WORKSHOP Mucogingival esthetic surgery
The hands-on course is planned and designed to develop
periodontal surgical skills. Each procedure is trained step
by step in detail with live demonstrations on pig jaws and
each participant also performs procedures on pig jaws. The
course is instructed by Prof. Zucchelli and his experienced
periodontal assistant Dr. Martina Stefanini.
The workshop will enable participants to do:
- Flap design for the treatment of single and multiple
recession defects
- Split-full-split flap elevation
- Connective tissue graft harvesting technique
- Suturing techniques
LECTURESoft tissue defects around osteointegrated
implants
The recession of the buccal soft tissue margin is a frequent
complication of well integrated dental implants. The appearance of
metallic structure or even their transparency through the thin buccal
soft tissues are common reasons for patient aesthetic complains.
Mucogingival plastic surgery can be successfully used in combination
with a prosthetic approach to treat soft tissue dehiscence around
dental implants. This approach consists of a presurgical phase in
which the crown is removed and the abutment reduced in order to
increase the space between it and the adjacent teeth that will be
filled with soft tissues. The second phase is the mucogingival one,
consisting of a connective tissue graft positioned above the abutment
and completely covered by a coronally advanced flap. During the
postsurgical phase a temporary crown is used to condition the
augmented soft tissue. The aim of the lecture is to describe step by
step this new combined surgical and restorative procedure for the
treatment of soft tissue defects around implants.

with simultaneous implant placement
The aim of the workshop is to present the possibility of
simultaneous implantation procedure with augmentation
of the horizontal alveolar ridge using a titanium membrane
attached to the implant. You will learn the rules, tools and a
protocol in order to achieve predictable results.
LECTUREOptimized soft tissue aesthetics based on
the surgical and prosthetic solutions
The stable and aesthetic effect of implant-prosthetic treatment is the
goal of the dentist focused on the fulfillment of the promise made to the
patient before starting treatment. Many factors and limitations affect
the choice of a particular treatment method, techniques or materials.
The aim of the lecture is to present an approach to treatment using
digital protocols and new technologies.

LECTUREHard and soft tissue augmentation in alveolar
ridge deficiencies
Modern implantology presents a permanent challenge to obtain an excellent
esthetic outcome together with the restoration of the lost function and the
good prognosis of the obtained result. One of the multiple treatment steps
in obtaining this goal is to surgically restore the alveolar ridge deficiencies.
Multiple methods have been described in literature for alveolar ridge
augmentation, according to the morphology of the defect and the type of
prosthetic reconstruction. A scientific and clinical evidence based wide
range of augmentation procedures according to the type of defect, the
reconstruction material and the treatment concept will be presented.
All kind of defects will be approached, from dehiscence defects and
fenestrations, thin ridge with horizontal bone deficiency, vertical ridge
deficiencies to combined horizontal and vertical alveolar ridge defects.
The reconstruction methods vary from GBR techniques with autologous
bone and biomaterials, resorbable membranes or titanium nonresorbable
membranes to different bone block techniques – onlay, shell, horizontal or
vertical augmentation with intraoral or extraoral source of bone, combined
vertical and horizontal techniques with autologous bone blocks and GBR,
inferior alveolar nerve repositioning. Soft tissue management with FGG, CTG
and corrective techniques for implant exposure in the esthetic area will be
outlined.

WORKSHOP Horizontal bone augmentation with
simultaneous implant placement
During the workshop the first part will consist of theoretical
presentation of the procedure including etiology, clinical and
imagistic diagnosis, surgical treatment and some clinical case
discussion.
On a pig mandible the participants will perform an implant
placement and a concomitant horizontal bone reconstruction using
GBR technique with xenograft and resorbable membrane fixated
with pins or periosteal sutures, flap management for passive closure
and suturing techniques.

That hands-on will give the participants the possibility to
exercise different flap designs step by step, with special
splitters that were developed by Dr. Steigmann. The tissue
thickness dictates the way it is manipulated for high volume
augmentation. The gain of soft tissue for tension free closure
even in cases of reduced soft tissue thickness.

LECTURESoft tissue management for bone
augmentation
In time bone augmentation has moved from highly specialized clinics
into the dental office. With the help of modern grafting material, the
augmentation volume in the dental office has increased year by year.
However, the main issue remains soft tissue closure for high volume
augmentation, especially in the posterior mandible and posterior
maxilla. We developed special flap designs and suturing techniques
specific to location to solve this ongoing soft tissue management
problem. The participants will learn and practice how to solve this soft
tissue closure problem according to the location.
The tissue thickness dictates the way it is manipulated for high volume
augmentation. Thick biotype can be managed in a classical manner.
On the other side thin biotype needs new surgical approaches with
specific instruments. The course will describe step by step the gain
of soft tissue for tension free closure even in cases of reduced soft
tissue thickness.

LECTURE Vertical bone grafting in the lateral maxilla
and mandible
In many cases, due to early extraction, periodontitis, trauma
or oncological pathology, alveolar process suffers a significant
loss and bone augmentation procedures are a prerequisite for
successful dental implant treatment.
Vertical augmentation is one of the most challenging procedures
faced by the surgeon in dental implantology and a thorough
planning and a proper surgical technique makes the difference
between a favorable and unfavorable outcome. An evidence-based
approach is advisable to be used to set accurate technique for a
specific clinical scenario.
The aim of the presentation is to make a literature review related to
the suitable techniques for bone grafting of vertical defects in the
lateral area of mandible and maxilla. Secondly, a new technique
of one stage vertical grafting and implant placement, in the lateral
maxilla with less than 1 mm vertical bone, will be presented.
LECTURE The future of reconstructive surgery:
What are the key ingredients for success?
Reconstructive surgical techniques have significantly evolved
during the last 30 years, but saving the compromised teeth by
applying established principles is still the primary goal. Success
of reconstructive surgery is highly correlated with wound stability
in the early and late phases. Biomaterial exposure or healing by
secondary intention may lead to bacterial contamination in the
surgical area and impair the outcomes of periodontal regeneration
particularly in the interproximal site. To date, different surgical
procedures have been proposed to preserve the interdental papilla
during the early and late phases of wound healing to prevent
contamination of the regenerating area and eventual wound failure.
However, published findings do not report 100% primary closure
over the defect-associated papilla during the wound healing
process. An ideal flap design may overcome this particular issue
and preserve the original anatomy of the entire interdental soft
tissues. In the current presentation, critical elements and new
perspectives of the reconstructive surgery will be discussed.

In times of growing age of the population the absolute
number of patients with periodontal disease and treated
with dental implants is increasing and alongside this fact the
complications related to these conditions. The presentation
shows a modern treatment protocol for long-term bone
preservation in natural dentition and in patients with
implants. Focus is the relation between good periodontal
health and long-term implant success dedicated to better
patient health.
The lecture puts its accent on the relation between
preservation of the natural dentition and periodontal tissues
in order to provide good quality of life to patients and save
the option to treat them with implants in a later stage without
the need for complicated protocols.
Knowing and using the biological principals gives the
options to maintain teeth and implants for long periods of
time thus providing successful healthcare for the population.
In addition, innovative technology gives the opportunity
to implement the local use of antibiotics in order to prevent
progress of periodontal and peri-implant complications.
New forms of antibiotics for local use in periodontal pockets,
peri-implant tissues and during bone augmentation procedures
provide the clinicians with a very powerful new tool in saving
teeth, bone and implants for many years.
This lecture will mainly show a contemporary protocol of
treating implants with perimucositis, and periimplantitis leading
to a very high implant survival rate.
