4) Minimizing effects on residential neighborhoods and business communities; and

5) Minimizing impacts on the environment.

A3.0 PROJECT LABOR AGREEMENT

A Project Labor Agreement will not be required on this project.The Department has undertakena due diligence study to determine whether there will be a public benefit to implementation of aProject Labor Agreement (PLA) for the Project. Upon review of the due diligence study, thecommissioner will determine whether a PLA will be included in the Project. Should thedetermination be reached to include a PLA, such agreement to be entered into between theDesign-Builder and appropriate trade unions will be prepared and provided to the Proposers inthe Final RFP or by Addendum. Any provision in the RFP inconsistent with the provisions of anapproved PLA shall be superseded by the PLA to the greatest extent permitted by federal orstate law.

C1.0 GENERAL INSTRUCTIONSThis ITP Appendix C provides the general instructions and establishes the content andformatting requirements for the Technical Proposal, Volume 2.

Each Proposer should submit the Technical Proposal required pursuant to this ITP Appendix C,organized, separated and labeled in accordance with the checklist in Table C.

The submittals should be limited to the page limitations (if any) specified in this ITP Appendix C.Each sheet shall be 8.5” by 11” and printed double sided, unless otherwise stated below. Textshall be in a standard font, a minimum of ten points in height, single-spaced. All designdrawings submitted with Proposals shall be printed single-sided on 11" by 17" sheets, and allas-printed text font sizes on plans shall be at least 8 points or per NYSDOT HDM Standards.C1.0 KEY PERSONNELThe Proposer should include a resume (maximum of two three 8.5" x 11" pages per person) foreach of the Key Personnel identified in the ITP Appendix A, outlining his/her experience andqualifications as described in ITP Section 7.1.2.1 and RFP Part 3, Section 2.2. In addition, theProposer shall include Form R – Summary Individual’s Experience.

The content of each resume, and the sequence of presentation, should be as listed below:

A) Proposed role on Project;

B) Relevant licenses, registrations and certifications;

C) Total years of professional experience and years of experience performing the work theindividual would perform on this Project; resume for Quality Manager should includedescription of experience in quality systems based on ISO 9001 if applicable;

D) Relevant project experience including project names, locations and total constructioncosts; the individual’s start and end dates on each project; the individual’s role on eachproject; the duties performed on each project; and the owner’s current contactinformation, including telephone numbers and e-mail addresses;

E) History of employment with participant;

F) Percent time (percentage of working time) allocated/committed to this Project for each12 month period of the Project from NTP;

G) If more than one key position is to be filled by the same individual, so indicate; and

H) Three (3) references including the name, position, company, or agency and currenttelephone number and e-mail address for each reference. References should beowners or clients for whom the individual has performed project work for in the past five(5) years and should not be current employers of the individual

The Proposer should include Form KP in Volume 1 to communicate any approved changes inthe Proposer’s proposed roster of Key Personnel, relative to the Proposer’s SOQ submission.For each change in Key Personnel since the SOQ, the Proposer should include in the Proposalwith Form KP a copy the written approval received from the Department for such change (seeITP Section 1.15), details of such Key Personnel’s role and a 2 page resume of the substitutepersonnel. If no changes in Key Personnel have been requested since the SOQ, Proposersshould use Form KP to state that there is no change relative to the SOQ.

SECTION 11 LANDSCAPE ARCHITECTURE

11.1 SCOPE

The Scope of Landscaping Work shall include localized removal of trees and vegetation alongthe bridge approach side sloped embankments as necessary to construct the wider replacementbridge and install guide railings and municipally operated and maintained road lighting along thewesterly side of the bridge approaches. Additional tree or shrub landscaping shall be providedas a visual buffer to the LIE drainage retention basin berms located south of the Bridge.

11.2 STANDARDS AND REFERENCES

The Design-Builder shall perform site work in accordance with the Contract Requirements andthe applicable Standards, Codes and Manuals cited in Section 1.6, unless otherwise stipulatedin this Project Requirement or otherwise applicable to the Project.

11.3 GENERAL LANDSCAPE DEVELOPMENT

11.3.1 Existing Vegetation

Existing vegetation removal and disturbance shall be minimized to the cut/fill limits and anyremovals beyond those areas shall be replaced in kind with native species appropriate forUSDA NY Plant Hardiness Planting Zone 7a. Replacement planting shall include Post PlantingCare with Replacement in conformance with the requirements of NYDSOT Special Specification611.190X0024.

The Design-Builder shall obtain approval from the Department’s Construction QA Engineer priorto the removal of any tree with a diameter breast height (DBH) of 4 6 inches or greater.

Vegetation outside the limits of disturbance shall be protected with temporary plastic barrierfence. The temporary plastic barrier fence shall be installed at or as close as feasible to the dripline of the trees to be protected.

Disturbed areas shall receive Topsoil and Turf Establishment. The type, either roadside orlawn, will vary based on location. The Design-Builder shall refer to guidance given in Chapter28 of the Highway Design Manual.

11.3.2 Tree Replacement Factors

A) Every live, native deciduous tree greater than six inches diameter at breast height (“DBH”)which is removed must be replaced with a total quantity of deciduous trees a minimum of 2inch caliper (size measured 6 inches above the base of the tree) equal to the total DBHsize of the tree removed (size measured at breast height ). For example, 10 inch DBHtree removed should be replaced with (5) two inch caliper trees; however the replacementquantity will go down if larger caliper trees are used.

B) Every live, native coniferous tree removed must be replaced with a total quantity ofconiferous trees equal to the height and width of the tree removed. For example, a 20 fthigh x 10 ft wide coniferous tree should be replaced by two (2) 10 ft high x 5 ft wideconiferous trees.

F) Provide temporary signs, signals and pavement markings for vehicular, bicycle andpedestrian traffic; and

G) Incorporate the use of 8’ wide staggered crosswalks for all pedestrian crossings of stateroads and all cross streets which have ladder bar crosswalks, as per the details includedin the reference documents.

The Design-Builder may present the respective signing and pavement marking elements onseparate drawings, but shall demonstrate that the proposed signs and pavement markings workin unison in the manner called for in these Project Requirements and the governing standards.

12.3.2 Construction Requirements

12.3.2.1 Signs

The Design-Builder shall be responsible for replacing all existing NYSDOT signs within theProject limits, including the design, fabrication and installation of all signs, posts, frames andother structural components required for the provision and support of all sign panels. Note thatthe Design-Builder shall not be responsible for the installation of overhead sign panels on theeasterly fascia of the Carlls Straight Path bridge over the westbound lanes of I-495. Theexisting sign bracing at this location shall be removed but not replaced.

The Design-Builder shall not reuse any existing NYSDOT sign materials as part of thepermanent signing installation and shall be responsible for the disposal of all signing materialsand structures that are removed from the Project. Standard signs owned by municipalities otherthan NYSDOT, and non-standard signs owned by private entities but placed within NYSDOTright-of-way, with the permission of the Department, shall be removed, stored and reinstalled asrequired.

NYSDOT Sign panels shall meet the requirements of the Department’s Special Specifications645.03010011, 645.03020011, 645.03030011 and 645.03040011.

12.3.2.2 BIN Plate Sign

The Design-Builder shall furnish and install a new BIN plate meeting the requirements set forthin this section.

The material requirements for the BIN plate are:

A) Panel with reflective background: The aluminum panel shall conform to the requirementsof Specification 730-01. The background material shall be green reflective sheetingconforming to the requirements of Specification 730-05 for Class A Sheeting. The size ofthe panels shall be 1/8" thick by 3" by 12". A thin rubber or plastic gasket or sheetingmatching the plate size shall be placed behind the plate prior to installation

B) Numbers: The numbers shall be reflective sheeting conforming to the requirements ofSpecification 730-05 for Class A Sheeting, except that the adhesive shall be pressure-sensitive such that the numbers can be applied to the background in the field. Thenumbers shall be 2" high and silver-white in color conforming to FHWA series Cdimensions.

Prior to placing the numbers on the panel, the reflective background shall be clean andfree of dirt and oil which may adversely affect proper adhesion. The numbers shall beplaced on the reflective background, perpendicular to the longitudinal axis of the panel and

SECTION 13 LIGHTING

13.1 SCOPEThe Design-Builder shall conduct all Work necessary to provide all required lighting and lightingcomponents required for the Project. This includes design, fabrication and construction of alltransportation related permanent and temporary roadway lighting of the bridge and roadwaywithin the Project Limits.

13.3 REQUIREMENTS13.3.1 General RequirementsThe Design-Builder shall be responsible for designing, furnishing and installing all componentsrequired for the implementation of the lighting system for the Project including new luminaires,controls, poles, mounting, wiring, conduits, and support hardware, as necessary for delivering acomplete and functional lighting system.

The Design-Builder shall be aware that there is existing roadway lighting on some utility poleswithin the project area. The utility poles will be relocated by others, as discussed in Part 4 –Utility Requirements of these Contract Documents; however, the Design-Builder shall beresponsible for removing, storing, and reinstalling this lighting together with all appurtenanceson these poles, with all activities related to the existing lighting coordinated with the Town ofHuntington. Any the existing equipment damaged by the Design-Builder during the progress ofthe Work shall be replaced by the Design-Builder at no additional cost to the Department or theTown of Huntington.The Design-Builder shall be responsible for coordination with the Town ofHuntington to determine street lighting requirements and the need for any relocation orreplacement. The Design-Builder shall be responsible for all costs associated with thereestablishment of lighting.

The Lighting System within the Project limits shall be fully maintained by the Design-Builder forthe duration of the Project.

SECTION 15 WORK ZONE TRAFFIC CONTROL AND ACCESS

15.1 SCOPE

The Design-Builder shall be responsible for the planning and provision of Work Zone TrafficControl (WZTC), required to perform the Project Work, in conformance with NYSDOT standardsand practices and in conformance with the Manual of Uniform Traffic Control Devices, includingthe New York State Supplement, until Project Completion. This Project Requirement applies toall roads, including the I-495 mainline, ramps, cross roads, local streets, maintenance roads,driveways, and active paths within and/or affected by the Project.

The Design-Builder shall provide WZTC for the safe and efficient movement of people, goods,and services through the Project areas while maintaining access and minimizing negativeimpacts to residents, commuters, businesses, and NYSDOT maintenance operations.

Note that, as used in this section, “Work Zone Traffic Control plan” or “WZTC plan” is theequivalent of “Maintenance and Protection of Traffic plan” or “MPT plan” as described inChapter 16 of the Highway Design Manual (HDM).

15.2 STANDARDS AND REFERENCES

The Design-Builder shall perform the work zone traffic control activities in accordance with theContract Requirements and the applicable Standards, Codes and Manuals listed in Section 1.6,unless otherwise stipulated in this Project Requirements, or otherwise applicable to the Project.

15.3 REQUIREMENTS

15.3.1 General Requirements

The Design-Builder shall maintain two-way traffic (one northbound lane and one southboundlane) on Carlls Straight Path at all times. No off-site detours will be permitted. Routes foremergency vehicles and movement of pedestrians shall be maintained and open duringconstruction.

15.3.2 Work Zone Traffic Control Plan

The Design-Builder shall prepare and submit a WZTC Plan for managing traffic operations andcontrolling access until Project Completion meeting the requirements of Chapter 21 of the HDMfor final WZTC Plans.

The WZTC Plan shall be submitted to the Department’s Design Quality Assurance Engineer aminimum of two weeks prior to initiation of any Work in proximity to traffic or the implementationof any change in traffic patterns.

The WZTC Plan shall indicate the location and treatment of all traffic streams (vehicles,bicycles, pedestrians) and shall include:

A) Contingency plans for weather and utility issues;

B) Duration of each WZTC stage, including duration of any lane closure(s);

APPENDIX A UTILITY REQUIREMENTS

The Department has reviewed the Project limits and has made a preliminary assessment ofwhich utility facilities located within the Project limits may be impacted by the Project

The Department has conducted advanced utility coordination with the utility companies listedbelow.

Preliminary coordination with the existing utilities directly impacted by structure removal andreconstruction was predicated on staged construction allowing for a single utility move forVerizon conduits within the structure and a single move for overhead LIPA, Verizon andCablevision facilities. Provisions for utility relocations for traffic signal replacement is included inthe event that the signal replacement design requires new utility poles and transfers orreallocation of utilities on existing poles to meet current horizontal and vertical clearancerequirements.

A-3.2 Electric

A-3.2.1 Long Island Power Authority (LIPA)

As noted in Section A-3.1.1, some or all of the utility poles along the west side of Carlls StraightPath may need to be relocated. LIPA will be responsible for all work associated with theinstallation of new poles and removal of the existing poles. After installation of new poles andtransfer of electric facilities to the new poles by LIPA, Cablevision and Verizon will transfer theirfacilities to the new poles. It is anticipated that the total time to install new poles and relocate allaerial facilities, including removal of the existing poles, will be approximately 18 weeks. Thistime includes the transfers of telecommunications facilities to the new poles discussed insections A-3.1.1 and A-3.1.2.

If utility relocations for traffic signal replacement are needed to meet current vertical orhorizontal clearance standards, the time required for adjustment of facilities will vary based onthe number of affected utilities and whether or not new poles are required. The Design-Buildershould assume that adjustment of facilities on an existing pole will take up to 3 weeks at eachsignal pole location. If new poles and transfers are required, the Design-Builder should assumethat the total time needed to install new poles, transfer all facilities and remove the old poles willbe up to 6 weeks at each signal pole location. All work required to relocate poles and/or relocatefacilities will be the responsibility of the affected utilities.

A-3.3.1 National Grid

There are no anticipated conflicts associated with the replacement of the bridge structure,reconstruction of the approaches, or signal work. Incidental paving in the intersections mayrequire gas valve adjustments. Valve adjustments, if necessary, will take approximately oneweek and will be performed by National Grid. In the event that the Design-Builder’s means andmethods require relocation of the existing gas main between the mainline and service road therelocation will require one week for each 100 feet of linear laying length required. This workwould not begin until after the Design-Builder provides notification that appropriate clearing andgrubbing and rough grading have been completed to provide access to National Grid.

The Design-Builder shall consider National Grid’s high pressure gas main to be a fixed objectwhich cannot be relocated.

A-3.4 Water

A-3.4.1 Dix Hills Water District (DHWD)

There are no anticipated water main conflicts associated with the replacement of the bridgestructure, reconstruction of the approaches, or signal work. Incidental paving in theintersections may require water valve adjustments. Any valve adjustments, replacementsand/or water main relocations necessary shall be the Design-Builder’s responsibility. Any suchwork shall be coordinated with the DHWD.
