1 PURPOSES This procedure provides a basic guideline for Installation / Erection for CHEMICAL INJECTION SKID of MAHARAJA LELA SOUTH PROJECT to ensure that the installation / Erection activities during Installation at PTSC work site is carried out safely and correctly.

2 SCOPES This procedure applies to Installation / Erection as following:

4 DEFINITION

5 PROCEDURE A. PREPARATION

For Installation / Erection, CONTRACTOR Supervisor team shall provide all information that need to be carried out such as: • Description of the Installation / Erection Operation • Structural / equipment drawings of the lifted object if necessary • Slings’ arrangement • Temporary Installation / Erection pad-eye locations if require. • Weight and C.O.G of the lifted objects. • The information of the equipment required to perform the Installation / Erection • Check suitable lifting gears (Cranes, Slings, Lifting spreader bar, shackles…) are

B. INSTALLATION / ERECTION CHECKLIST

Installation / Erection checklist shall be used to control Installation / Erection operation prior to performing the Installation / Erection. Every item shall be cleared off from the checklist prior to approval by CONTRACTOR Supervisor. The contents of Installation / Erection checklist cover the following:

NDT Clearance

This section describes the status of NDT clearance of all item belong to the lifted object.

Site Preparation

This section describes the status of site preparation for Installation / Erection.

HSE

This section describes HSE requirements throughout Installation / Erection operation. JSA & PTW shall be performed before installation / Erection work commencement.

Installation / Erection organization

This section describes the Installation / Erection organization, communication, between the personnel in the Installation / Erection team and responsibilities.

6 INSTALLATION / ERECTION ORGANIZATION Installation / Erection organization chart

Responsibility

a) Supervisor Installation / Erection supervisor shall be responsible for supervising all Installation / Erection activities, surveying actual status of Installation / Erection area and checking Installation / Erection status.

b) Signal man Signal man shall be responsible for giving instruction to rigging team, crane operator, Installation / Erection assistants to lift the object correctly.

c) Installation / Erection communication During Installation / Erection operation each person in the Installation / Erection team shall be provided with walkie-talkie for communication. The communication system shall be set up on the dedicated channel

7 SAFETY a) JSA for Installation / Erection

JHA for Installation / Erection shall be prepared and submitted to Client for approval prior to carry out the Installation / Erection if necessary.

b) Erection Toolbox Meeting

Installation / Erection Toolbox meeting shall be arranged prior to Installation / Erection to explain to all the Installation / Erection Team how to perform the Installation / Erection in a safe manner.

c) Installation / Erection Equipment Inspection

• Prior to installation all Slings and Shackles shall be inspected for condition and that they are within colour code (certification)

• A valid test certificates shall be made available and submitted to Client for review and approval.

• Other Installation / Erection attachments such as pad-eyes and weld areas of the assemblies shall be inspected by NDT and submitted to Client for review and approval.

d) List of Personnel in Installation / Erection Team

Personnel in the Installation / Erection team shall be listed out and submitted to Client for review and approval.

e) Installation / Erection Permit to Work

Installation / Erection permit to work shall be available at site prior to performing the Installation / Erection.

f) 5.6 Checklist

The Installation / Erection checklist shall be verified and signed by CONTRACTOR Supervisor and prior to Installation / Erection and submitted to Client for review prior to approval of Permit to Work for Installation / Erection if necessary.

g) Toolbox Briefing

The briefing shall be carried out at the site prior to Installation / Erection operation for all the Installation / Erection team, the contents of the briefing shall include HSE, JHA, PTW, personnel duties and responsibilities, Installation / Erection procedures and work scope.

8 INSTALLATION / ERECTION OPERATION Site Supervisor shall be at site to manage the Installation / Erection team to lift the object safely and correctly.

Direction guiding shall be used by 2 tag lines such as ropes to guide the lifted object to right position, to keep it stable during Installation / Erection time.

Load shall never travel above personnel.

All non-essential personnel shall be kept at a clear safe distance and only those directly involved in the operation and with due cause shall be allowed in close proximity to alive load.

PPE shall be worn at all times.

Prior to Installation / Erection, Contractor shall check the readiness of the structural in terms of vessel or skid seating, dimensions, location and painting to ensure all the requirements in the drawing and specification are followed.

9 SITE PREPARATION Clear all obstructions (handrail and others) around Installation / Erection areas at the installation areas before the cranes approaches to carry out the Installation / Erection activities SKID.

Provide barricade tape (Red / White stripe) and signage to barricade Installation / Erection and skidding / swinging area.

Walkie-talkie or radio shall be provided to crane operator and Installation / Erection supervisor.

Verify that the access gets through the SKID to its foundation, So SKID can be handled to the foundation, which is adequate and free from any obstruction.

10 INSTALLATION / ERECTION AND INSTALL AT FINAL POSITION Check the position Lifting Equipment near to SKID. Then Installation / Erection tackle (slings, shackles, Installation / Erection belt and guide rope) onto the equipment.

Execute initial Installation / Erection by hoisting up main block of the crane until the SKID is approximately one (1) meter above the ground. Check the condition of the equipment should be visual as well as its center of gravity and the orientation.

Continue hoisting up the equipment until the require height is reached at one (1) meter

Swing the crane booms towards the foundation. Once the equipment is 100% onto the foundation, slowly lower down the equipment prior dismantling the Installation / Erection tackles.

Then, start to install the bolts and nuts secure for SKID. After the SKID has been secured and hang, to install equipment protection (canvas) from the dust and debris.

Barricade and install warning sign board at this area.

11 SERVICE PLATFORM FOR VESSEL OR SKID INSTALLATION Prior to Installation / Erection, Contractor shall check the readiness of the structural in terms of service platform seating, dimensions, location and painting to ensure all the requirements in the drawing and specification are followed.

After installation / Erection the equipment shall be aligned and fixed to the foundation according to approved design and section 10 as above requirement. Piping connection to the skid or vessel only when the vessel or skid`s alignment is accepted by client.

12 SPECIAL REQUIREMENT Other special requirement (if any) shall be complied by CONTRACTOR before and during Installation / Erection. Such special requirement shall be well understood and practiced by all personnel involving in the lift.
