and rely on practitioners to follow them when

within the parameters of the law. Labor of patients The right to not be compelled to perform labor which involves the operation, support, or maintenance of the facility or for which the facility is under contract with an outside organization.

Privileges or release from the facility shall not be conditional upon the performance of such labor. Declining or consenting to services:

The right to consent to or refuse the provision of any Individual personal care activity and/or mental health services.

The right to refuse consent for major aversive interventions. The right to decline medication, except in a type 1 facility which employs staff authorized by the Ohio Revised Code to administer medication and when there is imminent risk of physical harm to self or others.

The right to actively participate in annual and periodic ISP reviews with the staff including services necessary upon dis-charge.

Declining or consenting to services The right to give full informed consent to services prior to

commencement and the right to decline services absent an emergency.

The right to be advised of and refuse observation by techniques such as one-way vision mirrors, tape recorders, televisions, such as one-way vision mirrors, tape recorders, televisions, This right does not prohibit a facility from using closed-circuit monitoring to observe seclusion rooms or common areas, which does not include resident bedrooms and bathrooms.

The right to decline any hazardous procedures The right to be free from restraint or seclusion unless there is

imminent risk of physical harm to self or others. Privacy

The right to reasonable privacy and freedom from excessive intrusion by visitors, guests, and non-facility surveyors, contractors, construction crews or others. Confidentiality

The right to confidentiality unless a release or exchange of infor-mation is authorized and the right to request to restrict

treatment information being shared.

The right to be informed of the circumstances under which the facility is authorized or intends to release, or has released, confidential information without written consent for the purposes of continuity of care as permitted by division (A)(7) of section 5122.31 of the Revised Code.

Grievances The right to have the grievance procedure explained orally and in writing; the right to file a grievance with assistance if requested ; and the right to have a grievance reviewed through

a grievance process, including the right to appeal a decision.

Non-discrimination

The right to receive services free of discrimination on the basis of race, ethnicity, age, color, religion, gender, national origin,

sexual orientation, physical or mental handicap, developmental disability, genetic information, human immunodeficiency virus status, or in any manner prohibited by local, state or federal laws.

No reprisal for exercising rights The right to exercise rights without reprisal in any form including the ability to continue services with uncompromised access.

No right extends so far as to supersede health and safety considerations

Outside opinions The right to have the opportunity to consult with independent specialists or legal counsel at one's own expense. Conflict of Interest

No residential facility employee may be a person's guardian or representative if the person is currently receiving services from said facility.

The right to receive an explanation of the reasons for denial of

service.

The right to have access to one's own psychiatric, medical or other treatment records, unless access to particular identified items of information is specifically restricted for that individual resident for clear treatment reasons in the resident's treatment plan. If access is restricted, the treatment plan shall also include

a goal to remove the restriction.

The right to be informed in advance of the reason(s) for discontinuance of service provision, and to be involved in planning for the consequences of that event.

In addition to the rights listed in paragraph (D) of this rule, each consumer residing in a residential facility shall have the following sixteen rights

Each consumer of mental health services are informed of

these rights.

The right to receive humane services in to a comfortable,

welcoming, stable and supportive environment. The right to retain personal property and possessions, including

a reasonable sum of money, consistent with the person's health, safety, service plan and developmental age. The right to vacate the facility at any time, except that the responsibility to pay for incurred costs of room and board shall continue unless appropriate notification has been provided to the facility concerning the termination of the residential

agreement.

The right to reside in a residential facility, as available and appropriate to the type of care or services that the facility is licensed to provide, regardless of previous residency, unless there is a valid and specific necessity which precludes such

residency. This necessity shall be documented and explained to the prospective resident.
