Accidents are usually complex. Anaccident may have 10 or more eventsthat can be its causes. A detailedanalysis of an accident will normally

reveal three cause levels:basic, indirect, and direct.

At the lowest level, an accident

results only when a person or objectreceives an amount of energy orhazardous material that cannot beabsorbed safely. This energy or

hazardous material is the DIRECTCAUSE of the accident. The directcause is usually the result of one or

more unsafe acts or unsafeconditions, or both.

The unsafe acts and conditions arethe INDIRECT CAUSES.

Indirect causes are usually traceableto poor management policies and

decisions, or to personal orenvironmental factors. These are theBASIC CAUSES
