Introduction

Cheshire and Merseyside Palliative and End of Life Care Network sits within Cheshire and Merseyside Clinical Networks.

The purpose of this document is to have a central directory of Specialist Palliative Care Services within the Network and can be found on the website at www.cmscnsenate.nhs.uk

Cheshire & Merseyside Specialist Palliative Care Services Groups

Palliative and End of Life Care provision, within the Cheshire & Merseyside Network, is organised and delivered through locality based services know as Specialist Palliative Care Services Groups (SPCSG), formally known as Integrated Clinical Networks or ICNs. Each SPCSG brings together personnel from the community’s health organisations, including hospice, hospital and community teams.

The team includes a team manager, clinical nurse specialists, advanced practitioners and a dedicated Consultant in Palliative Medicine. The service operates 7/7, 9 am – 5 pm and supports the specialist palliative care advice line during these times. The service is also actively engaged with education and audit which supports the dissemination of best practice.

Lung SPC Referral Criteria

Any patient can be referred with a diagnosis of lung cancer or mesothelioma to the service at any point on their disease pathway from pre-diagnosis to end of life. Referrals accepted from any health care professional or carer with patient’s permission and General Practitioner informed. Self-referral also accepted.

Hospice Services Referral Criteria

Volunteers are a key component of the workforce supporting delivery of services. 24hr advice line for professionals: 01625 666999 People with incurable, life-limiting disease.

Referrals are received from patients (consent for access to medical information will be requested form GP), carers, family member and other professionals.

Day services and outpatient referrals can be referred throughout disease trajectory to support ‘living well’ with incurable disease. Inpatient referrals will have advanced progressive disease and the focus of care will have changed from curative to palliative. Some patients, who have complex specialist needs, may be referred at an earlier stage of their disease.

Area covered:

Patients registered with GPs in Eastern Cheshire (inpatient and day unit). Patients registered with GPs in the High Peak area (inpatient unit).

Requests for Patients from out of these areas may be considered on an individual basis.

Provision of community-based specialist palliative care. Assessment, planning and evaluation of adult patients with a diagnosis of cancer and those with other life limiting illness who may be approaching the last two or three months of life. They should have existing complex problems. Interventions where there may be complex communication difficulties. Support of people the patient considers to be their family. Indirect supportive advice to colleagues in the management of patient symptoms. Involvement in Gold Standard Framework meetings and Community Neighbourhood meetings. Involvement in MDT at St. Luke’s Hospice. Strategic and operational support in Palliative and Specialist Palliative Care service development. Working with End of Life Partnership to develop services and planning and delivery of education. Support of clinical and caring colleagues. Adult patients with a diagnosis of cancer or life limiting illness who have existing complex problems with pain or other uncontrolled symptoms that have not responded to first-line interventions and those patients with complex social, psychological or spiritual needs over and above those that the key worker can provide. The patient should be aware of their diagnosis and must consent to the referral, unless the patient has a mental health issue which affects their awareness. If the patient lacks capacity the referral can be made in the patient’s best interest. Our service accepts written referrals from primary, secondary and tertiary health and social care professionals if the patient meets the referral criteria. Liverpool Women’s Hospital Crown Street Liverpool Tel: 0151 702 4186 Fax: 0151 4146 www.liverpoolwomens.nhs.uk The service offers outpatient, inpatient and telephone specialist palliative care services for women and their families across the Merseyside and Cheshire cancer Network who have been diagnosed with a gynaecological malignancy. The team provides holistic support from diagnosis to bereavement as appropriate. This may also include patients who have complex needs and require support to ensure seamless transition between hospitals or hospital to home. For professional colleagues to act as resource to support them in their role. The team provide 7 day cover 9 to 5 within an agreed protocol. All relevant up to date information e.g. scans, letters, etc. should be included with the referral form. All referrals from the community should be discussed with GP prior to referral. The service operates 7/7 9-5pm and supports the specialist palliative care advice line during these times. The service is also actively engaged with education and audit which supports the dissemination of best practice.

Tel: 0844 225 0677 There is a full complement of multiprofessional support working closely with the specialist palliative care team including social work, family support, information advice, chaplaincy, lymphoedema, dietician, psychologist, liaison psychiatry, site specific CNS, benefits advice, physiotherapy, occupational therapy, complementary therapy, volunteer support, patient support groups. There is a Maggie’s Centre situated on the Clatterbridge site. Patients with progressive life limiting illness who require specialist support in managing complex physical, psychological, social and spiritual issues. Advice is also available for complex discharges and decision making. First contact for an urgent referral is 24 hours. Routine referral is 48 hours. Inpatient referrals can be made via the CCC referral form, clinical letter, phone or fax. For inpatients the referral should be documented in the case notes. Outpatient services – there is a weekly nurse led outpatient clinic and a weekly medical outpatient clinic. These are held on Wednesday afternoon and Friday morning. Referrals can be sent by post or faxed to the team using the above number. Any outpatient referrals made by phone must be accompanied by either a referral form or clinical letter.
