STARTUP AND OPERATIONAL READINESS REVIEWS

L&A successfully assists organizations with preparation and planning for startup and commissioning of systems and facilities, both nuclear and non-nuclear, and mentors them through the process from planning through completion of commissioning, to full operations.

CAPABILITIES L&A has senior personnel who have extensive experience with DOE, the U.S. Navy, and the commercial nuclear power industry in developing plans for startup and commissioning, and preparing the organization for reviews, including operational readiness reviews, that are a prerequisite for startup and commissioning. Working with the client, L&A can: Begin by reviewing the requirements for startup and commissioning Develop the startup and commissioning plan and procedures Assist in executing those procedures Periodically assess the client’s progress Perform an operational readiness review

In the event that startup requires formal reviews by independent entities, L&A will prepare the client for those reviews by conducting pre-reviews and interviews with client personnel. During those reviews, L&A will be available to assist the client in correcting any deficiencies, and will provide advice and expertise as required.

PROJECT EXPERIENCE L&A associates have extensive experience with startup on conventional and nuclear facilities, both in the United States and around the world. Recently, L&A personnel have prepared a detailed startup and staffing plan for the first-of-a-kind Hanford Waste Treatment Plant for DOE. This plan lays out DOE roles and responsibilities, staffing, and training and qualifications for the federal oversight function during startup of the Low-Activity Waste Facility, Analytical Laboratory, and Balance of Facilities, as well as the overall oversight program and prerequisites for startup activities. L&A also successfully supported the Critical Decision-4 Operational Readiness Review of the LLNL National Ignition Facility. Other experience includes startup of U.S. Navy ships and their systems, as well as nuclear power plants, commercial nuclear facilities, and DOE facilities. CAPABILITIES Our highly experienced staff works with our clients to address issues that can plague the establishment and nurturing of a resilient safety culture. A robust safety culture is fundamental to the delivery of successful projects that meet DOE, other Federal agencies, or private sector objectives. It has been shown over and over again that a strong safety culture is essential for positive employee morale and enhanced productivity over the long term. Our comprehensive approach includes: Conducting assessments using established L&A methods, lines of inquiry and checklists Identifying deficiencies and opportunities for improvement to a site / project safety

conscious work environment (SCWE) that result in specific, actionable and measureable corrective action recommendations to improve the SCWE and overall safety culture

Advising and setting up and managing tools tailored to the situation to help support and reinforce the safety culture such as an ombudsman program or a Differing Professional Opinion program

Performing follow-up visits to assess corrective action effectiveness and SCWE improvement Mentoring senior managers and provide on-site assistance with continuous improvement of performance

We are experienced in assessing issues and obstacles in establishing and nurturing an enduring safety culture. We can conduct root cause analyses of the current safety program, and determine the pathways to correct any identified deficiencies. We can teach senior managers and executives proven techniques on how to nurture a strong safety culture and, equally important, how to avoid common missteps that can result in major setbacks to projects. We have performed external independent reviews under DOE Order 450.2 (Integrated Safety Management), as well as other related guidance such as DOE’s nuclear facility safety policy (DOE Order 420.1) and similar guidance and requirements by the NRC, INPO, and IAEA. We have developed and utilized project tools, systems, and procedures directed at safety culture and SCWE. Our clients rely on L&A to assist them in meeting performance expectations in these vital areas.

PROJECT EXPERIENCE L&A personnel have participated in SCWE and safety culture reviews at several sites across the DOE complex and in the commercial nuclear arena. At the Hanford Site, L&A led an external independent review for DOE of a Differing Professional Opinion regarding non-Newtonian fluid behavior in tanks and systems at the multi-billion dollar first-of- a-kind Waste Treatment and Immobilization Plant (WTP). L&A has provided a senior team to assess the safety culture at WTP, following issuance of the June 2011 Defense Nuclear

Facilities Safety Board Recommendation 2011-1, “Safety Culture at the Waste Treatment and Immobilization Plant”. L&A personnel have served on a similar team for the Depleted Uranium Hexafluoride Projects at Paducah, Kentucky and Portsmouth, Ohio, and on the safety culture task team chartered by EFCOG and the DOE-Headquarters Health, Safety, and Security Director. Several of L&A’s subject matter experts have been involved in successful safety culture assessment, recovery, and turnaround efforts at several commercial nuclear power stations over the past ten years. L&A supported Sandia National Laboratory, the lead laboratory for the post-closure science activities on the Yucca Mountain Project, with senior advisors for nuclear safety culture and quality. Based on decades of practical experience, L&A feedback and recommended improvements on specific programs or processes - such as the employee concerns programs, corrective action programs, employee communications, and worker survey results - addressed deficiencies that worked against the nurturing of a solid safety culture. Implementing the recommended improvements increased the efficacy of those types of processes and programs, which in turn improves the overall safety culture.

INTEGRATED SAFETY MANAGEMENT SYSTEMS

L&A provides highly specialized, responsive technical and management support to address client integrated safety management system (ISMS) safety and authorization basis issues, allowing projects to get back on track.

Our personnel are knowledgeable veterans who have worked at sites throughout the DOE complex as well as commercial nuclear utilities. L&A prides itself on producing safety basis documents that enable project personnel to accomplish their work in an efficient, technically sound and safe manner.

PROJECT EXPERIENCE L&A consultants were ‘difference makers’ at Hanford providing unmatched expertise on tasks including Waste Treatment Plant regulatory oversight, tank farm safety analysis final review, K Basin USQ preparation, categorical exclusion process preparation, and new USQ process development in accordance with DOE Guide 421 (DSA Development Guide). L&A also trained the new mission support contractor senior management at Hanford in preparation for its successful ISMS validation/verification review. Also, L&A provided the expertise and leadership to produce the first successful ISMS and supporting programs for the Hanford tank farms after several unsuccessful attempts by others. In addition to contributing to safety basis work products, L&A is also a proven ‘difference-maker’ through independent assessments of safety basis programs. At the LLNL National Ignition Facility, L&A Readiness Assessment team members concluded that while the facility was in a state of readiness to safely conduct Phase I operations in accordance with the safety basis, inconsistencies existed between the configured system Configuration Items lists and the Configuration Safety Basis Document. Once corrected the facility was authorized to start up.

Innovative approaches that integrate safety and health, management, Quality, and automation create cost effective solutions. Human factors emphasis is focused on the human-machine interfaces required for ensuring the elimination/ reduction of human error, safety functions, increasing personnel safety, productivity and efficiency. Human factors are considered in facility performance failures, accidents or injuries, or poor productivity or quality operations where humans are relied upon for preventive actions (e.g., system operations, surveillance testing, and maintenance activities during normal operations) and for mitigative actions during off-normal and emergency conditions. The extent of the application of human factors is based on a tailored approach commensurate with extent of the human interaction, the overall design efforts, and the risk associated with human.

As just one example of L&A’s value-added approach to improving safety through better ergonomics, at Hanford’s K-Basin Spent Nuclear Fuel Retrieval project, L&A provided support in resolving issues surrounding the design basis of the project and also conducted ergonomic reviews that dramatically improved performance (tools, work processes, deviated working postures, labeling, & lighting) resulting in a Better Workplace Award (2006) being given to Fluor Hanford.

INDEPENDENT MANAGEMENT AND TECHNICAL ASSESSMENTS

L&A is an industry leader in conducting management and technical assessment services for DOE and its contractors, leading to improved performance for each and every client.

CAPABILITIES Our staff includes highly experienced former nuclear facility executives, managers and operators with extensive assessment and mentoring skills who can provide:

Benefits of these assessments include finding and addressing issues before DOE or the regulator- avoiding fines, fee loss, adverse publicity, or costly repeat mistakes. Staff available for these assessments and related projects have managed nuclear facilities regulated by both NRC and DOE, and ranging from Class 1 reactors to processing facilities, to R&D facilities, to waste burial grounds. Staff members have also managed major projects and major elements of such projects such as nuclear reactors, enrichment plants, processing plants, and high-level waste/spent nuclear fuel and transuranic nuclear waste repositories.

PROJECT EXPERIENCE L&A provided an assessment and senior executive mentoring to assist the Hanford mission support contractor prepare for its ISMS verification. With a short intensive review to identify weaknesses and provide mentoring, the verification was successful. L&A has also conducted a review and assessment of the Idaho National Laboratory Project Management System for the DOE Office of Nuclear Energy, which has led to inherent improvements to how this DOE site has implemented the DOE Project Management Order 413.3. L&A has conducted corporate governance reviews for a number of industry giants, such as a comprehensive review of Oak Ridge’s Y-12 efforts to implement the ARRA projects and of Savannah River Site’s spent nuclear fuel and special nuclear material projects. These reviews led to changes in management approaches that have resulted in project success (scope, cost, and schedule).

L&A has conducted independent technical/management reviews at several sites across the DOE complex, including the Oak Ridge Reservation’s East Tennessee Technology Park and Y-12 National Security Complex, the National Renewable Energy Laboratory, Pantex, and Savannah River Site’s Mixed Oxide Fuel Fabrication Facility project. Ridge, L&A led an external independent review for DOE of the K-25 Gaseous Diffusion Plant decontamination & decommissioning project, focusing on the technical approach, risk management, project management and controls, EVMS, cost and schedule, and ESH&Q. This review’s findings and the project’s corrective actions helped turn this project from “red” to “green” performance. In another first-of-a-kind environmental cleanup project on this site, U-233 Disposition (to downblend hazardous U-233 into a stable waste form for long-term storage and eventual reuse or disposal), L&A provided senior advisor services that helped ensure delivery to DOE of the 90% design on schedule with a high degree of quality.

Contractor/Performance assurance is a cost effective approach to finding and fixing problems before they impact mission success.

Introduction to Contractor and Performance Assurance

Contractor/Performance Assurance is a valuable management tool that when utilized properly, can increase a contractor’s fee earning potential and reduce the life cycle costs for the client. This type of assurance involves management identifying the critical few challenges to mission success and then implementing an assessment program that is tailored to identify problems in these areas and propose solutions.

Contractor assurance systems are intended to provide a high degree of confidence that existing management systems and processes are adequate and effective for the work being performed. Adequate and effective management systems, when properly implemented, ensure that the public, workers, the environment, and national assets are adequately protected, and that business operations are performed effectively.

Contractor assurance encompasses environment, safety, and health programs, safeguards and security, cyber security, emergency management, and business operations. A comprehensive contractor assurance program is provided through integration of systems, programs, processes and procedures such as self, management, and independent assessments; incident reporting; issues management; lessons-learned; and performance metrics.

For DOE contractors, a robust contractor assurance program is required by DOE P 226.1, Department of Energy Oversight Policy. This policy and implementing order states the need to implement an assurance system that ensures compliance with applicable requirements, pursues excellence through continuous improvement, provides for timely identification and correction of deficient conditions, and verifies the effectiveness of completed corrective actions. Additionally, the system must determine whether programs, management systems, and assurance systems comply with requirements and are effectively implemented.

Experience Related to Contractor Assurance

L&A assisted the Lawrence Livermore National Laboratory National Ignition Facility (NIF) – a first of a kind complex laser facility supporting national security and practical fusion energy development – in developing and implementing contractor assurance integrated processes and tools designed to assist line managers in assuring and improving performance. L&A personnel filled key NIF leadership positions, conducted the CD-4 readiness assessment, and verified that integrated safety management programs were in place to ensure safe operations. L&A personnel revised the LLNL Supply Chain procurement system – the result: the total number of nuclear grade procurements was reduced by nearly 90%, resulting in significant savings both in Supply Chain and NIF personnel time and supplier costs. L&A is supporting the Nevada National Security Site contractor that provides high-caliber protective force, security engineering, technical, and support services to safeguard and secure special nuclear material, personnel, property, and information in support of the DOE, the NNSA, and the Nevada Site Office operations. L&A provides oversight of the contractor’s contractor assurance system function, mentors the management team, and performs a quarterly assessment to monitor progress in contractor assurance system implementation. L&A developed and implemented a pilot program for the environment, safety, and health portions of the contractor assurance system function for the DOE National Renewal Energy Laboratory. L&A has also completed performance-improving assessments for Lawrence Berkeley National Laboratory, benchmarked performance metrics for the Hanford tank farms project with recommendations for improvement, and developed/implemented corrective action systems for Ontario Hydro Nuclear and LANL. L&A has provided mentors and advisors to the United States Enrichment Corporation Gaseous Diffusion Plants, and also implemented a new issues management procedure and tracking software system at LANL to help improve their management of needed corrective actions leading to better performance as an M&O contractor. L&A has provided a complete Issues Management System for LANL, replacing a number of departmental-related 1ssues management systems. This effort included development of a new site-wide integrated process, software review and acquisition, training of implementers, creation of a senior manager review panel, development of a detailed procedure to guide the entire process, and software implementation.

NUCLEAR FUEL CYCLE SERVICES

Longenecker and Associates provides extensive expertise in the area of both commercial and federal nuclear fuel cycles, drawing on decades of experience in industry and government.

In the commercial area, L&A provides consulting support on the nuclear fuel cycle. We are an industry-recognized leading consultant on uranium procurement, uranium enrichment and spent fuel disposal activities. L&A is respected throughout the nuclear industry for its candid, in-depth, unbiased reports on uranium enrichment technologies, financial analysis including cost structures and market opportunities for each enrichment technology - gaseous diffusion, centrifuge, advanced centrifuge, as well as laser enrichment technologies including SILEX (Separation of Isotopes by Laser Excitation) and CRISLA (Condensation Repression by Isotope Selective Laser Activation).

In the federal area, L&A provides support to DOE on uranium and nuclear fuel activities, including extensive past involvement in the Yucca Mountain Project, support for the Blue Ribbon Commission on Spent Fuel and assessment of their recommendations, uranium market analyses for DOE, and support to DOE contractors involved in D&D projects at front-end fuel cycle facilities.

CAPABILITIES AND EXPERIENCE

Our consultants have decades of extensive experience in uranium procurement, enrichment technologies, front-end markets, and financial analysis, as well as spent fuel disposal. The L&A subject matter experts have provided a number of analytical reports. Examples include: 1. the United State Enrichment Company (USEC) financial situation regarding the proposed American Centrifuge Project, and the outlook for a DOE loan guarantee for that project; 2. the SILEX laser enrichment technology, its early development experience, and the future outlook for commercial development; and 3. enrichment developments and market outlook for various suppliers, including USEC, URENCO, AREVA, and TENEX.

L&A is frequently asked by utility clients for an assessment of various suppliers’ capabilities, financial strength, or the likelihood for successful development of specific projects. In addition, L&A prepares technical, market assessment, and event analysis reports for other consulting groups.

Based on a competitive bidding for consultation, last year L&A provided a major North American nuclear utility with an extensive assessment of their uranium procurement program including evaluating the terms and conditions in their individual uranium supply contracts, extent of contract coverage, spot versus term market strategies, recommended inventory levels, financial and market risk assessment, etc. An extensive report was prepared for submittal to the utility’s regulator.

L&A routinely has assisted large, international security companies in proposal and orals preparation, and independent reviews for penetrating DOE and DoD security markets.
