Medical physicists, radiobiologistsand clinical oncologists from aroundthe world gathered in Wirral, UK,recently for a unique course on radio-biology and radiobiological model-ling in radiotherapy. The aim wassimple: to give students and expertsalike the chance to share ideas anddevelop their skills in a field that’sattracting growing interest just now.

Course organizer Alan Nahum,from the UK’s Clatterbridge Centre forOncology, described radiobiology as“the science behind radiotherapy”.That means studying the effects ofradiation on biological tissue andusing the radiobiological insightsgained to improve the efficacy of radi-ation treatments.

Radiobiological modelling is amainstay of this endeavour. Over thepast decade, scientists have attemptedto model functions such as thetumour control probability (TCP), thenormal-tissue complication proba-bility (NTCP) and the relative biologi-cal effectiveness (RBE) for varioustypes of radiation therapy. Thesemodels can be used to evaluate treat-ment plans, or even to generate themin the first place.

Such models have been in the liter-ature for 10–15 years, but to date theiruse in the clinic has been limited. Nowthat radiation-therapy equipment isbecoming more sophisticated, how-ever, Nahum thinks that an under-standing of radiobiology is essential:“To make use of the possibilitieswhich the new technology gives us,you have to start using models for pre-dicting treatment outcome as part ofthe treatment planning process,” hetold medicalphysicsweb’s Michelle Jean-dron during the event.

The course is a step in the rightdirection, gathering together physi-cists and clinicians, and providingthem with the background necessaryto understand the biological basis ofradiation treatment. The first twodays of the course (part one) coveredthe essentials of cellular and molecu-lar radiobiology. Lecturers includedJ Donald Chapman, former radiobiol-ogy professor at Fox Chase CancerCenter (Philadelphia, PA), as well asNASA radiation-risk expert Hoo-shang Nikjoo.

“Don Chapman was personallyresponsible for much of our presentknowledge on how irradiated cellsrespond to changes in fractionation,oxygenation, position in the cell cycle,dose rate and so on,” Nahum claimed.

That background was reinforced ondays three and four with talks devoted

to the use of radiobiological models inradiotherapy. “We wanted to have astrong modelling element to thecourse, because this is not done byESTRO (the European Society forTherapeutic Radiation Oncology) oranyone else,” explained Nahum.

Conformal thinkingHe kicked off the second part of thecourse with an introduction to radio-biological modelling. He noted thattraditionally there has been little com-munication between radiobiologyresearchers and radiation oncologists.Instead, there has been a huge empha-sis on the physical optimization ofradiotherapy. “We, the conformaltherapists, need radiobiologists morethan ever, and vice versa,” he said.

Nahum ultimately envisions theemergence of a “conformal radiobiol-ogy”. This would incorporate the lat-est developments in radiation delivery– such as intensity-modulated radia-tion therapy (IMRT) – into TCP andNTCP models capable of generatingbiologically and physically optimizedtreatment plans. “This is the route todoing better for your patients, but youhave to do it with care,” he told delegates.

The rest of the presentations went

into more detail about the ins andouts of building reliable models, aswell as their applications. Ellen Yorkefrom the Memorial Sloan-KetteringCancer Center (New York, NY) gave arun-down of the latest clinical data onnormal-tissue complication rates andexplained how that could be used tomodel NTCP, while Nahum wentthrough the modelling of TCP. Lateron, the focus shifted to how thesemodels can actually be put to use forimproving radiotherapy.

However, if radiobiological model-ling is to gain broader acceptance then

clinicians need to be able to use theavailable optimization software. Toaddress this need, a chunk of thecourse was dedicated to working withORBIT, a modelling and plan opti-mization program developed bySwedish company RaySearch Lab-oratories, and a freely available program (BIOPLAN) developed byNahum and colleagues.

This is only the second time that thecourse has been held and the responsethus far has been “very positive”,according to Nahum. As long as inter-est remains high, the plan is to make itan annual fixture. “It’s important forclinicians and medical physicists tocome and learn about this becauseradiobiology is the science behindradiotherapy,” Nahum added. “Foroptimal use of radiotherapy, you haveto know about radiobiology, other-wise you’ve got a lot of nice new kitbut you’re not going to be able to use itfor optimal patient benefit.”
