1. INNOVATIONS/ INVENTIONS IN CURRENT POSITION:

2004- Structured and fill-in format referral forms for use by Medical Officers from Pusat Perubatan Primer-HUKM dan Low Risk Birth Centre, Batu 14, Hulu Langat to Specialist Clinics of HUKM and other hospitals. Referrals were designed to accommodate various disciplines such as

• Cardiology

• Ophthalmology

• Urology

• Emergency O&G (PAC)

2008- Compiled a manual (fail meja) for Klinik Lanjutan Strok at Pusat Perubatan Primer UKM, Bandar Tasek Selatan. This document serves as a prototype to integrate long-term post stroke care at community level. This manual contains detailed instructions for each visit, screening tests for stroke complications as well as care algorithm for evidence-based stroke care. The contents of this manual is reviewed annually to keep abreast of current best practice recommendations. 2008- Initiated ‘Long-term Stroke Registry’ which lists information on patients receiving treatments from Klinik Lanjutan Strok di PPP-UKM, BTS. This registry was started to improve the database for the cohort of patients receiving long-term care at community level, especially those managed at primary care. This database will provide a reliable and invaluable source for research and audit-related activities to improve stroke care delivery services at community level. This registry to date, is the first in the nation to have access to long term stroke outcomes of patients managed at community level, and also serves as a prime location for collaborative transdisciplinary research projects from within and outside UKM. The ten year review of the registry will be due in 2018

July 2012-July 2014 The use of the Integrated Care Pathway for Post-stroke patients© (‘iCaPPS’©) as a standard operating procedure (SOP) for community stroke care delivery. The iCaPPS protocol is my PhD project to improve and adapt the stroke care delivery for use in public healthcentres of the Malaysian MOH. To justify its appropriateness as an option for delivery of structured post stroke care delivery, the study will include analysis of

impact on quality of life and cost effectiveness, compared to conventional care. Copyright and patentship status certification.
