What do we do?
• Assess and treat people with a mental or physical health problems in the community including learning disability
• Support carers• Make people aware of lifestyle issues such as
smoking, obesity• Provide care for long term conditions• Promote healthy living for children and adults

• Mostly on a 24 hours 7 days a week basis
• As much care as possible is provided at home or as close to home as possible
