Child Welfare Targeted Case Management (CW-TCM) activities are MA reimbursable for certified counties, certified tribes and private vendors under contract to counties or tribes if all eligibility and documentation requirements are met.

Including:– mental health services, – habilitative services,– educational services,– health services, – vocational services,
The case manager helps the family access needed services:
– recreational services, – and related services
including, but not limited to, the areas of volunteer services, advocacy, transportation, and legal services.

Case Management includes:
Assisting the child and the child’s family to access needed services
Coordinating needed services with other agencies

– MA or MinnesotaCare recipients who are under age 21 and who meet one of the following conditions:
• at risk of placement or in placement as defined in MN Statutes, section 260C.212, subd.1
• at risk of maltreatment or experiencing maltreatment as defined in MN Statutes, section 626.556, subd. 10e or
• in need of protection or services as defined in MN Statutes, section 260C.007, subd. 6

Case Plan
A case plan needs to be developed before claims may be submitted.
A separate written CW-TCM plan is not necessary as long as CW-TCM activities are identified in the case plan to be used.
The case plan needs to address the individual child’s needs.

TELEPHONE CONTACTS
Reimbursement for CW-TCM may be claimed for a telephone contact if:– the child is placed outside of the county or the
reservation in foster care, or placed through the Interstate Compact
– placement is more than 60 miles beyond the county or reservation boundaries,
– there is at least one contact per month and not more than two consecutive months without a face-to-face contact.

SSIS DOCUMNETATION
To generate a CW-TCM claim staff must document the contact accurately in SSIS.
The service and activity selected on the contact screen must have a BRASS code and related HCPC that is potentially billable for CW-TCM
The contact note must reflect case management activity.

For each child for whom CW-TCM is claimed
the case manager must:– determine the child meets the eligibility criteria– determine the child’s need for service, – document the eligibility and CW-TCM finding, – prepare a written service plan, – keep a written record of contacts with the client or
others,– review eligibility criteria and service plan annually
All of the above must be documented for each child in the case file.
