Benefits Summary
For Benefits EffectiveOctober 1, 2013 September 30, 2014
Eligibility
Associates who are scheduled to work at least 30 hours per week and have also attained 60days of continuous service are eligible to participate in the programs described below. Theeffective date of participation will be the 1st of the month following attainment of 60 days ofcontinuous service. Associates scheduled to work less than 30 hours per week and Temporary
Associates are ineligible to participate in Mattress Firm benefit plans.
This Benefits Summary is an overview of the benefits plans that will be offered 1) during theOpen Enrollment period of September 7 September 22, 2013 and 2) to new Associates priorto their eligibility dates.
Dependents
Eligible dependents can enroll in Mattress Firms benefits plans; however, supportingdocumentation will be required to validate eligibility. Please contact the Benefits Department todetermine the required supporting documentation.
An eligible dependent includes:
1. Spouse (not fianc, but your legal spouse as determined by applicable law)2. Common Law Spouse3. Legal DependentAn Associates child up to age 26 (birth, adoption, or stepchild)4. Qualified Medical Child Support Order
Medical Insurance United Healthcare Choice Plus
Choice Plus is a PPO which provides for health care visits to a Primary Care Physician /Specialist and Emergency Room visits. Primary Care Physician / Specialist and EmergencyRoom visits.
Medical Insurance United Healthcare High Deductible Plan (New)
The High Deductible Health Plan provides low cost premiums for coverage involving health carevisits to a Primary Care Physician / Specialist and Emergency Room visits. This plan can beused in conjunction with an optional Health Savings Account.

Health Savings Account (HSA) United Healthcare (New)
The HSA is a tax-advantaged medical savings account that is available to Associates enrolled inthe Medical High Deductible Plan. Associates may contribute pre-tax dollars in their HSA topay for qualified medical expenses, such as deductibles, co-pays and co-insurance forthemselves and their covered dependents. There is also a $1,000 catch-up provision for
Associates age 55 and above. To assist Associates in achieving their medical savings goals,Mattress Firm will also contribute to their HSA. Any balance at the end of the year will becarried over to the following years.
Dental Care United Healthcare
Dental Care provides dental benefits including diagnostic and preventive care, covered at100%. Orthodontic Care is also included in this plan.
Vision Care United Healthcare
Vision Care provides for comprehensive eye exams, eye glasses and contacts (once every 12months). A deductible amount of $25 is required and members may choose from a diversifiednational provider network.
Flexible Spending Account (FSA) United Healthcare
Flexible Spending Account is available for qualified healthcare and dependent care expenses.Contributions to the FSA are deducted on a pre-tax basis and the maximum allowed amount is$2,500 for the Health Care Account and $5,000 for the Dependent Care Account. The FSAHealth Care and Dependent Care accounts are available to Associates enrolled in the Medical
Choice Plus Plan; whereas the FSA Dependent Care is available to Associates enrolled in theMedical High Deductible Plan.
Important Note: If you want to participate in the FSA, you must re-enroll each year.
Basic Term Life Insurance Sun Life
Basic Term Life insurance is provided at no cost to benefits eligible Associates, equal to 1 timestheir annual salary, up to $500,000.
Basic Accidental Death and Dismemberment Insurance
Sun Life
Basic Accidental Death and Dismemberment insurance is provided at no cost to benefits eligibleAssociates, equal to 1 times their annual salary, up to $500,000.

Voluntary Term Life Insurance Sun Life
In addition to Basic Life Insurance, Associates may choose optional Voluntary Life Insurance
coverage for themselves and their eligible dependents. Note: Due to a change in our HRIS,
you must re-enroll in the Voluntary Term Life and Voluntary Accidental Death and
Dismemberment plans during Open Enrollment, if you want to continue coverage.

Voluntary Accidental Death and Dismemberment Insurance Sun Life
In addition to Basic Accidental Death and Dismemberment Insurance, Associates may choose

Voluntary Term Life and Voluntary Accidental Death and Dismemberment during Open

Short Term Disability Sun Life
Short Term Disability provides income for Associates who are unable to perform their job due toa short term illness or disability. Mattress Firm pays 100% of the premium and benefits are paidat 60% of weekly salary for 11 weeks, after a 7 day elimination period (maximum benefit up to$2,000 per week).
Long Term Disability Sun Life
Long Term Disability provides income for Associates who are unable to perform their job due toa long term illness or disability. Mattress Firm pays 100% of the premium and benefits are paidat 60% of monthly salary, after a 90 day elimination period (maximum benefit up to $6,000 permonth).
401(k) One America
The 401(k) provides Associates the opportunity to save for retirement on a tax-advantagedbasis. In addition to the eligibility requirements described under Eligibility in this document,Associates must also be at least 20 years of age to participate in the 401(k). Associates maycontribute up to IRS limits of $17,500 for 2013. Mattress Firm will provide matchingcontributions of 33-1/3%, up to 6% of the Associates contributions. Vesting will occur at a rateof 20% over a period of 5 years of continuous service.
