Primary Care VisitsNew Patient ProcessIf you are a new patient in need of a primary care provider, there are resources available to help you every step of the way.

Getting StartedWe’ve made updates to our new patient process to improve processing times and patient experiences! There is now one universal application for all Community Hospital-owned practices and a coordinator to help you through the entire process.

Want to become a new patient at Grand Valley Primary Care, Internal Medicine Associates of the Grand Valley, Grand Valley Pediatrics or Fruita Family Medicine? Follow the steps below.

1. Go to www.GVPrimaryCare.com and click on the New Patients tab.

2. Click the box on the left-hand side, indicating Community Health Partnership, and complete the application.• Paper applications are available upon request. Contact

your CHP Coordinator (page 9).3. You will receive a call within 2 business days confirming that

your application was received.4. Your CHP Coordinator will process your application

accordingly, depending on your selections. It will be overseen throughout the entire process to ensure you are able to get connected with a provider as quickly as possible.• Please note the new patient process may take up to 2 weeks.• If you are sick or need to see a provider while your application is processing, please go to Community Care of the Grand Valley (page 4) or utilize Virtual Visits (page 5).

4. You will receive another call once you have been assigned to a provider and given instructions on next steps and how to schedule your first appointment.

5. Schedule an appointment to establish care with your new provider.6. See your provider for your health care needs:

• Annually for preventative care• For referrals to lifestyle modification programs• If you get sick• As a first point of contact for any other specialized needs

Acute Care Visits

Community Care of the Grand Valley (CCGV) accepts walk-ins and appointments for acute medical needs for a $25 copay.* CCGV is staffed by physicians and mid-level providers. We are open late and on weekends to better serve you and your family.

Lifestyle ModificationLifestyle Medicine Program

The goal of Lifestyle Medicine is to replace unhealthy behaviors with behaviors that promote health. These include, but are not limited to, optimum nutrition,

physical activity, stress management, and improved interpersonal relationships. A team of Lifestyle Medicine trained professionals provide specific interventions designed to improve your health.

This program is available to all employees at no cost. The $300 fee is paid by Strive.*

The program includes:• Nutrition assessment and plan• Hands-on cooking classes taught by chefs• Exercise evaluation and plan• Pre/post lab draw• Support and accountability

If you would like more information on the Lifestyle Medicine program, please contact Lana Birch at (970) 263-2637. This program is offered twice a year and available to participants who see a specified CHP Medical Clinic Provider.

* Accountability Deposit: $50 deposit from employee is required at first class. If employee attends 80% or more of the classes, the deposit is returned in full.
