Challenges for providers todayThere are nearly 3 million employees working
in the tens of thousands of private social
service providers in the U.S. There is almost
no digitization of their administrative and
professional work, but the practical, profes-
sional, economic and political pressure for
increased use of technological tools is intense
and soaring.
Organizing and getting access to relevant
documents poses great challenges for providers
today, as information and documentation
about the consumers and daily activities can
easily pile up in stacks of paper and binders.

possible to store relevant information and

about individual consumers.
Andrew Baberi, who is on staff at Richmond
Foundation in Malta, treats all forms of mental
health disorders. He is certain that other
Maltese service providers could benefit from
Bosted System.

A study documents savings
A study conducted by the
University of Southern
Denmark found that
care providers can save
134 hours per employee
per year by implemen-
ting Bosted System and by working digitally
with plans, journals and medicine handling.
These savings give each employee 172 minutes
per week to spend on care rather than on paper-
work. In the Danish disability sector (comparable
in size to Arizona’s disability sector), this trans-
lates into a reallocation of nearly 90M USD in

Jeanette Tynan is a consultant in the Arizona
office and reflects on her dialogue with users.

Jeanette Tynan.

administration and documentation and real-
locate time to care,” says Jeanette Tynan.
She explains that the monthly license fee
includes three annual updates of Bosted
System. This ensures that new modules and
improvements to the system are available to all
users at no extra cost. She is certain that the
dialogue with users is essential for continued
development of the system.
“Bosted System contains a lot of sensitive
information about you and each consumer's
life.
– We take data privacy very seriously, and
Bosted System prevents unauthorized persons
from accessing, viewing, or changing the information in the system,”
says Rune Andersen, Head of Development. He explains that only
the staff at the service provider and entrusted employees at Team
Online have access to the information in Bosted System, and
everyone who has access to the information is bound by professional
secrecy.
A secure system

There are no start-up costs of any kind associated with Bosted System. A low monthly license fee is charged for each user who access the system.
Hacker-proof and encrypted
Bosted System is a web-based system that is accessed via the internet.
The system employs a secure Internet connection which makes it impos-
sible to obtain data being transmitted between Bosted System and a
computer.
Accessing Bosted System requires a personal user name and a confi-
dential password. The user name and password are used to log informa-
tion about when staff members have accessed Bosted System and which
actions they have performed in the system. To protect transmissions bet-
ween the server hosting Bosted System data and the individual user, we
use a secure connection that ensures all communication is encrypted.

TRAINING
Paper is OK – but expensive
Bosted System eliminates paperwork, copying
and mailing tasks, and the need for expensive
filing and storage solutions by automating
the reporting, documentation and invoicing
procedures.
From our experience with more than 600
providers, we know that this will create substantial
cost savings, faster workflow and better working
conditions for staff.
We cannot, however, eliminate all paperwork
over night, and many state agencies still need
to receive plans, progress reports, incident
reports, seizure observations, invoices, and other
documents on paper.
Bosted System has incorporated the official
forms, documents, and schemes from all states,
so if needed, Bosted System can automatically
fill out the requested documents with the relevant
data. In Arizona, our customers have Individual
Support Plans (ISPs), incident reports, thousands
of monthly progress reports, and many other
reports automatically generated in the format
requested by the Department of Develop-
mental Disabilities (DDD).
The working procedures are
automated, so reports are made
in seconds and can be e-mailed
or printed without having to sort
through binders of information, hand-
written notes, or old filing cabinets.
Reporting and invoicing is automated, ma-
king new plans becomes much faster, and the
filing and storage is not only automatic, faster
and cheaper, but the information can be found
within seconds.
We will generate paper versions if needed,
but can also provide the support coordinators
in state departments and agencies with a user-
name and password, so they can check on the
status of their consumers in real time and from
any location with an internet connection. This
will not only save a tree, save transport time,
and provide better information faster, but will
also increase the quality of dialogue between
state authorities and providers.
Paper will be needed for some time to come,
but the future belongs to web based, electronic
solutions.
Team Online has developed courses customized to new care providers
wishing to get off to a good start with Bosted System:
Start-up course: Two days for decision-makers and key staff at the
care provider who must be able to determine the basic setup of
Bosted System during the course.
Superuser course: One day for selected staff with technical flair and/
or a desire to help and support colleagues in connection with
everyday questions about the use of Bosted System.
User course: One day for all employees who will be working with the
Bosted System.
Written course material has been prepared for all courses and modules,
and there are also illustrated user guides available for all modules
and functionalities.
Apart from the basic courses, Team Online also offers a number of
supplementary courses such as a standard IT competency identification
process and a course in basic IT skills. In addition, we can also
develop special courses in collaboration with the care provider adapted
to local wishes and needs, e.g. if the care provider wishes to introduce
special modules after Go Live.

Connecting and protecting everyone
Cletus Thiebeau comes from a paperless culture,
the super-digitized Bank of America. But in
2003 he became president and CEO of VALLEY-
LIFE, a large nonprofit provider in Phoenix that
serves people with disabilities, and discovered
that the world of social agencies was built of
paper.
Cumbersome files were stored wherever space
allowed, some of it redundant, all of it subject
to risk from fire or water damage. The prolife-
rating paperwork cut into the
time professionals had for care
and thwarted data retrieval and
privacy protection. Having seen
what it did for banking, Thie-
beau turned to technology for
solutions.
“The Bosted System was a
natural fit for us to move toward
our goal, which is to maintain
the health and safety of our
members. Since 92 percent of
the money we receive comes from taxpayers, we
have a huge responsibility to be very frugal. The
best way is through efficiency, and technology
creates efficiency,” he said.
Arizona’s system of caring for the disabled
requires a lot of reporting, which ac-
counts for most of the paper glut.
“We are not here primarily to report
incidents to the state, but to deliver
care. This software system is the best way to do
both, and is a necessity,” Thiebeau said.
The task required merging three data systems,
upgrading existing computers and adding more
workstations.
Although the social systems of Denmark and
Arizona differ, the software adapted well. “Over-
all there is not that
much difference.

Project manager
Karla Cleveland im-
plemented Bosted’s
U.S. Beta project,
which would proceed in stages through four
service areas.
“It turned out to be incredibly easy. We expected
issues about the system not meeting our needs
but were pleasantly surprised. Our accounting
software was harder to learn,” said Cleveland.
She began by giving the staff a heads up on
the system to promote positivity before training
started.
“Some were a little fearful because they
didn’t regularly use computers. They received
one-on-one training and now some have taken
to using computers at home. Others are asking:
when do I get the training?” Cleveland said.
Now all reports for residential and day services
are digitized. Existing paper files of medical
and other records have been scanned in. And
doctors have marveled at the detailed records at
members’ appointments.
“Bosted will complete the Beta project for
VALLEYLIFE with a completely customized
system. The reporting for most other Arizona
agencies is identical. So it will travel well and
will be easier to adapt for them,” said DiGiacomo.
The concept is new to the disability field in
Arizona, which uses little technology in admini-
stration, but CEO Thiebeau believes electronic
records will grow.
“The alternative is less efficiency and less
security. We owe it to our members,” he said.

Defeating the Paper TigerFor VALLEYLIFE, the Bosted System means
shifting staff from paper work to caregiving. The
system frees up 130 hours per user per year for
direct care.
“The old way involved completing a daily log
for each member, copying it, filing it in a binder
that travels with the member, filing the copies
and periodically tabulating the data,” says Cori
Dunn, Residential Program Manager, who reports
on 40 members daily.
Now she inputs new info and stores it securely
for instant retrieval by other professionals cleared
for access, hastening solutions to problems.
Skype and instant messaging features reduce
travel time and expenses.
The agency previously had sought electronic
solutions to handle documentation required
by the State of Arizona. Tools are available but
costly in time and money. “Team Online created
a pre-packaged system, so the upfront cost was
a lot less. It’s already been tested for this use,”
says Stephan DiGiacomo, IT Specialist.
The staff adapted surprisingly quickly to the
system, DiGiacomo says. “They have more pride
in what they do when they can see their record
so easily. They have a larger purpose than just
complying with the state,” he said.
Dunn confesses feeling “giddy” having all
the information in one place “at the touch of
a button.”
Administrative Assistant Dianna Rowan is very
focused on safeguarding files and ducuments.
Her greatest concerns with implementing the
new system were that no files would be lost and
they would remain secure.
“I’m used to guarding paper files, which can
get lost. I get apprehensive because there is
so much information that can be eliminated or
altered -- but not with the Bosted System. We
haven’t lost any information,” Rowan says.
“The record is there, whatever happens, even
if a member leaves. No information is ever
removed. It’s just as confidential and secure
as it used to be. That’s very important for our
members. They are vulnerable and I would not
like to see them exploited,” she says.
White-knuckle typistLynette Sisson admits she typically has been nervous about computers and used
them as little as possible. But fate intervened.
“I realized my job demanded learning the Bosted System. The project leaders
told us about its advantages, and I thought it was a good idea. I decided to have
a positive attitude about it to make it easier to learn. It was very easy to learn.
Now I’m learning all the short cuts,” says Sisson, who trained in VALLEYLIFE
Vocational Services and works as a Direct Support Professional.
Clicking adeptly through the system, she easily explores a broad range of details,
noting her group members’ recent accomplishments with pride.
Lynette confesses she has a new goal: improving her skills as a typist.

Arizona's disablity sector gears up
Americans use digital technology to pay bills,
buy music, book movie tickets and more. A broad
majority supports digitized medical records,
which are racing toward 100 percent adoption.
Arizonans have set up a non-profit, Arizona
Health-e, to help healthcare providers switch to
digital systems. While Arizona’s Developmental
Disabilities community talks about digitizing
case records, it rarely has been able to turn that
interest into action.
“The concept is new to the industry,” said
one expert source. An Arizona agency leader said
he knew of only two agencies seeking to digi-
tize client records. Developmental Disabilities
caregivers are an especially good fit for e-records
because of the range of disabilities they address,
from autism and Down syndrome to cerebral
palsy. Tracking all data and having each client’s
information in one place improves comprehensive
care. Because of the need to document a variety of
services, from residential to job training, e-records
are even more valuable.
The lifelong nature of developmental disabilities
suggests the need for a lifelong record. On paper
such a document is extremely cumbersome, but
it becomes a dynamic, accessible tool, electro-
nically. The U.S. disabled community is living
longer, and as individuals age, their issues can
be more complicated, illustrating the need for
documents that can’t be lost or altered with
time.
With Arizona’s fragile state budget, some have

working committee on technology. And why not?

ARIZONA
Mayo Clinic: early adopterWhen Mayo Clinic Arizona opened in 1998, all of its medical records were electronic, a first for
the state, although Mayo’s Minnesota campus had gone digital in 1993.
The easy access adds up to a patient care advantage, says Deborah Jaskowski, Mayo Arizona’s
Operations Administrator.
“In the old paper days, your record was that file you held in your hand in one location at a
time. Now if a patient comes in with some rare neurological condition for which we don’t have a
specialist on site, a doctor can share the data instantly with a specialist at another campus,” says
Jankowski. Today, Mayo is focusing on connecting with non-Mayo health entities.
“We need to agree on some standard nomenclature,” Jankowski says. That way different systems
can share data with each other.
Spoonful of Sugar The U.S. door-opener to electronic records is HITECH, the 2009 Health Information Technology
Economic and Clinical Health Act. It encourages “meaningful use” of EHR, electronic health
records. Congress authorized $27 billion in cash incentives for healthcare providers and $2 billion
to facilitate exchange of EHRs. Arizona providers are eligible to pick up $500 million.
Meaningful use means the technology must show a patient benefit, says David Blumenthal,
M.D., National Coordinator for Health Information Technology. Implications for the social services
are apparent.
“Standards-based electronic information can become dynamic, interacting with other information
to generate useful safety alerts, call attention to treatment alternatives and enable instantaneous
assessments,” he says.
Arizona’s Division of Developmental Disabilities
(DDD) requires detailed reports on people with
disabilities and their care. The state safeguards
these individuals because they may have cognitive
impairment or their families are not involved in
their lives. The resultant paperwork threatens
to overwhelm the system in Arizona and other
states. Nearly 75 percent of Americans favor
starting a national electronic health records
system to share patient data among health pro-
fessionals. A similar idea could help the social
services.
• Approximately 4.5 million individuals in
the United States have developmental
disabilities, according to the CDC. Among
those of working age, 30 percent are
employed.
• Arizona’s DDD serves nearly 30,000
Arizonans with developmental disabilities,
who require reporting on a broad spectrum
of care.
• In Arizona, 88 percent of those individuals
monitored by the state live in their own or
their family’s homes.
• Arizona monitors 3,000 independent
providers of home and community-based
services.
• Advocates with disabilities made Arizona
one of the first states in the nation to
discontinue use of the term “mental
retardation” by promoting legislation
that replaced the term with “cognitive
disability” in state laws related to
individuals with developmental disabilities.

Q1: Are Arizona and its developmental disabilities community ready
to go paperless?
A1: I think they are. It’s a matter of having the resources to set it up
and implement it.
Q2: Is there a need?
A2: Digital records should be possible everywhere, because the paper
and space demands now are huge resource takers. They make
data far more accessible, also.
Q3: How difficult is it to comply with Arizona’s reporting mandates?
A3: The amount of reporting is burdensome. Most clients require
monthly progress reports for each service they use. The AAPPD
questions the utility of some of the reporting, but it’s not going down.
Q4: How does the paperwork affect care givers?
A4: With staff turnover and so much paper, case managers can’t
always find reports, usually requested by the state. Digitization
would streamline the ability to access information.
Q5: What concerns do your members have?
A5: The cost. Big agencies can handle it. Smaller providers may not
have the resources. And training. We have people with very
different levels of education.
Q6: What would it take to see widespread use of e-records?
A6: Many would want the state to reimburse for the cost of converting.
Most funding comes from the federal government, but that only
covers 75 percent of actual costs. It is a touchy thing to add
anything else.
Q7: Will the Arizona disabilities community adopt e-records?
A7: It’s the wave of the future. There’s no way to avoid it.
