EditorialThis editorial has been simmering in my head formany months now. However, time, procrastinationand numerous other excuses have somehow pre-vented me from crafting my thoughts. This delayor detouring the “to do list” is exactly the focus ofmy editorial. For all of us it is very easy to reroute, give excuses and waffle. We are all very busy. Each of us have moments when we hope thatsomeone else will do the extra job. Our subcon-scious defaults to passing the responsibility tosomeone else. We are physicians/professionalsand we are required to wear many hats, volunteerour time, and give of our energy to improve theprofile of dermatology! To this end I have createda Canadian dermatology “to do list”.

Attend the Annual MeetingOur annual meeting has become a high quality event with a shar-ing of dermatology education through small learning groups, ple-nary sessions and symposiums. The mix of international andCanadian specialists is outstanding. Where else can you meet andinteract with such experts at a meeting, and chat on a ‘one onone’ over cocktails. The meeting gives a great opportunity notonly for education but for networking and for making newfriends and socialize with other families. Go one step further andconsider presenting a paper, poster or abstract. Let’s makeVancouver, June 2009, a meeting with maximum attendance.

Give back with some input ofyour years of expertise to the upcoming dermatologists just asthose before us helped us to gain knowledge and confidence inour practices. To be in touch with the growing mind of the youngphysician has hidden rewards. I encourage you to reach out andget to know these young dermatologists at our CDA meeting.Demonstrate your support and interest in their presentations andefforts.

President’s ColumnThe members of the CDA wish to thank Dr BethO’Brien and her team for putting on a first classmeeting in Montreal. The academic sessions andsocial activities were superb. We aim to have anequally successful meeting next year inVancouver. Please mark your calendars now!

We are in the process of changing the annualconference format to more closely meet theneeds and ideals of our members. In 2011, wewill move one week earlier to the St. JeanBaptiste Day weekend, which will free up theJuly 1st long weekend when many of our mem-bers wish to be at their summer cottages or toattend family events. This would also be better for our mem-bership in Quebec and we hope that we can attract more atten-dance from La Belle Province. Needless to say, this change isgreatly appreciated by our friends in industry.

Our membership is keen for a slightly shorter format and thereis a wish to have a free afternoon where there is a scheduledevent or outing. This was very popular when it was done in thepast and it is one time that families can be together. We are aim-ing for a three and a half or four day family orientated confer-ence, to include half day courses on medical and basic cosmet-ic topics. That is the amount of time that most us spend at theAAD and other meetings. There are many factors to consider, such as symposia and high school exams, and we will need to“fine tune” the meeting over the next few years.

Next year we will begin offering a GP course the day before theCDA annual conference. This will be an annual event. Dr BrianKunimoto has kindly agreed to help set this up, modeling itafter his very successful UBC Spring Dermatology Update forFamily Physicians. The aim is to have attendance from the localareas of the CDA meeting. However, the UBC course attractsattendees from across the country and elsewhere and hopefullythe same will be true for the CDA course once we are up andrunning. Local residents-in-training will be invited.

We are continuing to look into the possibility of recognizingmoisturizers, modeled after our highly successful sunscreenprogram. As with the latter, the revenue received would beapplied solely to education of the public and health careproviders. This would complement our new “annual nationalskin disease awareness program”, this year’s condition which isto be Atopic Dermatitis. The National Eczema AwarenessCampaign was announced on Sept. 13th at the OttawaDermatological Society meeting in Iqaluit on Baffin Island.

The Canadian Skin Patient Alliance continues to grow and tobecome more established. We look forward to working withsome very accomplished members of the alliance in our effortsto improve skin care in our country. The activities of the CSPAhave been noticed by members of the executive of the Canadian

Medical Association and they are considering a similar medicaladvocacy group – a real tribute to the CSPA.

The CDA has endorsed the Canadian bid for the XXIII WorldCongress of Dermatology in 2015, to be held in Vancouver.Drs Jerry Shapiro and Harvey Lui are spearheading the pro-posal, which is to be presented to the members of theInternational League of Dermatologic Societies at the nextWorld Congress meeting in May, 2011 in Seoul, Korea. Wehave a very good chance of a successful bid. Jerry and Harveywould appreciate your promotion and assistance.

Remember next year’s annual conference, July 1 to 5, inVancouver.

Treasurer’s ReportAnother year has come to a close and I am defi-nitely looking forward to the next. Our recentMontreal conference was a great success provingthat CDA has what it takes to host a meeting that isfinancially sound, educational and fun. My chil-dren are already providing me with suggestions fornext year!

As I reported at the AGM in Montreal, your associ-ation’s finances are in good shape leaving somecapital that can potentially fund future initiativesthat support the mission statement.

Already, head office has received proposals forprojects. It seems the membership has no limit ofimagination and energy! Please remember that it is CDA policythat proposals must be accompanied by a detailed business plan inorder to be considered by the board. The coming year will be occu-pied launching the national awareness campaign on the treatmentand management of eczema (September-February) and definingthe moisturizer recognition program.

As mentioned at the AGM, a challenge for the future will be theeconomic impact of a shorter conference in Vancouver in 2009.The biggest reason for this change was to attract a larger audience.Of course, time will tell if this was the correct decision. Further tothis, the board is open to suggestions on how to increase member-ship in the CDA. It appears there is a large number of practicingdermatologists who have not chosen to join their professional soci-ety. The organization, in order to properly represent the interests ofdermatology in Canada, probably needs to consist of a biggermajority of our specialty. That will be a challenge for the next fewyears.

Work has already begun on next year’s conference. Attached tothis will be a one-day workshop appealing to primary-careproviders, and, in particular, family practice residents. In addition,Drs David McLean, Harvey Lui, and Jerry Shapiro are busy work-ing on the World Congress bid for Vancouver. It will be up to all ofus to support this endeavour.

Best wishes for the coming year!

Secretary’s ReportCongratulations to the Montreal organizing com-mittee for putting together a super meeting thispast summer. I want to take the opportunity tothank members of CDA board for extending awarm welcome to me. I am looking forward toworking with them for the next couple of years inmy role as Secretary of the CDA.

Although we have made great strides in increas-ing our membership over the last few years, westill need to reach out to nonmember dermatolo-gists to have a stronger representation. Thanks toDr Karen Edstrom for her unwavering dedicationand efforts for the last two years as Secretary ofour organization.

I would like to welcome Drs Nathalie Shaffer, Nathan Shafferand Amy Cheng as full members and Dr Virginia Killby as lifemember. We look forward to their involvement with the CDAin the years to come.

At the last meeting of the CDA board, it was unanimouslyagreed that certain dermatologists trained in countries otherthan USA and recognized by their respective provincial licens-ing bodies as dermatologists with restricted practice can applyfor and be admitted as associate members of CDA. Boardmembers felt that even though these individuals are not quali-fied under the criteria set by RCPSC for full membership, they had gone through a rigorous screening process by theprovincial colleges to be allowed to practice dermatology underrestricted circumstances to alleviate the shortage of dermatolo-gists working in teaching hospitals or underserviced areas. Itwas also agreed that each application to the category of associ-ate membership will be decided on its individual merits; theneed to create a new membership category or revise the currentassociate category was not necessary as long as the boardretains the authority to review and accept or reject membershipapplications. Associate membership under these circumstanceswill be reviewed every few years and the board reserves theright to renew or suspend membership depending on the cir-cumstances. Drs Stephen Glassman, Wei Jing Loo and FatemehJafarian have now been admitted as associate members.

A new “CDA Master Dermatology Award” will be created for‘lifetime achievements’ to honour member dermatologists whohave done and continue to do superlative work. Further infor-mation will follow once a set of criteria has been developed bythe Membership and Awards committee.

Finally, the Membership and Awards Committee is pleased towelcome Dr Maureen O’ Malley as a new member. We lookforward to working with her.

The number of requests to CDA and its headoffice for things that should or could be doneincreases each year. The only way to “corral” thismultiplicity of requests is to work within the twinmissions of the board-approved strategic plan –Representation and Continuing ProfessionalDevelopment.

CDA Annual ConferenceHead office has been working with the planningcommittee and the task force on the future of theannual conference to set up the new format anddates, and invitations to potential conferencechairs. So far, Drs Janice Liao (Edmonton 2011),Louis Weatherhead (Ottawa 2012), Yves Poulin(Québec City 2013), Jerry Shapiro (Vancouver2015) and Cheryl Rosen (Toronto 2016) have accepted.

RepresentationHO work is primarily focused on Representation, whichfocuses on the need to increase the visibility and credibility ofCDA as the professional society, and dermatologists as highly-qualified medical specialists. The underlying message isalways to highlight the dermatologist as the skin expert.

Government RelationsHead office contains to maintain a positive relationship withthe bureaucracy: At the federal level, we provide expert opinionon the communication of skin issues for the CompetitionBureau; research expert dermatologists for Health Canada proj-ects and communicate with the federal government on policydecisions of relevance to CDA members. I continue to sit on theadvisory board of the Institute of Musculoskeletal Heath andArthritis (IMHA – CIHR).

At the provincial level, we have increased the number of skincancers screenings in provincial legislatures. These events go along way toward demonstrating to decision-makers the essen-tial work of the dermatologist.

Member relationsCDA has increased its membership in the past few years; how-ever, there is still a way to go to invite all Canadian dermatolo-gists to join the association. Some people are just not joinerswhile, unfortunately, others do not see the benefits of member-ship (free quantities of top-notch materials for patients; mem-bers’ rate at the CDA annual conference, free subscription tothe Journal of Cutaneous Medicine and Surgery, advocacy onbehalf of the specialty, among others).

All CDA members are invited to explain the benefits of mem-bership in the association to their non-affiliated colleagues.

Sun Protection Evaluation ProgramThe increase in the number of SPEP contracts demonstrates thevalue placed on the CDA brand by both industry and the pub-lic. CDA members are enthusiastic supporters of the program,

pointing out to patients that they would be well-served bychoosing a sunscreen that carries the CDA logo.

We take every opportunity to inform those who ask (media, thepublic and the tanning industry via the media) that the fundsreceived from this program are held in a restricted account andused exclusively for sun safety education programs.

The sun protective clothing program is now with the CDAlawyer, to ensure that all ethical and legal issues are covered.

Sun Awareness ProgramThe public skin cancer screening events continue to be verypopular and a worthwhile investment in terms of money andtime. Media coverage increases yearly across most of the coun-try – the visual elements of the screenings make for good TVcoverage.

Under the direction of Dr Cheryl Rosen, this program increas-es in strength and popularity. Head office continues to designall print and web materials in-house. We have produced onemore edition of Sun Facts, giving an overview of sun safety. This edition of the 81/2” x 11” publication will probably be thelast: public sales are decreasing as people makes increasing useof issue-specific materials printed directly from the internet,Starting with the “melanoma” theme for SAW 2009, CDA willproduce a more compact format to be offered without cost toCDA members as a benefit of membership, and sold to publichealth groups upon request. These brochures are alwaysordered in large quantities by CDA members, and are also usedfor the school programs by medical students under a CDAmember’s tutelage.

A major new SAW project for 2008 was the production of theOutdoor Workers Manual and Outdoor Worker Program. The audience for this now ongoing program is two-fold: the HRand/or Safety Officer in organizations that hire outside workers, and the workers themselves. Different materials were produced for each group. The Manuals are selling steadily – wehave undertaken an advertising campaign to MunicipalGovernments this summer.

Another new sun awareness project this year has been the ten-page special report on Sun Safety, published with the May 20edition of the Globe and Mail. We continue to receive very pos-itive feedback on this venture We have negotiated with theGlobe that editorial text strictly comprises interviews conduct-ed by a Globe journalist with individual CDA members on apre-determined issue – the interviewee has the opportunity toreview his/her text before it goes to print (thus no surprises).Editorial text mentions neither brand nor company name and iscompletely free of any bias, perceived or real. Editorial com-prises at least 60% of the total page space and is kept separatefrom advertising.

On to the future – 2008-2011 Strategic PlanThe proposed 2008-2011 strategic plan was ratified at the 2008AGM. This document now forms the foundation for CDA headoffice priorities for the next three years. New priorities/projectswill be added as circumstances dictate.

Executive Director’s priorities for 2008-2009This list was submitted to, and accepted by, the CDA board atits meeting in July.

Election for 2009-2010 boardNominations are sought for the following positions:

Vice President (must reside outside of Atlantic Provinces andQuebec)TreasurerRegional Director for Quebec Regional Director for Ontario

Please consider which of your colleagues would make goodcandidates. How about you yourself?

Serving on the CDA board is an excellent means of havinginput into the future of the specialty in Canada. It is an inter-esting but not onerous task – head office deals with the man-agement and administration for the board and its individualmembers, as well as committees.

Communications/media relations/public awarenessThis new edition of the strategic plan focuses more heavily onpublic relations and communications to highlight the specialty.Although head office has worked in this area for a number ofyears now, we couldn’t do as much as we wanted because of thelack of resources. However, with the welcome addition of anew staff member with experience in communications, we willbe able to accomplish much more.

The first increased venture in media relations was the newsconference put on at the 08 Montreal conference. The twin top-ics were the shortage of dermatologists and the increase inmelanoma.

Speakers were Drs Jack Toole, CDA president, Brian Day,CMA president, and Joël Claveau, director of the CDA Québecsun awareness program. This event gave rise to a very satisfac-tory 91 pieces in the print and electronic media, which put theincreasing lack of specialist dermatological care for Canadiansdirectly in the spotlight.

Another communications initiative will be a winterskin/eczema report to appear in the Globe and Mail onDecember 1, 08. These reports are very popular with skinpatients and their families, and the public in general. Thechoice of the Globe and Mail as CDA’s media partner wasmade because of the Globe’s national reach, high circulationand the demographics of the readership.

Annual skin disease education campaignThe board approved this new head office project proposal withthe request that the first condition reviewed would be the treat-ment and management of eczema. Schering Canada andGalderma have provided unrestricted educational grants in sup-port of the campaign – such grants allow us to provide highquality and relevant public education materials.

The Ottawa Dermatological Society held an exceptional meet-ing in Iqaluit in mid-September. The ODS had agreed to letCDA launch its eczema campaign from Iqaluit. The campaignitself will be a country-wide public education campaign,

focusing on print and web-based information as well as radioPSAs. However, a launch from Nunavut, where there is a preva-lence of eczema, should attract media attention as well as pro-viding a real public service to both health professionals and thepublic who live in areas where such information is not readilyavailable.

For the first time, CDA has prepared a patient-orientedbrochure in not only English and French but Inuktitut also. Thebrochures will be made available to the twenty-six health cen-tres across Nunavut. If there is a feasible means of dissemina-tion, we will also distribute the brochures in Nunavik andLabrador. Quantities of the brochure in English and French(and Inuktitut, if you so desire) are available to CDA membersat no charge as a benefit of membership.

The board will shortly be making a decision as to the skin con-dition that will be the focus of the second edition of a skin dis-ease education campaign. The success of these campaignsdepends on the willingness of CDA members to share their spe-cialist knowledge with head office. We have the knowledge in-house to carry out all the communications/media relations/pub-lic awareness aspects of these campaigns; however, we arestymied if we do not have access to medical knowledge andinput So thank you in advance to all CDA members who cangive a little of their time to share in helping us produce highquality materials.

Sun Awareness ProgramNational Sun Awareness Week 2009 will run Monday June 8-Sunday June 14. SAW has become just the official launch weekfor the program, which now runs from about March toNovember.

The 2009 twin themes are the continuation of the outdoorworkers program, and melanoma, for which head office willbe producing new print materials available free to CDA mem-bers as well as downloadable information on the CDA websiteavailable to anyone.

Because of the increasing popularity of the SPEP program, there are sufficient funds to put on further events as volunteerand staff resources become available. Dr Cheryl Rosen has alsoproposed a shade competition and is working on the businessplan for this new project. Working with Dr Rosen, I would liketo implement some fresh ideas, possibly in conjunction withthe Canadian Ophthalmological Society or the CanadianPediatric Society. A one-off event with another professionalsociety would strengthen the messaging without compromisingSPEP revenue. We are also hoping to put on a major sun aware-ness event at the national level that will attract media, and thuspublic, attention.

Suggestions for further sun awareness campaigns that youwould like to participate in would be welcomed.

As soon as the sun protective clothing program is ready tolaunch, CDA will organize a media, public and industry aware-ness campaign.

Another of the Sun Safety reports is scheduled in partnershipwith the Globe and Mail, to be published just before Sun

Awareness Week. Please let us know if you are willing to beinterviewed for this report.

Government RelationsIn conjunction with Dr Joel DeKoven, CDA has written toHealth Canada regarding the revealing of product ingredients toprevent allergic reaction.

CDA will also follow up regarding the new Spectro 3, which isnow been approved for OTC use and which some members feelshould only be used under a physician’s supervision.

In what is a vicious circle, “skin” is disappearing from theIMHA agenda, due primarily to the fact that, there are so fewdermatologists doing research. The IMHA advisory board hasa strong “skin” component – Dr Jan Dutz, Dr Lucie Germain,Allan Stordy (CSPA) and me, so this is not the issue.Suggestions from CDA members as to what can be done toimprove the situation are most welcome.

Membership/Corporate DirectoryFeedback received demonstrates that the print version of thedirectory remains useful a useful tool for the membership. Themembership directory is also available on line (www. derma-tology.ca on the members’ only site). You will find only a fewemail addresses here as CDA members must give us direct per-mission to add this information. Please let us know if youwould like to add your email address to this online directory, available only to other CDA members. The directory is sent toall members at the beginning of the academic year.

Also, please note that the directory is confidential and restrict-ed to CDA members only. Please do share any of the content, particularly member information, with anyone else.

www.dermatology.caWe are in the process of redesigning the CDA website. Thecontent that is there is excellent quality, thanks to CDA mem-bers. However, the navigation system now leaves to be desired;the website grew so quickly, there is no longer a form or for-mat. The new design will be issue- rather than target-audienceoriented – today’s internauts do not like to be told what theymay or may not view.

Product Recognition ProgramThe product recognition committee is in the process ofresearching the possibility of setting up a recognition programfor moisturizers, based on the successful template for the SPEP(sun protection evaluation program). The committee is at thestage of defining criteria and putting together a business plan. All ethical considerations will be taken into account.

CDF Treasurer’s ReportI would like to take this opportunity to announce the officialassumption of the Secretary’s position by Dr Gilles Lauzon andwelcome him, on behalf of the CDF Board, to his new role inthe CDF. Gilles has been a Director on the CDF Board since2004. As many of you will know, Gilles is well-versed in manyaspects of Canadian Dermatology, including research, and willbring a unique perspective and experience to this vital positionwithin the CDF. I am sure Gilles will help propel the CDFonward!

The financial status of the CDF remains in a healthy state withjust over 3 million dollars in investment funds, currently beingmanaged by Phillips, Hager & North Investment ManagementLtd. The recent acquisition of PH&N by RBC does not appearto have caused any impact in the management of our funds todate.

Look for a summary of the Financial Statements in theFebruary edition of the CDA Bulletin.

In my role as the former Secretary-Treasurer and now, sinceFebruary of this year, as the Treasurer of the CDF, I have beenpleased to work closely with our accountant Mr. Lloyd Wright, partner in McMane Dunkel Kingston & Stranz, LLP (MDKS).MDKS is also the Auditor for the CDF and together over thepast two years we have tightened up the various processes ofthe CDF in a proactive fashion to remain ahead of the CRArequirements for non-profit organizations.

As Chair of the Medical Advisory Committee, Dr Harvey Luihas worked fastidiously to revise the Research GrantApplications and the reviewing processes to fall into line withthe Canadian Institutes of Health Research (CIHR). Dr JanDutz, one of our Directors, has also played a major role in pro-moting the future of Dermatology research at the level of theInstitute of Musculoskeletal Health and Arthritis (IMHA).

Consequently, the CDF is well poised to push ahead and steadily increase support for research in Canada. The CDF cancontinue to assure our membership, including our dedicatedLife and Benefactor Life Members and Benefactor LifePatrons, as well as our generous Corporate Sponsors that thefunds they contribute remain well-managed in keeping withcontemporary high standards.

Greetings from ManitobaVICTORIA TARASKA, MD, Winnipeg

Summer has seen a slow down in activities. Everyone is enjoy-ing the beautiful lakes. Congrats to Dr Jack Toole for a won-derful, busy and productive year as president of the CDA.Those of us who have sat on the board know how many hoursthe CDA can consume in a year. The president is the most indemand, with the most time sensitive decisions, the most com-mittees and almost daily correspondence with head office.Thank you Jack for all your hard work, time and effort. Youmade all the Manitoban derms proud and hopefully allCanadian derms feel the same way, too. We are lucky that youwill continue to volunteer as past president this next year. Thanks again for all you guidance and insight. Enjoy your extrafree time!!

Marcie Ulmer has started her practice at the Winnipeg Clinic. It is a delight to have her in Winnipeg and hope she remainswith us for many years. We have a new resident, Dr RobertSnead. He started his residency this July and will be practicingin Brandon, Manitoba when he is finished. Brandon isManitoba's second biggest city and has not had a dermatologistfor 6 years. It will be a most welcome edition for our rural cit-izens to have a dermatologist back in the city. We extend ourwarmest wishes to you both. Hopefully, we can attract a der-matopathologist and a dermatologist for Thompson, Manitoba.Any takers??

Quebec City NewsYVES POULIN, MD, Québec

Dermatology life has been very active in Quebec City duringthe last months. Quebec is under the stamina of its 400thanniversary celebrations and this was the theme of each activity.

The highlight of all activities was the Atlantic DermatologicalConference held April 25-27, 2008.

Three hundred dermatologists attended this prestigious event, making it the largest dermatological event ever held in Canadaup to now!

The scientific program was of very high quality, featuring 10prominent lecturers (Drs J Bolognia, C Drouin, N Shear,R Bissonnette, MF Demierre, J Claveau, C McCuaig,R Pelletier, J Powell, D Sasseville ).

Four exciting symposia were presented. The following expertsgave presentations during these symposia: Drs D Rigel, ABGottlieb, J Tan, E Stockfleth, J Claveau, Y Poulin, L Stein Goldand M Bourcier.

Most importantly the patient viewing session included 63 cases(65 patients) showing a wide variety of skin disorders. Thepatient booklet was 171 pages. This was the highlight of the

meeting. The venue was the bright new research unit buildingof the Hopital Hotel-Dieu. This patient viewing session, fol-lowed by a very well organized case discussion was trulyexceptional.

The venue was gorgeous, the Chateau Frontenac being a fan-tastic location and their services reaching the highest expecta-tions.

General comment was that no one ever attended such a greatsetting with such a high quality of a patient’s viewing session

at an Atlantic Dermatological Conference. Comments havebeen dithyrambic from both our Canadian and US colleagues.Sorry for those who missed it!

For more information please visit the meeting website. Itincludes a lot of information and photos of the event:www.sorelcomm.ca/atlanticderm2008

Almost all dermatologists from the Quebec DermatologicalSociety attend practically all the events held in Quebec. Journalclubs are held each month, general meetings of the QuebecDermatological Society are featured every 3 months and spe-cial events are occurring almost every month. The attendanceis exceptional, reflecting the high quality of presentations andthe importance to meet together.

Dr Renee Beach has won the Montgomery Resident ResearchAward for the resident with the best research project in theDept. of Dermatology, University of Ottawa, 2008.

Greetings from the Division of Dermatology and CutaneousSciences of the University of Alberta. This is an exciting timeof year for our program as we welcome new faces to our pro-gram, follow the exploits of recent graduates and prepare foranother exciting academic year.

We would like to welcome our two new first year residents –Isaiah Day and Vimal Prajapati. They represent the best of whatboth the University of Alberta and University of Calgary haveto offer. We look forward to working with the both of them inthe future and wish them luck in their first exciting year of res-idency. We would also like to welcome two new additions toour dermatology family. We would also like to congratulate our recent graduates. All ofthem definitely made their mark in our dermatology program, and those individuals that are leaving us will be sorely missedas both residents and friends. Dr Jay Coffey has decided toreturn back to Newfoundland with his family. And even thoughwe are upset to see him go, we know that Newfoundland is verylucky to have a dermatologist of his caliber. Dr John Elliott hasaccepted an academic position at the U of A. He is currentlyjuggling the day-to-day responsibilities of his immunology lab and the establishment of a dedicated Patch Testing clinic. We look forward to working in his clinic when it starts up inOctober of this year. Wewish him nothing but good fortune when he makes his finaldecision as to where he will establish his practice.

We are also gearing up for a number of events with regards toour Dermatology division. Dr Alain Brassard is currently in theprocess of organizing our Alberta Society of Dermatologymeeting. This is a wonderful biannual meeting that gives thedermatologists of Edmonton and Calgary an opportunity to gettogether. The central focus of this meeting, as it has been in thepast, is the wonderful live patient teaching rounds and the dis-cussions that follow. We know Dr Brassard will do an amazingjob organizing this important event. Also, we are currently inthe process of collaborating with the Calgary dermatopatholo-gists, Drs Martin Trotter, Andrea Bruecks, and Duane Barber,in order to set up a teleconference dermatopathology session. These sessions will run on a biweekly basis and help supple-ment our regular dermatopathology teaching with Dr EricSchloss. We are grateful for the support of our program direc-tor, Dr Gordon Searles, and our divisional director, Dr ThomasSalopek, for organizing this important initiative. We have alsobeen privileged to have the addition of a number of pediatricdermatology clinics through Dr Loretta Fiorillo. We are verypleased with this unique opportunity to increase our exposureto pediatric dermatology. I received an email from the national reporter, consumer affairs,at CanWest News Service about professional organizations’endorsement programs, including the financial aspects. Theanswer below may provide a framework if a journalist, patient, etc, asks you about CDA’s Sun Protection Evaluation Program(SPEP).

CDA has a program called the Sun Protection EvaluationProgram (SPEP) for sunscreens and other skin products thatcontain a Sun Protection Factor (SPF) of 15+. We are alsoworking toward a similar program for sun protective clothing. Please note that these are recognition and not endorsement programs. This means that following a stringent review of inde-pendent laboratory reports on the product by a volunteer dermatologist expert in this area, CDA may recognize that theproduct does what it claims it will do. This is different than anendorsement, which implies that CDA would encourage thepurchase of any specific product. Nor does it imply that CDAis in a partnership with any one or more companies.

A list of criteria for inclusion in SPEP can be found athttp://www.dermatology.ca/sap/safety_resources/sunscreen_faqs/logo_criteria.html

CDA does not charge a percentage of sales. We do charge a flatfee (the same for every company) for a contract for the use ofthe CDA logo and recognition statement. This fee covers theindependent laboratory reviews, administration of the programand, above all, the many public skin cancer education eventsorganized, and production of information materials (print, web-based, PSA’s) on skin cancer awareness for Canadians, createdwithin the mandate of CDA’s national sun awareness program

(SAP). The monies accrued from SPEP are put into a restrictedfund for SAP and are used for a number of specific purposes, including:

• free public skin cancer screening clinics across the country

• skin cancer screenings and educational events in provinciallegislatures, to make MPPs/MLAs aware of the prevalenceof skin cancer, and especially melanoma,in Canada; as wellas giving these officials a chance to have a personal screen-ing to enable them to see part of how dermatologists work

• national sun safety public awareness events

• free quantities of sun safety materials for CDA members tohand out to their patients in their practices and clinics. These materials are also available to physicians and otherhealthcare providers at cost. Other public information mate-rials are downloadable, free of charge, from the CDA web-site (www.dermatology.ca).

A recent major product for CDA under its national sunawareness program was the production of the Sun SafetyManual for Outdoor Workers, created for use by humanresources and safety officers in companies, governments, municipalities, etc. that employ outdoor workers.

Neither the association nor any of its members profit financial-ly from the Sun Protection Evaluation Program. All the derma-tologists who provide of their time and expertise to the CDA’snational sun awareness program do so on a voluntary basis. The program could not be put on without them.

Safety of prescription drug products is a major issue and isbecoming increasingly important. A recent comparative, analy-sis of international postmarketing surveillance systems by DrsMary Wiktorowicz and Joel Lexchin at York University,Toronto found that active surveillance is important to the earlyidentification of unanticipated adverse drug related eventsespecially those from new medications. Coordination ofresearch and information dissemination among regulatoryauthorities, drug benefit plans and academic research networkscan enhance regulators' capacity to investigate safety and effec-tiveness issues.

A number of national organizations have collaborated to sup-port this initiative. These include:

• Canadian Patient Safety Institute (CPSI)

• Canadian Institutes of Health Research (CIHR)

• Canadian Agency for Drugs and Technologies in Health(CADTH)

• Statistics Canada

• Canada Health Infoway (CHI)

• Canadian Institute for Health Information (CIHI)

As I have pointed out before, mostformer Canadian dermatologistswrote about down to earth day to dayproblems they met in their practice.Only a few papers created much dis-cussion and dissention. One that didwas a short note in 1977 by JohnMitchell of Vancouver entitled “Theangry back syndrome: eczema creates eczema1”. His basic thesiswas that “a single strong positive patch test reaction createsan angry back which is hyper-active to other patch test appli-cations”.

One particularly critical article admits that there is some truth toDr Mitchell’s concept,2 but they point out that similar ideas havebeen expressed in the German literature since 1921 and thatseveral names have been given to the same or similar reactions. Some of their comments sound like sour grapes, would thatthey had thought of the concept and the neat name.

The problem is not simple. There are several names given tothe phenomena of hypersensitive skin over the years whichmight have relevance to Mitchell’s observations. These includethe autosensitization of Whitfield (1921),3 status eczematicusof Bloch (1924),4 hyper-irritable skin of Roper and Jones(1982),5 the excited skin syndrome of Mitchell and Maibach(1982),6 and Evolution of the Terminology of “Crazy” inAngry Back Syndrome (1996)7 .

No matter because of John Mitchell’s article the “Angry BackSyndrome” is now well established in the world literature nomatter who first saw it or what other names it had been called.

R ROY FORSEY, MD, Montréal

Denis Howell graduated inmedicine at St. Bartholomew’sHospital, London, England, in1939. He was a distinguishedfootball player (full-back) for the English AmateurInternational Football Team.Shortly after graduation hejoined the Royal NavyVolunteer Reserves and heserved with distinction in theFar East and was awarded theD.S.C. (Distinguished ServiceCross). During the war he met

his wife Libby, from St. John, New Brunswick, while she wasserving in London. After the war he chose Halifax as a place tolive. Denis stated that they chose Halifax as he wanted to practice in a university centre. He completed his dermatologytraining at the University of Toronto under Norman Wrong andopened his practice in Halifax in 1949. At that time he was theonly dermatologist east of Quebec City and the Maritimes andNewfoundland was his beat. He covered this vast area in post-graduate training and local medical meetings.

Denis served as President of the Canadian DermatologicalAssociation in 1958.

Denis was a dedicated and jovial man. Always a pleasure tomeet. He certainly served the Maritimes well and became aninspiration for all who followed him.

Arthur Robert Burns

BOB CONKLIN, MD, Richmond, BC

I am honored that you shouldask me to add a few personalcomments re Dr Bob Burnswith whom I practiced inRichmond, BC for manyyears. I recall one experienceabout Bob Burns that I havealways remembered, veryfondly. When I qualified inDermatology, 33 years ago,(having benefited greatly myfinal year by the teaching ofall you great teachers inOttawa) I asked Bill Stewart,

the head of Dermatology UBC, how I should approach choos-ing a practice site in BC in what was then a very tight market.

(How times have changed!)

I spoke to most of the practicing dermatologists in BC and eventraveled to the Interior. I was met with a range of responses, allnegative, and I was becoming very discouraged. Finally, I vis-ited Bob Burns in Richmond. He showed me his appointmentbook. There were openings only a few days ahead. This was atime when the population of Richmond was one fifth or less ofwhat it is now. Yet, he said he would welcome me. I wasfloored!... and quite overcome by his generous attitude. I feltbad about setting up in his territory, but I had few options. Infact, he did contact me several times in the next year and askedme how I was doing! Imagine! I knew from my receptionistthat his practice had slowed even more, initially. (Don’t recep-tionists know everything?)

It was really very slow starting my practice, but I have alwayshad a very fond memory of Bob Burns because of this experi-ence. He was a very non assuming, and very compassionateperson, possessing the best traits for a doctor.

Money was not his major concern.

I came to know that his compassion to people was not just lim-ited to his practice; it was a way of life for him. His wife,Sylvia, is a patient of mine in later years, and misses him dear-ly. Bob, in retirement, used to come in with her fromTsawwassen where they lived. I could see he was sufferingfrom his disease, but he would always rise in his chair, manag-ing his friendly smile, shake my hand, and ask me how I wasdoing with no complaint about himself..

Ricky Kanee Schachter

JONATHAN SCHACTER, Toronto, Grandson

Many people in the dermatol-ogy community, both inCanada and abroad, have beensaddened by the loss of DrRicky Kanee Schachter (23December 1918 – 1 July2007). Dr Ricky, as her col-leagues and patients calledher, had an extraordinary lifeas a committed healer andeducator, who had to fightfiercely to be taken seriouslyin a profession dominated bymen, and in an era when Jews

faced anti-semitism across the globe. She was an intenselydriven person who could do whatever she wanted. She did sowith considerable grace, perspicacity, aplomb and care whenshe could, and with sheer strength, nerve and perseverancewhen necessary.

She was born in Melville, SK, in 1918, the only daughter ofRose and Sam Kanee. Her parents were Russian immigrants, who became leaders in their community. Sam worked for theCP Railway until he opened a general store in Melville, andsoon after got involved in the grain business. The Kanees werea nurturing and progressive family, who saw education as theonly means of achieving equality for Jews (and for women, andfor immigrants…) in Canada.

Her brothers were her inspiration: Abe was a trader in the graintrade and was instrumental in interfaith dialogue in Winnipeg;Ben went to Columbia University to study dermatology and hewas Ricky's medical role model; and Sol was a lawyer whojoined his father in the milling business, became the longestserving member of the board of directors of the Bank ofCanada, and held prominent positions in the Canadian JewishCongress and the World Jewish Congress. Harry suffered fromcerebral palsy, and died at a young age. Rose Kanee was aheadof her time in her care of her handicapped son, and this taughtRicky about dealing with chronic illness, caring for the sick, and becoming sensitive to the special needs of others.

Indeed, the narrative of Ricky Kanee Schachter's success isinseparable from the success of her parents and siblings. Moregenerally, the narrative of her success is part of much biggernarratives, of the integration of immigrants in Canada and ofthe growing movement for equality (for women, for Jews) inher time.

The Kanees felt that a young woman should be educated, achieve her goals and be equal to men. So, in the footsteps ofher brothers, Ricky went off to university at the age of 14, toget a B.A. Her parents' mindset was unusually forward-think-ing for that era, but when she decided that she wanted tobecome a doctor, her father admitted that even he thought shewould be taking up a place for a man. He would have preferredher to get married and start a family, but she threatened that she

would never get married, until she had a university education. With her science degree in hand, she moved to Toronto in 1939to study medicine. She would later follow in her brother's foot-steps, going to Columbia in NY to specialize in dermatology.Along the way, she was constantly reminded of what the Deanof Medicine at U of T told her: her two congenital anomalies, being a woman and being a Jew, would ensure her failure in thefield. No one can blame the Dean for underestimating herresolve, after all she was a tiny Jewish woman trying to betaken seriously in a world dominated by tall, Christian men.

In 1942, she married Dr Benjamin Schachter – perhaps thesmartest decision of her career. In Benj, she found a caringintellectual whose own research was cutting-edge, and whosebelief in the capacity of women to be equals was only surpassedby his belief in Ricky Kanee Schachter to achieve greatness.

The two of them supported each other in every venture, ensur-ing their success in their careers, and building a family thatknew the importance of education, new ideas, and occasionalchutzpah when necessary. His support and equal partneringmade it easier for Dr Ricky to achieve her goals. Through hisunremitting support of his wife, he proved to be quite progres-sive in his own right. The balance they struck between careerand family was impressive, even by today's standards.

In medicine, she challenged centuries-old traditions, and peo-ple were not entirely comfortable with the changes in medicinethat she and her colleagues represented. She always pushed theboundaries of what constituted good care. She delighted intelling people about her experience on rounds in London. Shefollowed the doctor from patient to patient. The doctor wouldask the nurse to ask the patient about his or her discomfort. Dr Ricky got sick of this method; although the doctors didn'tapprove, she preferred to ask patients directly. She empoweredboth nurses and patients, as central to the healing process. In2008, it is almost impossible to imagine medicine practiced anyother way.

She made important contributions to our understanding of pso-riasis and scleroderma, and her model of treating psoriasis onan outpatient basis has remained best practice, being copied inmany other jurisdictions and for many other ailments. This wasless about treating psoriasis than it was about treating sick peo-ple. Health, to Dr Ricky, was about more than just pills, bloodpressure and sunscreen; it was about ensuring the wellbeing ofthe whole patient, and this holistic approach was quite revolu-tionary among doctors trained in her era. She was interested inhelping her patients to help themselves, manage their illnessesand live normal lives. This is the mainstay of healing today. AsProf. Shear put it, “Ricky was not only the right person, but inthe right place to really deliver a model of care that, even 30years later, is still innovative and cutting-edge.”

It is often difficult for grandchildren to see their grandparentsas anything more than grandparents. Not so for the four of us!I remember being told in third grade that I had huge shoes to

fill. More recently, during my grandmother's recovery from herfirst stroke, and in the wake of her second stroke, I have beentold by many people of her significance to them, to her field, toour country: “She had balls!”, “She made dermatology sexy!”,“She changed the way we look at patients”, “She was the onlyseptuagenarian who could pull off wearing a mini-skirt!”

We know from the way she laid out her CV that she was a wife, mother and grandmother first. Other than that, she was ateacher, student, healer and fighter, in equal parts. To quote Dr Vera Price: “She was a great physician in the style of SirWilliam Osler. She had an amazing perception of people andtheir problems. She was a most incredible woman, with a greatimpact on everyone around her.” Many people have told me,since I delivered my eulogy, that my grandmother was theirfavourite teacher, and this may be even more important than hercontributions as a doctor, because this is how she lives on.

Fortunately, she received recognition of her contributions, throughout her career. In 1979, she served as president of theCDA, the first woman in Canada to lead specialists in her field. Also Chief of Medicine at Women's College Hospital. She washonoured by the Ezrath Nashim Herzog Hospital, which dedi-cated a unit for the enhancement of healing to her, in Jerusalemin October 1991. In 1995, the Women's Dermatologic Society,a branch of the American Academy of Dermatology, gave herthe Rose Hirschler Award for Excellence, named after the first known female dermatologist in the US. In 1997 she was selected to receive one of the “Women Who Make a differenceAward” in Ontario. She was inducted into the Order of Canadain 1999. In bestowing this honour, the Governor General statedthat she was “a researcher, teacher, administrator and healer, whose work in the areas of skin cancer and psoriasis hasimproved the lives of her patients, and inspired fellowresearchers across Canada.” The Governor General’s wifecalled her "an inspiration and role model for all women." She

was awarded a Queen's Jubilee medal in 2002. And these arejust a few of the awards she received after her first retirement. Most recently she received an honorary doctor of laws fromQueen's, and she was named 2005's CDA Practitioner of theYear. Still, I can't help but think that if one of the four grand-children gave her a “World's Greatest Grandmother” mug, shewould have thrown that on her CV, and told everyone about itbefore she mentioned the Order of Canada.

Was Dr Ricky Kanee Schachter the most important doctor, dermatologist or grandmother of her time? Certainly not. Butshe was part of something bigger: she and her colleaguesbrought about important changes in the way patients areregarded, and in the way women are seen in professional occu-pations. As a student of history and political science, I knowthat the twentieth century brought many important changes inmedicine and in gender equality. I can say, without a doubt, thatmy grandmother was an important part of this. Throughout herlife she was able to tackle adversaries head-on and solve mostproblems. We will remember her as a valiant, learned, aggres-sive fighter for many causes, especially the role of women. Shewas a great woman, a great person.

Ricky Kanee Schachter had a long and fulfilled career and personal life. She was undeniably lucky to have had the driveto do what she wanted, the intellect to be taken seriously, theaudacity to overcome prejudice, the longevity to be recognizedfor it, and the dignity to know that it was far more than enoughfor one person to do.

Publisher’s Note:Ricky’s brother, Dr Ben Kanee practiced dermatology formany years in Vancouver..

Wine & Food GemsLOUIS VOGEL, Publisher, Montréal

Stratford & Niagara-on-the-Lake

For many years, Sylvia & I have enjoyed our Ontario theatreweek, usually seeing two plays each day, with dinner and winein between.

There are differences: Niagara-on-the-Lake is the prettier townand more fun to explore. The main street is alive with tourists, children and beautifully lined with flowers and plants. Also theFalls are not too far away.

Stratford has continued to provide excellent theatre produc-tions, including the musicals. Shakespeare is featured, but otherauthors are also produced. Stratford can be compared to thebest productions of Broadway, London and Stratford-on-Avon.

This season’s production at Niagara-on-the-Lake has comeback from the previous two seasons, which seemed to haveslipped in quality. Shaw here is the feature along with contem-poraries and musicals.

A trip here has the advantage of enjoying a winery dinner. Ihighly recommend Peller Estates; and also Strewn Winery.

Another highlight here are winery tastings. This season, we visited Stratus. Their Stratus Red and White are both assem-blage wines, i.e. using a variety of grapes in each. The winesare excellent; both complex and flavorful; and will age well.

We have had excellent dinners at Peller, Cave Spring, andVineland. Our favorite restaurant here is Tony de Luca; and thelast few years we have stayed at Shaw Club Hotel, next to theFestival Theatre. In Stratford, I highly recommend the ArdenPark Hotel. The rooms are spacious, inexpensive and a 10minute walk to the main Theatre; as well as an inexpensive wellrun dining room.

Our favorite restaurant in Stratford is Keystone Alley Café. Wealso recommend Down the Street; a bistro type. We have givenup on the 3 heavy hitters as we found they do not respect yourtheatre reservation, or too pretentious.
