Athletic Eligibility Requirements

In accordance with the Arizona Interscholastic Association and the Deer Valley Unified School District eligibility rules, the following items must be on file in the athletic office prior to any athletic participation, including tryouts and/or practices:

1. Athletic Emergency Information Form (required each year)

This form includes acknowledgement for medical insurance. You may provide personal insurance information or you may obtain a school insurance form through the athletic office. By initialing and signing off on the Emergency Information form, you acknowledge that you have reviewed the Parent/Athlete Handbook located on the district’s website as well as reviewing the following forms with your athlete:

· Family and Coach Agreement form

· DVUSD Code of Conduct form

· Pursuing Victory with Honor Parent form

· Pursuing Victory with Honor Student form

2. Annual Physical Exam and Acknowledgment Form (required each year)

Physical exam must be performed after March 1 for the following year. All 4 pages of the AIA forms must be signed by a doctor. The forms also include acknowledgement of risk statements, includes medical history provided by the parent and must be signed by parent and athlete. Make sure you read and keep the information pamphlet attached to the packet.

3. AIA Mild Brain Injury/Concussion Statement and Acknowledgment Form (required each year)

Parent/Guardian and Student must read and acknowledge that they are an active participant in their own health and well being. This form summarizes the risks of concussions in sports.

7. Copy of your Birth Certificate (required only once in your school athletic career)

The athletic office must have a separate copy of your birth certificate. The athletic office does not have access to any copy you may have provided to the counseling office. This certificate must be certified by the appropriate state agency – hospital or religious certificates cannot be accepted.

8. AIA Brain Book – Concussion Education Module (required once at Barry Goldwater)

This is an online course at aiaonline.org. Click on the AIA Academy link and select Concussion Education – Brainbook. Register as a student, complete module, print the certificate and provide a copy to the athletic department.

Varsity Football:

Adults ~ $6.00

Senior Citizens (55+) ~ $5.00

Students ~ $5.00 with a School ID Card

All Other Sports including lower level Football, Basketball, Wrestling, Soccer, Volleyball, Baseball, Softball, Track, Swim:

Adults ~ $5.00

Senior Citizens (55+) ~ $5.00

Students ~ $4.00 with a School ID Card

Children under 10 are free and must be accompanied

by an adult to all events.

AIA Regional & State Events have a separate Admission Price Scale

Student Athletic Pass (Purchase in the Book Store) ~ $30

The student athletic pass is available to all current OHS students for the cost of $30. This pass allows the student to attend all home athletic contests. If a student attends only 8 athletic events during the course of the year, he or she will have paid for the entire cost of the pass. Just by attending the five home Varsity Football games, the student will have paid for over 83% of the cost of the pass ~ what a great deal! Please note, however, that regional, state and school invitational contests are not included in the Student Athletic Pass.

Family “20 for $60” Pass (Purchase in the Book Store or Home Games) ~ $60

The OHS Athletic Office is pleased to offer your family another great value with the “20 for $60” family athletic pass. We encourage and support family participation in all of our athletic events and this pass will allow 20 people to attend home athletic contests for only $60 ~ this comes out to only $3.00 per person ~ an exceptional value.

Upon receipt of the $60, a pass will be issued to the purchaser. This pass will include 20 numbers located on the edge of the card. For every person who enters an athletic event, one of the numbers will be punched at the admission gate. This will allow an entire family, friends, etc… to attend an event whenever they choose. This pass can be used in conjunction with the Student Athletic Pass for the maximum savings possible.

These passes will be available in the bookstore and at home games throughout the year. Please note, however, that regional, state and school invitational contests are not included in the Student Athletic Pass.
