PROCEDURE/ REQUIREMENTS

IN APPLYING FOR COLLEGE ENTRANCE EXAMINATION

A – For those who are undecided to take the CSC College Entrance Examination, please seek advice from the CSC Guidance,

Counseling and Testing Officer.

B – For those who have already decided to take the examination in a particular course, be guided by the following

procedure/requirements:

1. Secure testing fee issuance slip from the CSC Guidance, Counseling and

Testing Office.

2. Fill-up the testing fee issuance slip then submit it to the Cashier’s Office and

pay the testing fee of one hundred pesos (P 100.00).

3. Present testing fee official receipt to the Testing Officer to be able to secure

the College Entrance Examination Forms.

4. Accomplish completely the College Entrance Examination Forms

5. Submit to the Testing Officer the requirements listed below within the filing

period of application to be able to secure test permit.

6. Report to the testing room at least 30 minutes before the scheduled time and

date of examination.

7. Bring your TEST PERMIT, BALLPEN and PENCIL (Mongol # 2)

A – GRADUATING FOURTH YEAR HIGH SCHOOL STUDENTS

Duly accomplished College Entrance Examination form

One (1) brown envelope (long)

Two (2) copies 2” X 2” recent identical I.D. photo

B – HIGH SCHOOL GRADUATES

Duly accomplished College Entrance Examination application form

One (1) photocopy of /Student Report Card (Form 138) original copy to be shown

One (1) photocopy of Certificate of Good Moral Character

One (1) brown envelope (long)

Two (2) copies 2” X 2” recent identical I.D. photo

C – STUDENT SHIFTING COURSE

Grades/evaluation of the subjects from the previous course taken with general weighted average signed by the

College Dean/ Program adviser (original copy to be shown)

Endorsement from the Dean of College where the student was formerly enrolled.

One (1) photocopy of Certificate of Good Moral Character

One (1) brown envelope (long)

Two (2) copies 2”x 2” recent identical I. D. photo >

Note: Students can shift course on the same colleges without College Entrance Examination provided that

no specialized tests required.

D – TRANSFEREES

Duly accomplished College Entrance Examination application form

One (1) photocopy of Transcript of Records or Certification of general weighted average

in previous course taken (original copy to be shown)

One (1) photocopy of Certificate of Good Moral Character

One (1) brown envelope (long)

Two (2) copies 2” X 2” recent identical I.D. photo

NOTE:

College Entrance Examination Application Forms are issued from Mondays to Fridays only.

NO TEST PERMIT – NO TEST POLICY will strictly be followed.

TESTING FEE of P100.00 FOR THREE PRIORITY COURSES

Filing Period: One week before the scheduled date of examination.

Changing of priority courses will be entertained after the examination was taken by applicant himself/herself

only.
