Atlanta, GA 30333 Dear Dr. Frieden: We are all gravely concerned about the Fiscal Year 2011 cuts to the Centers for Disease Control and Prevention budget, particularly its potential impact on programs that seek to prevent and address violence against women. On behalf of the following organizations and hundreds of thousands of adults and children who are victims of domestic violence, dating violence, and sexual assault, we urge you to protect the following programs that support research, education, prevention and intervention to stop this violence. CDC Center for Chronic Disease Prevention and Health Promotion Sexual Assault: As you know, the Preventive Health and Health Services Block Grant (PHHSBG) allow states, tribes, and territories to address their own unique public health needs and challenges with innovative and community driven methods. Out of FY10’s $102 million budget for PHHSBG, approximately $7.5 million was used to support sexual assault services and prevention efforts. This focus directly responds to the goal of the Center in reducing chronic disease and its impact. In addition to the immediate trauma caused by abuse, the violence contributes to a number of chronic health problems, including depression, chronic pain, alcohol and substance abuse, sexually transmitted diseases such as HIV/AIDS, and often limits the ability of women to manage other chronic illnesses such as diabetes and hypertension. Without addressing sexual assault, chronic health prevention efforts will be limited. The importance of this funding to local programs as they seek to achieve healthy communities by addressing sexual assault cannot be overstated. In some states, such as Louisiana, they do not have any state funding for sexual assault services, and PHHSBG funding is an important source of direct service dollars available to help victims. We have heard from other states that this funding is the only source of funding for a 24/7 hotline as well as legal and health advocacy services. advocacy services. A cut or elimination of the sexual assault funding through PHHSBG would be devastating to communities. CDC National Center for Injury Prevention and Control In addition to PHHSBG, the CDC’s National Center for Injury Prevention and Control is a critical component within HHS for preventing teen dating violence, intimate partner violence, and sexual assault. As you know, teen dating violence, intimate partner violence, and sexual assault are serious public health issues that begin with immediate and untenable trauma and injuries and persist to impact victims’ long-term health needs. Reduced funding for the CDC’s violence prevention efforts would undermine the critical advances made by CDC’s successful private-public collaborative with the Robert Wood Johnson Foundation and the 33 Statewide Domestic Violence and Sexual Assault Coalitions which have actively encouraged and supported this work. As significantly, the CDC’s failure to continue support for prevention work would tragically eliminate the country’s most critical public health leadership voice from the prevention discussion. Eradication of the CDC’s voice would also risk the complete loss of the CDC’s unique role in evaluating certain promising strategies and directing the prevention field’s evidence-based approach to prevention analysis. Lastly, where other sectors, such as justice and social services, focus on the critical need for responses to violence after it occurs, the CDC’s prevention efforts to structure the public health infrastructure to focus on violence prevention work has been a crucial lynchpin in the prevention community’s efforts to prevent even the initial occurrence of violence. Teen Dating Violence: A funding cut to this area would essentially end Dating Matters, a new teen dating violence prevention initiative. It is aimed at developing and evaluating comprehensive, community prevention programs for middle-school-aged youth in urban, risk communities to help adolescents learn healthy relationship skills that they can use to form positive relationships with others throughout their lifetimes. In addition to the burden on the populations that would have been served, an elimination of the initiative would prevent the identification of new effective strategies, particularly for underserved populations. This work focused on underserved populations is a key gap in the teen dating violence field and serves to further understand the overlap with bullying behavior.

Domestic Violence: The Domestic Violence Prevention Enhancements and Leadership through Alliances (DELTA) Program currently supports 14 states to build capacity and state leadership to prevent IPV before it happens through prevention-focused training and technical assistance. Due to CDC funding, many efforts to prevent intimate partner violence have been successful with many states implementing more than 100 organizational changes to integrate primary prevention within traditional IPV victim service work. For example, Delaware now requires IPV prevention programs for grades K-12. Schools use a model instructional unit designed by the Delaware DELTA program. DELTA PREP, funded by the Robert Wood Johnson Foundation, supports another 19 states to begin to incorporate DELTA prevention strategies into their Coalitions and to provide information about prevention to local member agencies. DELTA funding is a critical source of prevention funding to state coalitions that we ask you to protect.

Sexual Assault: The Rape Prevention Education (RPE) program is authorized and funded through the Violence Against Women Act to provide base funding to every state and territorial health department to raise awareness of the problem of sexual assault, support efforts to prevent first-time perpetration and victimization, and bring together diverse partners to develop, implement, and evaluate statewide sexual assault prevention plans. The RPE program engages boys and men as partners, supports interdisciplinary research collaborations, fosters cross-cultural approaches to prevention, promotes healthy relationships, and funds the National Sexual Violence Resource Center (NSVRC). In 2008-2009,

RPE grants enabled prevention educators and their partners to reach approximately 16 million community members. The 2010 National Prevention Needs Assessment found that RPE funds equip prevention education staff with greater capacities and skills to design, deliver, and evaluate prevention programs (NSVRC, 2010). One recent success is California implementing a statewide social marketing initiative called the MyStrength Campaign, which has improved respectful attitudes and created healthier social climates in high schools. We appreciate your hard work in this budget climate of competing priorities. The programs above are making practical and short-term gains in the fight to end violence against women.
