The blade-guide is too high!It should be adjusted to 1/8- 1/4” above the wood.This allows the guard to cover the blade, and the blade-guides to protect the blade from twisting.

Always sand on the downward side of the disc.

This is correct.

Please note that not all sanders turn the same direction.Always sand on the downward side.
