Dear Scholarship Applicant, Thank you for your interest in an educational award with the Martis Camp Community Foundation. The MCCF provides scholarships to the children of eligible Martis Camp Ambassadors for colleges and universities. Application Policies Award amounts, which will be smaller in this first year of the program, are determined by these criteria after all applications are received: • Financial Need – based upon income, size of household and individual circumstances • Tuition Cost – based on tuition cost for entire school year • Personal letter outlining education goals and family circumstances Applications are accepted March 1, June 1, September 1 and December 1 of the calendar year. Dear Scholarship Applicant,

The Martis Camp Community Foundation is proud to sponsor a College Scholarship program to

dependents/legal dependents of all Martis Camp Ambassadors. An award of up to $25,000 (lifetime) per

Ambassador may be granted, subject to available funds and approval by the MCCF Scholarship Committee.

Application Policies and Requirements

• Children must be enrolled or planning to enroll as a full-time student in either community college

or a four-year university/college. Preference will be given to full-time students at four-year

colleges/universities.

• Maximum award amounts are $25,000 (lifetime) per Ambassador.

• Award amounts can range from $250-$5,000 per year. Individual award amounts will vary depending

upon family size, tuition/income ratios, and total award budget for all applicants. • Awards are based on financial need. Families with annual gross income** of $156,667 or less may apply. • Awards are for licensed institutions only; evidence of enrollment is required to receive payment. • Ambassadors must have been employed by Martis Camp for at least year and have worked a minimum of 700 hours as of the application deadline. • Parents/Guardians must submit a personal letter outlining special needs and a description of financial needs to be eligible. The applicant will be responsible for any tuition expenses not covered by this award.

• The child of the Ambassador applying for this award must also write a personal statement, that includes

personal background, and educational and career aspirations.

• Aplications may be submitted in English or Spanish.

• If you receive an award, you must provide an invoice, tuition statement or receipt of proof of payment

directly to MCCF. Award payments are made once per year, prior to the start of school.

Eligibility

Children of Ambassadors are eligible to apply for a College Scholarship Award under the following conditions:

• Student must be the legal child of a Martis Camp Ambassador. Ambassadors who are not parents of

children they are applying for must provide documentation of legal guardianship. If the child does not

residewiththeambassador,financialinformationforthecustodialparentwillberequired. • The Martis Camp Ambassador* must be employed with Martis Camp at least one year as of the application deadline. • Families earning a combined annual gross income** of $156,667 or more are not eligible. • Ambassadors under the $156,667 threshold are eligible to apply; however, in addition to income qualifications, all other aspects affecting each applicant’s financial need will be considered before an award is granted.

Eligible Recipients

To be eligible for the College Scholarship Award, the applicant: • Must be enrolled full-time. Enrollment confirmation and tuition statement will be required.

Definitions

*Martis Camp Ambassador:

Any employee of Martis Camp Club or Martis Camp Community Association

who has worked at least 700 hours and for a minimum of one year

by the college scholarship application deadline

**Annual Gross Income:

The total of all forms of gross income, prior to deductions, including

pre-tax adjustments or deductions, contributions to retirement accounts

orcafeteriaplans,i.e.SocialSecuritywagesperW-2form(s),andalso includes any self-employment or rental property income

• Must meet and maintain a minimum GPA (grade point average) requirement of 3.0 (B) for each semester. Previous transcripts will be required. For students with a GPA below 3.0, special consideration will be made if extenuating circumstances are outlined in the personal statement. • Parents/Guardians must submit a personal letter outlining special needs and a description of financial needs to be eligible. The applicant will be responsible for any tuition expenses not covered by this award.

• The child of the Ambassador applying for this award must also write a personal statement, that includes

personal background, and educational and career aspirations. • Must submit the financial aid offers from each school the student is considering. • Must submit copies of both W-2 form(s) and the previous year’s (if available) 1040/1040EZ IRS Tax Return, instead of just one or the other. If either parent has additional forms of income, copies of associated schedules are required, such as: Self-Employment (Sched. C), Investment (Sched. D), Rental Income (Sched. E) and/or Farming (Sched. F).

Award PaymentsScholarship awards are paid only for tuition costs, official student fees, room and board, and textbooks that correspond with each semester’s official curriculum. This award does not cover extracurricular activities,

supplies or transportation costs.

If you receive an award, you must provide proof of enrollment and tuition directly to the foundation. The tuition

information provided in your application packet to the foundation is not acceptable for payment of your award.

Award payments are made once per academic year, prior to the start of school.
