The Carolinas Medical Neighborhood Forum is the first event in a series to bring together leading authorities and practitioners in the field of healthcare transformation. The first day of the two-day event will consist of experts in the area of Patient-Centered Care and the Medical Neighborhood, from organizations like the National Committee for Quality Assurance (NCQA) and the Patient Centered Primary Care Collaborative (PCPCC).TRANSCRIPT
2014 Carolinas Medical Neighborhood Forum
The Medi Awards is a program sponsored by United Charitable Programs (UCP) a 501(c)(3), 509(a)(1) public charity. (EIN 20-4286082), Charlotte AHEC and UNC-Chapel Hill Eshelman School of Pharmacy. Our mission is to recognize individuals and organizations that have made significant contributions to advance healthcare quality and access as we forge collaborations within the Patient-Centered Medical Neighborhood. Engaging local communities will strengthen Medical Neighborhoods and help deliver on the Triple Aim of the “Patient-Centered Medical Home” (PCMH) - Improve Health Outcomes, Enhance Patient Experience and Reduce Costs.
The Medi Awards will host several regional forums throughout the year to heighten awareness of changes as they occur in the healthcare ecosystem. In addition, a National Annual Awards Gala will recognize the contributions of local leaders, stakeholders, employers, public health agencies, faith-based organizations and others that support the adoption of patient-centered care.

Evaluations and Course Credit All attendees must sign-in and verify your email address is correct (Please sign in each day if it is a multi-day program). After the completion of the CME Activity, you will receive a survey link via email. Please take the time to complete the evaluation. This information is very important to our office, as well as to the course director and faculty, in helping to plan and achieve quality courses. Once you have completed the online evaluation you will be prompted to add your name and the amount of hours you attended this program to the survey and to your Certificate of Credit. Then print or save a copy of the Certificate of Credit for your records.
PLEASE NOTE: the evaluation will only be open for 2 weeks following the program, and therefore you can only download your certificate within 2 weeks following the program. For AMA PRA Category 1 Credit(s)™, physicians, PAs and NPs should only claim credit commensurate with the extent of their participation in the activity. For other non-physician credit, there is no awarding of partial credit.
