As a contracted Civic Smiles Senior provider and provider of health care services to Medicare members, you and each of your employees must complete compliance training by December 31, 2017, to meet this requirement. You may download the course entitled: Combatting Medicare Parts C and D Fraud, Waste and Abuse Training Web Based Training Course by going to: https://www.cms.gov/Outreach-and-Education/Medicare-Learning-Network- MLN/MLNProducts/Downloads/CombMedCandDFWAdownload.pdf. Further links can be found at www.DeltaDentalMN.org/resources/provider-resources/ . Follow the following path from the homepage for needed forms and training material: • Providers o Forms and Publications § Resources for Government Programs You are responsible for administering this training and for tracking you and your employees’ completion. Enclosed, please find an attestation for the participating doctor and a training log for all employees to sign upon completion. Please remember to keep copies of all training-related documentation for the required record retention period of 10 years. If you have your own training materials, attestation or training log, they are also acceptable. If you have any questions or concerns regarding the compliance-training requirement or disclosure form, please contact us toll-free at (800) 328-1188 ext. 4170, option 3.
