T h e s u r v e y w a s c o n d u c t e d i n t h e f a l l o f1 9 9 3 i n t h e M i n h a n g D i s t r i c t o f S h a n g h a i , a

l a r g e l y s u b u r b a n a r e a s o u t h w e s t o f c e n t r a lS h a n g h a i w i t h a p o p u l a t i o n o f m o r e t h a nh a l f a m i l l i o n . A 2 - s t a g e s t r a t i f i e d c l u s t e rp r o b a b i l i t y s a m p l e w a s u s e d t o r a n d o m l ys e l e c t h o u s e h o l d s f r o m t h e o f f i c i a l h o u s e -h o l d r e g i s t r y . A t o t a l o f 2 7 1 6 h o u s e h o l d sw i t h 8 8 7 6 i n d i v i d u a l s w e r e s e l e c t e d t o b ei n t e r v i e w e d b y t r a i n e d m e d i c a l s t u d e n t s a n dl o c a l h e a l t h w o r k e r s . Of t h e i n d i v i d u a l ss e l e c t e d , 8 5 5 2 ( 9 6 . 3 % ) w e r e s u c c e s s f u l l yi n t e r v i e w e d . T h e 3 . 7 % n o n r e s p o n s e r a t er e p r e s e n t e d e i t h e r r e f u s a l s o r r e s i d e n t s n o t a th o m e d u r i n g 3 a t t e m p t e d v i s i t s .

T h e s u r v e y i n s t r u m e n t i n c l u d e d q u e s -t i o n s o n d e m o g r a p h i c s , s o c i o e c o n o m i cb a c k g r o u n d , h e a l t h s t a t u s a n d h e a l t h s e r -v i c e s , b e h a v i o r a l r i s k s , m a t e m a l h e a l t h , a n de l d e r l y h e a l t h . D i s c u s s i o nT h e p o p u l a t i o n - b a s e d d a t a f r o m t h i s

survey d o c u m e n t a d r a m a t i c i n c r e a s e i ncesarean s e c t i o n r a t e s i n t h e M i n h a n g D i s -t r i c t o f S h a n g h a i . T h e s e r a t e s i n u r b a n C h i n anow a p p r o x i m a t e l e v e l s f o u n d i n t h e U n i t e dS t a t e s a n d o t h e r d e v e l o p e d c o u n t r i e s .

T h e r a t e i n S h a n g h a i i s p a r t i c u l a r l yd i s t u r b i n g g i v e n t h a t r e l a t i v e l y few

cesarean s e c t i o n s a r e p e r f o r m e d b e c a u s e o fa p r e v i o u s cesarean b i r t h s i n c e t h e l a r g e s tp r o p o r t i o n o f d e l i v e r i e s i n C h i n a a r e f i r s tb i r t h s . I n t h i s s u r v e y , over 90% o f b i r t h s t owomen younger t h a n a g e 3 5 were f i r s tb i r t h s . I n c o m p a r i s o n , more t h a n 45% o ft h e i n c r e a s e i n US cesarean s e c t i o n r a t e sd u r i n g t h e 1 9 8 0 s was d u e t o r e p e a t cesarean

6s e c t i o n s . S i m i l a r l y , r e p e a t cesarean sec-t i o n s a c c o u n t e d f o r more t h a n t w o t h i r d s o ft h e i n c r e a s e i n C a n a d i a n r a t e s . 3O t h e r f a c t o r s seen i n d e v e l o p e d coun-t r i e s s h o u l d a l s o b e l e s s i m p o r t a n t i n

T h e cesareanr a t e was t h e h i g h e s t among women w i t hg o v e r n m e n t i n s u r a n c e , f o l l o w e d b y t h o s ew i t h l a b o r i n s u r a n c e i n s u b u r b a n a r e a s .Women w i t h l a b o r i n s u r a n c e i n r u r a l a r e a s ,c o o p e r a t i v e i n s u r a n c e , a n d s e l f - p a y m e n th a d e q u a l l y l o w cesarean s e c t i o n r a t e s .
