All entries are to be made in pen (black or blue ink only). The Work Placement Goals are to be completed at the start of your work placement. The Attendance Record must be completed after each shift.

This booklet must remain in your possession throughout your work placement: you must ensure all relevant sections are completed and signed by the supervisor and you.

The supervisor responsible for completing your evaluation must have witnessed each required criterion outlined before confirming and signing. On completion of your work placement, you must ensure that all relevant forms in Sections 1-4 have been completed before scanning and sending them to [email protected] for assessment.

Section 1Work placement is an integral part of the CHC43115 Certificate IV in Disability and provides a pathway to work in various settings such as community centres, non-government organisations, after care support, and mental health support services.

This booklet enables the collection of specific workplace evidence and helps document your development of work skills during placement. It also assists the workplace supervisor and college assessor to evaluate your progress and develop strategies to assist you to successfully complete your placement.

Work placement requirementsTo achieve this qualification, you must undertake a minimum of 120 hours of work placement (excluding lunch breaks) in a suitable work placement venue with access to the resources, equipment and facilities specified below:

● A suitably qualified workplace supervisor

● Relevant policies, procedures, and workplace documentation to carry out tasks

● Policies and procedures related to working at the service, including WHS procedures

● Workplace information and resources for supporting persons’ needs

● Persons with whom to interact

The workplace supervisor (also referred to as a ‘third party’) is typically a person with substantial qualifications and experience in their discipline. Your supervisor has a key role in assisting you during your work placement.

Important information for learners: You should note that any offer of work placement by a host employer is made at the discretion of the employer. You are given 90 days after your date of enrolment to secure a confirmed work placement and to notify ACCSC via email at [email protected].

During work placement, you will have the opportunity to demonstrate competency by the following means:

● Direct observation and collection of evidence by a workplace supervisor

● Demonstration of skills in a simulated environment

● Completion of third-party reports by a workplace supervisor

● Maintenance of a reflective journal throughout your work placement.

The workplace supervisor must be satisfied that you have the necessary knowledge and skills to support competent industry practice. This booklet and its associated reports are vital pieces of evidence to support the assessor’s judgement of your competence.

Please note that some competencies may not have relevance at certain workplaces, while additional competencies may be required in others. Your supervisor and you are advised to reach mutual agreement about competency achievements in accordance with work placement objectives and specific workplace requirements. The competencies for this program are based on National Competency Standards (NCS) as defined in the relevant Training Package and are supported by industry.

Frequently asked questionsWhat is work placement?

Work placement is workplace education. It is an integral part of your program and must be undertaken for you to complete your qualification. The aim is for you to gain experience applying your theoretical knowledge in a practical setting. This enables the college to collect evidence that supports your ability to put theory into practice. It is also a valuable opportunity for you to gain contacts and establish networks within your preferred industry.

Work placement provides you with the opportunity to apply the skills you learn in online or distance mode in real-world environments. It is anticipated that you will develop further skills that are recognised by the industry while undertaking work placement.

When can I start work placement?

It is condition of your enrolment that you organise a suitable work placement venue with all necessary resources, equipment, and facilities in accordance with qualification requirements, within 90 days from the date you submit your enrolment application. The college recommends that you complete the theory components of a minimum of six (6) to eight (8) units of competency prior to commencing work placement. This is to ensure you have the foundation knowledge and skills to receive maximum benefit from your placement.

Will the host organisation be required to pay me for my work placement hours?

Your work placement can be paid or unpaid. If you commence paid employment, you may utilise the hours worked to gain competency if suitable.

Is the organisation expected to offer me employment once I finish work placement?

There is no obligation for the host organisation to offer you paid employment. However, your placement can be an excellent opportunity for you to build a relationship with a prospective employer. The college encourages you to look for opportunities while on placement; however, we do emphasise that the host organisation is under no obligation to offer you employment.

How many hours of work placement am I expected to complete?

To complete CHC43115 Certificate IV in Disability, you must undertake a minimum of 120 hours of work placement (not including lunch breaks) and successfully complete all tasks within this booklet.

You are responsible for reporting the hours you work in the Attendance Record in Section 2 and obtaining your supervisor’s signature for each entry. If these time records are incomplete, you may be required to complete additional hours or shifts to meet requirements, which could extend your course beyond its date for completion.

What kind of work will I be expected to do?

During your work placement, you are expected to undertake a wide range of tasks as outlined in each unit task requirements of section 3 in this document. These must be performed under the direction and supervision of your workplace supervisor. Since placement is conducted over a period of time, it is expected that you will have opportunities to practise and further develop your skills to meet the diverse needs of the organisation.

What documents do I need to complete prior to commencing work placement?

All parties (learner, host organisation and ACCSC) are required to complete and sign the Work Placement Agreement.

What if the host organisation wants to cancel the work placement before I start?

In this instance, it is your responsibility to organise an alternative host for your placement. Please contact your college trainer for assistance.

What happens if I’m sick and can’t attend work placement?

If you are unable to attend your work placement for any reason, you must:

1. Notify the host organisation before the scheduled commencement of your shift (if possible, 2-4 hours prior) to enable them to prepare for your absence

2. Obtain a medical certificate for any missed time exceeding one day, or as per organisational requirements

3. Produce a clearance from a doctor on your return (depending on the illness/ condition).

Please be advised that the host organisation is not automatically required to provide you with additional hours to make up missed time. This is sometimes beyond the host organisation’s control and you may need to complete your work placement at another host organisation or at a later date. The granting of such make up shifts is dependent on the approval of the host organisation and ACCSC has no say in this matter.

What if I have an accident or injury or am involved in an incident during placement?

If you are injured in the workplace, you MUST take the following steps:

1. Notify the supervisor on duty immediately

2. Report the incident to ACCSC at [email protected]

3. Complete and photocopy an incident report and forward the copy to the host organisation and to ACCSC at [email protected]

What is the resolution process for any conflicts or disputes during my placement?

Any conflict or dispute that may arise must be resolved in accordance with the host organisation’s policies and procedures. You must advise both your work placement supervisor and your college trainer of any dispute. The trainer is authorised to act as a mediator between the host organisation and you if the situation requires.

What happens if the host organisation wishes to cancel my placement?

We encourage you to speak to the host organisation directly for clarification and advice about the parts of your performance that need improvement. You must communicate any issues to the college to ensure these concerns are addressed and your work placement is successful.

Should a situation arise that needs immediate attention, contact ACCSC at [email protected] so we can conduct mediation with the organisation and you. In the case of serious misconduct or unsafe conduct, you may be required to leave the organisation immediately. You must notify ACCSC within 24 hours of leaving the organisation. Please note that this may impact your enrolment depending on the severity of the conduct.

Under what circumstances can the college terminate my work placement?

You are reminded that any criminal, unethical, unprofessional or unsafe conduct by you may warrant the termination of your work placement. You may also be removed from work placement if your mental or physical health may affect customers, safety or your work placement performance.

What are the rules in relation to clothing and appearance?

Please be guided by the host organisation’s individual requirements. However, as a rule, your dress standard should be smart, clean and business-like. It is important to project a positive and professional image. Closed in shoes must be worn at all times.

What are my responsibilities during work placement?

● Return your signed work placement agreement to ACCSC prior to starting placement

● Arrive at least 15 minutes prior to the agreed commencement time

● Take only the allotted time for tea and lunch breaks

● Store your mobile phone safely and in silent mode whilst attending placement

● Follow the host’s WHS guidelines to report any hazards you observe

● Always abide by the Code of Conduct of the host and ACCSC

● Seek out learning opportunities and ask for feedback from your supervisor

● Maintain host, person, staff, and peer confidentiality

● Be available to undertake shift work where required (except overnight shifts)

● Notify the appropriate persons if you are unable to attend your shift

● Complete all required tasks in this booklet

What are the responsibilities of the host organisation?

● Sign the Work Placement Agreement outlining host obligations

● Provide opportunities for you to demonstrate required competencies

● Provide you with an orientation to relevant areas of the host’s operations

● Observe your work and complete the supervisor observations in this booklet

● Ensure suitable staff are allocated to supervise your work

● Notify ACCSC of any actions by you that may jeopardise your placement

What are the responsibilities of ACCSC?

● Hold and maintain the necessary insurances to cover the learner

● Allocate a trainer responsible for overseeing the work placement who will:

Notify you of your obligations whilst attending placement

Work placement objectivesSupervised work placement focuses on further developing the knowledge and skills you need to be an effective mental health worker. You will be required to maintain a reflective journal throughout your placement and submit this to ACCSC for assessment. In addition to providing evidence of your development of competencies, your journal assists you to make connections between your theoretical learning and practical placement experiences.

Whenever it is safe and appropriate to the work area, you may be asked to demonstrate your skills with appropriate supervision. The college acknowledges that you may not be able to perform certain tasks e.g. respond to a fire emergency, so these skills may be performed under simulated conditions.

Guide to collecting evidence in the workplaceYou may undertake several tasks to demonstrate your skills, including:

● Performing workplace tasks with supervisor observing

● Participating in discussions with supervisor and others after completing tasks

● Maintaining your reflective journal after undertaking direct practice

● Researching specific topics related to your practice

● Participating in team meetings

● Undertaking problem-based learning

If you are unable to demonstrate a task at work placement or cannot perform it to industry standard, it will be your responsibility to find another opportunity to demonstrate the skill. Please consult with your trainer in these situations.

Confidentiality agreementAll people in your host organisation (persons, staff and visitors) expect and are entitled to confidential, fair, and respectful treatment of information about them. You must be careful to never use any person’s name when talking to your family and friends, both at home and in public places.

It is of the utmost importance that every work placement learner observes strict confidence regarding personal information concerning customers, other learners and staff of the host organisation. Any discussion or disclosure of information concerning persons, other learners and staff is a serious betrayal of the host organisation’s trust and could lead to disciplinary action.

Learners on work placement are not permitted to disclose information gained in the course of their work placement unless specifically authorised to do so.

Task completion and evidence collection Instructions to the learner

In Part 1: You are to answer all questions relating to the tasks you performed during your placement shifts. You must include specific information about the situations, the work you performed, and the people involved etc. Please remember that you must not breach the confidentiality of persons or staff members. You may only use a person’s first name or initials. Once you have finished, please sign and date the Task Completion Record in the appropriate space.

NOTE: At the beginning of placement, you must have read both Part 1 (Learner) and Part 2 (Supervisor) questions and checklists to ensure you are aware of the specific tasks you will be marked against, and therefore MUST perform while on placement.

IMPORTANT: If you are submitting attachments with your evidence, please ensure these are on separate A4 pieces of paper and attached to the rear of this booklet. Number each document and reference the unit and page number of the task it relates to. Please do not use staples, sticky tape, or other mediums to attach documents in the middle of this booklet.

Instructions to the supervisor

In Part 2: Review the task requirements at the beginning of the student placement and check the relevant boxes to indicate whether the learner performed all the required tasks to workplace standards and expectations. Please include as much information as possible about the specific tasks the learner performed and provide feedback about their overall performance at the end of each unit of competency. Note that you must have directly observed all tasks performed by the learner. Once finished, sign and date the document in the appropriate space.

As part of completing this checklist, we also ask that you review the written responses provided by the learner in Part 1 to verify the information provided and check this off.

In Part 3: Provide a detailed explanation of how you validated the information provided by the learner and workplace supervisor. Once finished, sign and date the document in the appropriate space.

CHCCCS015 Provide individualised support

To meet the practical requirements of this unit, the learner must show evidence that they have:

1. Participated in 3 client interactions/support plan implementation activities that will allow them to display their ability to:

● Determine support needs through consultation

● Provide confirmed support services

● Monitor the implemented support activities

● Complete all required reporting and documentation

This can be evidenced by the learner completing the provided questions based on their interactions, as well as providing copies of the client plan documentation (all names and personal details to be removed prior to submission – (Consent from organisation supervisor/manager and client must be obtained)

2. Consistently displayed the ability to meet the requirements outlined in the Supervisor Observation section of this unit

● This must be evidenced across the 3 client interactions, where the learner is to use individual plans as their basis for providing support to their clients

For each of these client interactions, the supervisor will need to complete the observation checklist, provide the date of observation and additional notes to support their indicated outcome (examples of relevant notes can be found in the ‘Instruction to assessor’ section of this document).

During their time on placement, the learner will need access to the following organisational documents and resources:
