Persistent challenges

However, despite these positive trends, serious barriers remain to improving health outcomes acrosstheglobe.Persistentchallengesfallintothreemajorcategories:

1. Rising costs of care The cost of healthcare continues to skyrocket, and worse, the increased spending is not correlatingtobetterpatientorpublichealthoutcomes.IntheUnitedStates,spendingonhealthcarestandsat16.9%ofGDP,farmoreperpersonthananyotherdevelopedcountry.YethealthoutcomesintheU.S.fallshort,andaredescribedas“amongtheworst”comparedtootherdevelopedcountries.2 While EU countries spend considerably less on healthcare per capita, the cost of healthcare for six large member states (France, theNetherlands,Germany,Denmark,BelgiumandAustria)nowexceeds10%ofGDP,andthe European value of universal healthcare is increasingly being considered against a backgroundoffinancialinstability.3Anestimated80%ofEnglishhospitals,forexample,arerunningadeficit.4

Inconsistent quality of care Inconsistentandsuboptimalqualityofcareremainsadifficultchallenge,withsignificantdifferencesintimelinessofcare,diagnosis,andtreatmentexperiencedbydifferentpopulations.Again,qualityhasnotimprovedwiththelevelofinvestment.“Wecannotexplainhigherspendingintermsofhigherqualityhealthcare,”saysLucaLorenzoni,healtheconomistandheadofhealthaccountsattheOECDHealthDivision.“Thehighestexpendituresdonotnecessarilymeanresidentsarethehealthiest.”5IntheU.S.,440,000people die each year from preventable medical errors—the country’s third leading cause ofdeath.Theseerrorscostupto$1trillionannually,withmostattributabletolackofdataorinaccuratepatientinformation.6Infact,1in25hospitalpatientsintheU.S.contractatleast one hospital-acquired infection (HAI), 7lendingtothestaggering$235billionspentincatastrophic care attributable to chronic conditions2.

2. Inadequate access to care Access to care varies widely by location, income, insurance coverage, and available technology.HealthoutcomesacrosstheEU,forexample,arestrikinglydifferent,accordingtowherepeoplelive,theirethnicity,sex,andsocioeconomicstatus.8Anestimated58.9millionpatientsinEnglandwaitedaweekormoreforaconsultationin2014.9 Emergency wait times are also increasing in many countries, including developed economies with establishedsingle-payersystemssuchasCanada,wherepatientsoftenwaitseveralhoursbeforetheyareseenbyemergencyroomphysicians.IntheU.S.,Washington,DCrankstheworst,withanaveragewaittimeof46minutes.10Issuesofaccessareprojectedtogetworse.Today most countries do not have an adequate number of skilled health professionals to servetheirpopulations.Infact,83countriesdonotmeettheminimumthresholdforbasichealthcaresetbytheWorldHealthOrganization(WHO),whichstatesthatthereshouldbeatleast22.8skilledhealthprofessionalsperpopulationof10,000.11IntheU.S.,issuesofaccessarebeingexacerbatedbytheinfluxofanestimated32millionpeoplenewlyinsuredundertheAffordableCareAct(ACA)intoanalreadyoverburdenedhealthcaresystem.What’smore,thegapiswidening,withthenumberofpracticingphysiciansdroppingaspopulationsgrow.Whole nations face an access crisis unless they can make available physicians and healthcare expertsmoreefficient.12By2025,it’sestimatedthattherewillbeashortageofmorethan130,000doctorsintheUnitedStates.13
