The Columbus District Golf Association will send $5 per player from each entry fee to the Capital Campaign for the Hamilton House at The Ohio State University.

EVANS SCHOLARSHIP Evans Scholars Foundation administers the nations largest privately funded college scholarship program. Since 1930, over 8,500 deserving young caddies have earned their college degrees through Evans Scholarships. Today, more than 820 caddies attend college each year as Evans Scholars. The Evans Scholarship was founded by golf great Charles Chick Evans Jr. (1890-1979).

ELIGIBILITY Entrants must be a full member of a CDGA Affiliated Club. CDGA GHIN # must be on application. Team participants must be full, non-restrictive golfing members of the club they are representing.

RULES USGA rules will govern all play except for local rules approved by CDGA.FORMAT Field limited to 26 teams, one from each club. Any open remaining spots will be

filled by draw after the deadline. 18 Holes, each event. Points awarded for top finishers, total points after three events determines champion team. All play is scratch. York GC two 2-man scrambles, switch partners after nine; Oakhurst CC Points, All four balls count; Muirfield Village GC Two Best Balls of Four.

ENTRY Entry is subject to approval or rejection at any time by CDGA Tournament Committee. No practice round is included in entry. Entry must be approved by club staff.

ENTRY FEE $780 per team. Entry fee for teams from York GC, Oakhurst CC and Muirfield Village GC is $702 (10% discount as a host club member). Entry includes cart, range and continental breakfast at York GC and Oakhurst CC. No continental Breakfast at Muirfield Village; lunch will be provided with a 1:00 p.m. shotgun to follow. Fee will not be refunded after deadline unless team submits a written medical excuse.

DEADLINE June 2, 2015, 4 p.m. (or when field fills). No Tee Entries.CARTS Golf carts and caddies are permitted. Players may carry clubs if host club permits.CADDIES The following constitutes a condition of the competition and is authorized by

Appendix I, Part C, Section 3 of the Rules of Golf. A player is prohibited from having a parent, guardian or another player in the same competition serve as his caddie during the stipulated round.

TEE TIMES Requested tee times can not be guaranteed. Copies of starting times will be sent to CDGA Pro Shops approximately one week in advance of event. Contestants may also check OHIOGOLF.ORG for tee times of the events.
