Take Charge of Planning and Managing Your Own Health and Career Goals
October is National Disability Employment Awareness Month (NDEAM)
NDEAM is celebrating its 70th year and the theme for 2015 is “My disability is one part of who I am.”
NDEAM was created to celebrate the contributions of workers with disabilities and to encourage the employment of people with disabilities, including transition age youth, who face low employment rates.

Disability Employment Statistics
Current students with disabilities can expect to face much higher adult unemployment rates than their peers without disabilities.
Young adults with disabilities are three times likelier to live in poverty as adults than their peers without disabilities.
71.7% of Americans ages 16-64 without disabilities are employed, while only 26% of Americans ages 16-64 with disabilities are employed.
The employment rate for youth with disabilities ages 16-19 is 16.6% for youth with disabilities compared to 29.9% for youth without disabilities.
The employment rate for youth with disabilities ages 20-24 is 31.6% for youth with disabilities compared to 65.5% for youth without disabilities.

The Need for Transition ImprovementsNot just 500,000 youth (ages 18-21) with special needs
transitioning to adult health care annually … It is ALL ADOLESCENTS who must transition
Emerging young adults (ages 18-25) as a group have the highest use of ER among those younger than age 75
Most likely to report no health care visits in last 12 months even with the ACA changes in health insurance.
Majority of youth are ill-prepared for this change
To go to school or start a career, it is necessary to transfer to adult model of care, while also managing own health care and health insurance

Achieving Successful Youth Transition
Research tells us that in order for youth to transition successfully, it is critical to address the needs of youth in these five issue areas:
Going to and completing school
Preparing for and going to work
Having youth development and leadership opportunities
Connecting to activities, services, and supports to achieve goals and meet individual needs
Having family involvement and caring adults

Youth Career Development
Career development is a critical step as we think about employment for youth with disabilities, which addresses how youth prepare for going to work and having careers.
Many young people, particularly youth with disabilities and chronic health conditions, do not have the opportunity to gain work skills and practice work.
Career development is an interactive process of developing skills in three domains.

Three Skill Domains of Career Development
1. Self Exploration - Ability to identify personal interests, skills, and values to better understand oneself and inform decisions
2. Career Exploration – Ability to identify and analyze various career options in terms of what education, training, experience, and competencies are required for success
3. Career Planning & Management – Various skills needed to maintain employability and navigate career changes throughout the lifespan (Includes soft skills, career-specific skills, job search skills, financial literacy skills, youth development and leadership competencies)

Youth Transition & Health Care Youth and caring adults cannot only think about
career development without addressing connecting activities, such as housing, transportation, financial planning, and health.
Youth cannot successfully get or maintain a job if they do not manage their health care.
Today we will be focusing on issues related to health and why managing health care is so important to achieve success in work and career.

Health Care Transition = the process of changing from a pediatric to an adult model of health care.Goal of transition is to optimize health and assist youth in
reaching their full potential.
Requires an organized transition process to support youth in acquiring independent health care skills, preparing for an adult model of care, and transferring to new providers without disruption in care.
