(3) ING/TRANSAMERICA, PREFERRED CHOICE FIXED ANNUITY CONTRACT 02TPC047479 IN THE AMOUNT OF $1,600,000.00, WITH ALL ACCUMULATED INTEREST THEREON; and

(4) ING/TRANSAMERICA, PERFERRED CHOICE FIXED ANNUITY CONTRACT 02TPC080685 IN THE AMOUNT OF $1,800,000.00, WITH ALL ACCUMULATED INTEREST THEREON

NOTICE OF VERIFIED CLAIM

COMES NOW Beaumont Independent School District (Beaumont ISD) and makes this

potential claim in the above-referenced matter.

On April 10, 2012, the United States of America filed a civil complaint seeking

forfeiture, pursuant to 18 U.S.C. 981, in the United States District Court for the Eastern District

of Texas against the following property:

a. Real property located at Tract 31A, Lots 31 & 32, LafittesLanding Phase Two, Port Arthur, Texas, with allappurtenances, improvements, and attachments thereon,more particularly described as:

Tract 31A of a Replat of Lots Numbered Thirty-one andThirty-two (31 & 32) of LAFITTES LANDING PHASETWO, a 106.97 acre subdivision out of and part of the N.Coleman Survey A-12 and the Dennis Gahagan League,Abstract No. 123, in the City of Port Arthur, JeffersonCounty, Texas, as the same appears upon the map or platthereof, on file and of record in Vol. 16 page 309, MapRecords of Jefferson County, Texas;

b. 2010 Mercedes S550V, 4 door, VIN:WDDNG7BB0AA288631 with all accessories and

modification thereto;

c. ING/Transamerica, a Preferred Choice Fixed AnnuityContract 02TPC047479 in the amount of $1,600,000purchased on September 21, 2009, using funds from the JPMorgan savings account number xxxxxx7080 of CalvinWalter [sic] and Stacy Walker;

d. ING/Transamerica, a Preferred Choice Fixed AnnuityContract 02TPC080685 in the amount of $1,800,000purchased on October 28, 2009, using funds from the JPMorgan savings account number xxxxxx7080 of CalvinWalter [sic] and Stacy Walker.

Beaumont ISD through its counsel received the Notice of Complaint for Forfeiture on

August 2, 2012.

Beaumont ISD assisted the United States Attorneys office in gathering information,

witnesses for prosecution of this matter. Beaumont ISD seeks to obtain payment for attorney

fees and costs associated with the attempted prosecution of Calvin Walker.

Beaumont ISD has incurred costs which can be calculated and proven.
