2. Job Purpose/Main Duties/Responsibilities 

The post-holder will plan and lead in the delivery of care and rehabilitation programmes working incollaboration with patients, their carers and the multidisciplinary team within the specialistrehabilitation services; this will include the assessment, planning, implementation, evaluation andsetting of individualized patient goals. 1  To perform all aspects of the role in accordance with the policies established byStaffordshire and Stoke on Trent Partnership Trust

2   Act at all times in accordance with the NMC Code of Professional Conduct andassociated guidance

3  Direct the delivery of quality patient care by participating in the assessment, planning,

8  Ensure the maintenance and security of all equipment belonging to SSOTP9  Use of all available opportunities to further develop clinical leadership10  In conjunction with the Ward Sister and Senior Clinical Nurses / Multidisciplinary

Patient, client and staff safety is paramount. You will promote a just and open culture toreporting of incidents and adverse events. In accordance with the Risk ManagementStrategy, you will participate, whenever required, with the risk management process. You willsupport line managers by attending mandatory and statutory training, completingincident/accident forms for every adverse event or near miss that occurs, report all defectsand complaints, and communicate any dangerous situation of individuals potentially at risk.

You will promote a risk management culture within your working environment and ensuringparticipation and involvement of all staff, in line with the Partnership Trust’s risk managementstrategy and policies.

You will identify potential risks that may impact on the Partnership Trusts ability to achieveits objectives, including details of risk likelihood, severity and impact, and record thesedetails in the appropriate system and on the Partnership Trust Risk register. 

Health and Safety at WorkHealth & Safety is the responsibility of all staff and you are required to take due care at work,

report any accidents or untoward occurrences and comply with the Partnership Trust Health& Safety Policy in order that it can fulfil its Health and Safety responsibilities.

You should be aware of current health and safety policies of the Partnership Trust andattend all mandatory health and safety training. You will maintain a safe workingenvironment for patients, clients, visitors and employees. You must not willingly endangeryourself or others whilst at work and safe working practices and safety precautions must beadhered to. Protective clothing and equipment must be used where appropriate.

You will comply with regular Health & Safety inspections with the Health and Safety Managerto ensure compliance with all current legislation. You must co-operate with those in authorityand others in meeting the statutory requirements and in following policies and procedures.

You are reminded that in accordance with the Health and Safety at Work Act 1974 you havea duty to take reasonable care to avoid injury to yourself and to others affected by your workactivities.

Infection Prevention and Control In accordance with the Health and Social Care Act 2010, you will actively participate in theprevention and control within the capacity of your role and act as a role model to all staff.The Act requires you to attend Infection Prevention and Control training on induction, regularupdates as required in the Partnership Trust Training needs analysis policy and to takeresponsibility for the practical application of the training in the course of your work. Infectionprevention and control must be included in any personal development plan or appraisal.

Safeguarding of Children & Young PeopleThe Partnership Trust is committed to the Safeguarding of Children and Young People andhas signed up to the Safeguarding Children Board Policy & Procedures to which you mustbe familiar with and adhere to. It is your responsibility to attend mandatory training withregard to child protection as per the Partnership Trust’s Policy and Guidance. 

Safeguarding Vulnerable AdultsIt is the responsibility of all employees to safeguard vulnerable adults and to attendmandatory training. You must adhere to the Partnership Trust’s Adult safeguarding policiesand also Staffordshire & Stoke on Trent Interagency Adult Protection Procedures.

Alcohol & DrugsEven small amounts of alcohol can impair work performance and affect the ability to dealwith patients/clients and the public in a proper and acceptable manner. Consumption of

alcohol during work hours in not permitted, neither is attending work already under theinfluence of alcohol or other substances. 

Smoke Free Environment All employees are required as a condition of their employment to be non-smoking whilst onduty. During any unpaid breaks in duty, such as the formal lunch break or a split shift, staffwill be free to smoke but only provided the following conditions are observed:

Smoking takes place away from the Partnership Trust premises, grounds andimmediate environment; it is not acceptable for staff to congregate in groups tosmoke out Partnership Trust premises and smoking will not be tolerated atentrances/exits to the Partnership Trust sites. Smoking “breaks” are not permittedunder any circumstances i.e. smokers will not receive additional breaks to allow themto smoke.

 Any members of staff smoking in any location must not be identifiable as NHSemployees. Employees wishing to smoke should ensure their uniforms are fullycovered in all seasons.

Smokers must give sufficient time for smoke smells/smells of tobacco to dissipatebefore returning to direct patient care.

Staff who wish to quit smoking will be supported and encouraged to seek advice andassistance in doing so in order to comply with the requirements of this policy.

Counter FraudYou are expected to comply with Partnership Trust counter fraud policies and proceduresand to note the obligation, where appropriate, to inform and seek advice from the LocalCounter Fraud Specialist.

Employees are expected to act in accordance with the standards laid down by theirProfessional Organisations and statutory standards where applicable and are expected to:-

Ensure that the interest of patients/clients remains paramount at all times.Be impartial and honest in the conduct of their official business.Use the public funds entrusted to them to the best advantage of the service, always ensuringvalue for money.

Ensure that they do not seek to advantage or further private business orother interest, in the course of their official duties.
